Vertical translocation. Part II. Outcomes after surgical treatment of rheumatoid cervical myelopathy.

This is a prospective observational study in 116 patients with rheumatoid arthritis and vertical translocation who underwent cervical spine surgery after developing symptomatic myelopathy. These patients, whose mean age was 62 years, had suffered from rheumatoid arthritis for almost 25 years. Surgery was performed via a combination of anterior (67 transoral decompressions) and posterior approaches. Surgical morbidity was recorded in 39% of patients, with a 30-day mortality rate of 10.3%, which was largely related to poor preoperative neurological grade. Neurological improvement of at least one

Preoperative radiographic factors and surgeon experience are associated with cortical breach of C2 pedicle screws.

STUDY DESIGN: A retrospective review study. OBJECTIVE: In this study, we attempt to identify radiographic variables associated with likelihood of intraoperative breach during C2 pedicle screw placement. In addition, we attempt to correlate surgeon experience with breach rate. SUMMARY OF BACKGROUND DATA: Pedicle screws have emerged as an effective approach for obtaining fixation of the axis, yet placement of C2 pedicle screws is technically demanding and poses the risk of injury to the vertebral artery. Given the evidence for substantial variation in C2 anatomy, preoperative assessment of com

Reproducibility of sagittal radiographic parameters in adolescent idiopathic scoliosis-a guide to reference values using serial imaging.

BACKGROUND CONTEXT: Knowledge of sagittal radiographic parameters in adolescent idiopathic scoliosis (AIS) patients has not yet caught up with our understanding of their roles in patients with adult spinal deformity. It is likely that more emphasis will be placed in restoring sagittal parameters for AIS patients in the future. Therefore, we need to understand how these parameters may vary in AIS to facilitate management plans. PURPOSE: This study aimed to determine the reproducibility of sagittal spinal parameters on lateral film radiographs in patients with AIS. STUDY DESIGN/SETTING: This w

Spectrum of occipitoatlantoaxial injury in young children.

OBJECT: Injuries of the occipitoatlantoaxial (Oc-C2) region are the predominant form of cervical injury in children younger than 10 years of age. Magnetic resonance (MR) imaging can be used to visualize directly the traumatic ligamentous and soft-tissue abnormalities of the Oc-C2 region. A retrospective review was undertaken to examine the spectrum of pediatric Oc-C2 injuries seen on MR imaging, their correlation with plain x-ray film and computerized tomography findings, and their clinical course. METHODS: Seventy-one consecutive children younger than 10 years of age underwent cervical MR im

Comparison of 3D and 2D characterization of spinal geometry from biplanar X-rays: a large cohort study.

BACKGROUND: Biplanar X-ray system providing anteroposterior and sagittal plane with an ultra-low radiation dose and in weight-bearing position is increasingly used for spine imaging. The original three-dimensional (3D) reconstruction method from biplanar X-rays has been widely used for clinical parameters, however, the main issue is that manual adjustments of the 3D model was quite time-consuming and limited to thoracolumbar spine. A quasi-automated 3D reconstruction method of the spine from cervical vertebra to pelvis was proposed, which proved fast and accurate in 57 patients with adolescent

Normalized vertebral-level specific range of motion corridors for female spines in rear impact.

OBJECTIVE: It is well known that the biomechanical responses of female and male spines are different in rear impacts. Female-specific finite element models are being developed as improvements over generic models. Such advancements need female-specific segmental responses for validation. The objectives of the study were to develop vertebral level-specific range of motion corridors from female human cadaver head-neck complexes exposed to rear impact loading. METHODS: Previously conducted experiments from five human cadaver head-neck complexes were used in this analysis-based study. Briefly, the

Stabilization of metastatic lesions affecting the second cervical vertebra.

PurposeMetastatic spine disease is an ever-increasing burden on health care systems. Certain levels in the spine confer unique biomechanical characteristics and hence are of interest. Isolated C2 lesions are rare. We aimed to review our results in surgical management of C2 lesions.Materials and methodsWe reviewed all surgical stabilizations of metastatic spine lesions over the preceding 4 years. Six patients with C2 lesions were identified. Of these five underwent surgical stabilization primarily for disease affecting the second cervical vertebra. Case notes and radiology were reviewed to dete

Imaging Evaluation of Bone Tumors in the Cervical Spine: A Comprehensive Review.

The cervical spine, a critical junction between the head and torso, is a rare but significant site for both primary and metastatic tumors. While primary tumors of the cervical spine are uncommon, certain types, such as chordomas and giant cell tumors, are particularly notable for their potential to affect this region. Metastatic lesions, although more frequent, present unique diagnostic and therapeutic challenges due to the complex anatomy of the cervical spine. Imaging is indispensable for the evaluation of cervical spine tumors, serving as the foundation for diagnosis, treatment planning, an

Posterior occiput-cervical fixation for metastasis to upper cervical spine.

BACKGROUND: Metastasis to craniocervical area may result in instability manifesting as disabling pain, cranial nerve dysfunction, paralysis, or even death. Stabilization is required to prevent complications. Nonoperative treatment modalities are ineffective in providing stability and adequate pain relief. We present our experience of diagnosis, presentation, and surgical management for metastatic tumors to the upper cervical spine (UCS). METHODS: Single-center single-surgeon database of consecutively operated posterior occiput-cervical fusion for metastasis to UCS was reviewed from 2007 to 20

Craniovertebral Junction Instability after Oncological Resection: A Narrative Review.

The craniovertebral junction (CVJ) is a complex transition area between the skull and cervical spine. Pathologies such as chordoma, chondrosarcoma and aneurysmal bone cysts may be encountered in this anatomical area and may predispose individuals to joint instability. An adequate clinical and radiological assessment is mandatory to predict any postoperative instability and the need for fixation. There is no common consensus on the need for, timing and setting of craniovertebral fixation techniques after a craniovertebral oncological surgery. The aim of the present review is to summarize the an
