Alpha-fetoprotein is a well-known marker of hepatocellular carcinoma. Alpha-fetoprotein-producing intrahepatic cholangiocarcinoma is rare. This report detailed our experience with such a case and reviewed the relevant literature. BACKGROUND: HCC patients are heterogeneous in terms of both tumor and liver factors. Alpha-fetoprotein (AFP) is an important prognostic tumor marker for those patients with elevated AFP levels.
AIMS: To examine the differences in HCC patients with high or low AFP levels in blood and evaluate the prognostic parameters in low AFP patients.

The use of maternal serum alpha-fetoprotein (MSAFP) in screening for open neural tube defects and other fetal abnormalities is discussed. The arguments for the introduction of this screening test in certain areas of Iceland are provided and its possible application. A case of cancer of the ampulla of Vater producing alpha-fetoprotein is reported and the literature is reviewed. As far as we know, the literature contains no reports of such a case. The preoperative serum alpha-fetoprotein was remarkably high (7,480 ng/ml), but it fell to normal values (under 5.0 ng/ml) after pancreatoduodenectomy, by means of which the lesion could be radically resected. In this case, the immunohistochemical findings at optical and electron microscopy clearly indicated the production of alpha-fetoprotein by the cancer cells of the ampulla of Vater.

Reports of alpha-fetoprotein-producing lung tumors are rare. Only 24 such patients with these tumors have been previously studied. We report the case of a patient with a large cell carcinoma of the lung and a serum alpha-fetoprotein level of 9,300 ng/mL, with no evidence of hepatic or other systemic abnormalities. Serum levels of alpha-fetoprotein returned to normal postoperatively.

Alpha-fetoprotein (AFP) is a commonly used prenatal screening test for congenital anomalies. However, when anomalies are excluded after high resolution ultrasound and/or amniocentesis, an elevated maternal serum AFP (MSAFP) has been found to be associated with a 2- to 4-fold increase in low birthweight resulting from both preterm delivery and intrauterine growth retardation. Increased AFP levels in patients with hepatocellular carcinoma are mainly related to tumor size and in a lesser degree, to AST levels. Abnormal and/or diagnostic AFP levels will be observed in a reduced proportion of patients with small HCC (less than 5 cm). We evaluated a two-site enhanced luminescence immunoenzymometric assay (Amerlite; Amersham International) for alpha-fetoprotein (AFP) in maternal serum and amniotic fluid. The assay is rapid, involving two incubations totalling 4 h. The working range of the assay for serum AFP is 5.5 to 750 kilo-int. units/L (CV less than 10%), with a sensitivity of detection of 0.2 kilo-int. unit/L.
