Ischioplasty and Secondary Proximal Hamstring Repair for Ischiofemoral Impingement Syndrome: A Solution to a Deep-Seated Pain.

BACKGROUND: Ischiofemoral impingement (IFI) syndrome is the narrowing of the space between the lateral aspect of the ischium and the medial aspect of the lesser trochanter. IFI is a frequently unrecognized generator of vague or posterior hip pain that is commonly preceded by trauma or previous hip surgery. IFI can also be a result of atypical pelvic/proximal femur anatomy, adductor/abductor imbalances, or ischial tuberosity enthesopathies. Management typically consists of conservative treatments, including activity modification, anti-inflammatories, physical therapy, and corticosteroid injecti

Bromocriptine in rheumatic and autoimmune diseases.

BACKGROUND AND OBJECTIVES: Multiple lines of evidence support the concept that the anterior pituitary hormone prolactin has a pathogenic role in rheumatic and autoimmune diseases including, but not limited to, rheumatoid arthritis (RA), systemic lupus erythematosus (SLE), Reiter's syndrome, psoriatic arthritis, and uveitis. Conversely, the dopaminergic agonist bromocriptine appears to have therapeutic effects through suppression of pituitary prolactin secretion and, perhaps, through actions on peripheral dopamine receptors. This article reviews the experimental and clinical data supporting the

The ankle in XLH: Reduced motion, power and quality of life.

BACKGROUND: X-linked hypophosphatemia (OMIM 307800) is a rare bone disease caused by a phosphate-wasting condition with lifelong clinical consequences. Those affected suffer from bone pain, complex skeletal deformities, impaired mobility and a reduced quality of life. Early osteoarthritis and reduced range of motion of the lower limbs are known pathologies in XLH patients. However, XLH-specific data on the affected compartments such as the ankle joint through the evaluation of radiographic and gait analysis data is still lacking. PATIENTS AND METHODS: In this cross-sectional study, patients w

Sexuality in spondyloarthritis--the impact of the disease.

INTRODUCTION: The impact of rheumatic diseases on patients' sexual life has been gathering the attention of the scientific community over the last decade. The existing studies are scarce, especially related to spondyloarthritis, and particularly to psoriatic arthritis. Several factors associated with the disease may condition sexual function: pain, stiffness, decreased range of motion, joint swelling and extraarticular features such as fatigue, enthesopathy and cutaneous lesions in psoriatic arthritis. OBJECTIVES: To assess sexual satisfaction and limitations in sexual activity in a cohort of

Evaluation of a quantitative scoring of enthesitis in ankylosing spondylitis.

BACKGROUND: Enthesitis is inflammation at the insertion of ligaments, tendons, joint capsule, or fascia to bone, and a well-known characteristic feature of ankylosing spondylitis (AS) and related spondyloarthropathies. The clinical evaluation of enthesitis is an important outcome measure and is scored by applying pressure on entheses to elicit tenderness at these sites. OBJECTIVE: This study assessed the validity of an enthesitis index calculated by algometric pressure pain threshold scoring in comparison with digital palpation scoring and intra- and interexaminer reliability of 2 grading met

Laboratory findings in psoriatic arthritis.

Psoriatic arthritis (PsA) has been classically defined as an inflammatory arthritis associated with psoriasis. However, in comparison with other relevant inflammatory arthropathies, in which a definite diagnosis is frequently possible only by means of laboratory investigations, in PsA true laboratory diagnostic markers are lacking. Some markers are utilised more to differentiate other diseases than to characterise PsA. For example in polyarticular PsA, which may be in some cases indistinguishable from RA, the rheumatoid factor (RF) or the more specific and recently introduced antibodies to cyc

Albuminocytologic Dissociation and Intravenous Immunoglobulin Therapy in Parsonage-Turner Syndrome With Bilateral Involvement: A Case Report.

Parsonage-Turner syndrome (PTS) is a rare neurological disorder characterized by acute neuropathic pain followed by motor and sensory deficits, typically affecting the brachial plexus. While often self-limiting, atypical presentations can complicate diagnosis and management. We present a case of a 53-year-old male patient with a history of cervical foraminal stenosis and progressive left upper extremity (LUE) flaccid paralysis for over 14 months, with no clear cause for worsening symptoms. Diagnostic evaluation, including magnetic resonance imaging (MRI) and computed tomography (CT) of the bra

Evidence in support of the bone marrow as the primary lesion in axial spondyloarthritis.

PURPOSE OF REVIEW: Over the past several decades, the concept that the primary lesion accounting for the development of axSpA is an enthesopathy has been widely accepted. However, the hallmark abnormality of axSpA occurs in the sacroiliac joint at the interface of cartilage and bone at a location remote from any anatomical enthesis. Both imaging and histopathological data from the sacroiliac joint point to immunopathogenetic events in the bone marrow as being of primary importance. Here, we discuss new developments in our understanding of immune events in the bone marrow relevant to axSpA that

Three-dimensional ultrashort echo time (3D UTE) MRI of Achilles tendon at 4.7T MRI with comparison to conventional sequences in an experimental murine model of spondyloarthropathy.

BACKGROUND: Due to the very short T2 of its components, the normal anatomy of Achilles enthesis is impossible to define with "conventional" long echo time (TE) T2 sequences. However, this is a common site affected by rheumatologic disease. Early abnormalities related to inflammatory processes are impossible to detect in this location. PURPOSE: To assess the feasibility of a 3D-UTE (ultrashort echo time) sequence to evaluate normal and pathological Achilles entheses, determining both anterior fibrocartilaginous and posterior collagenic portions at 4.7T, in a rat model of spondyloarthropathy (S

Knee impingement syndromes.

INTRODUCTION: The so-called knee impingement syndromes are very frequently reported in both professional and amateur sportsmen. PURPOSE: The objective of our study was to classify the most frequent knee changes responsible for such syndromes considering both pathology and diagnostic work-up. MATERIAL AND METHODS: Our patients complained of aspecific symptoms related to articular meniscal, ligament or cartilage, conditions. The site of pain was periarticular and there was no apparent sign of acute traumatic events. All individuals, aged 16-55, practised sports at different levels and women we

A rational management of tennis elbow.

Tennis elbow is due to a torque injury or sudden overstretching of tendons which insert into the epicondyles of the humerus. The predominant lesion is an enthesopathy--a pathological lesion at the insertion of tendon into bone. The most common site is at the lateral epicondyle and this is 3 times as frequent as at the medial epicondyle. Approximately 50% of tennis players can expect to get a tennis elbow at some time during their playing lifetime. In one-third of the players this will be severe enough to interfere with their tasks of daily living. The major unresolved question about the aetiol
