An Orthodontic Approach for Garre's Sclerosing Osteomyelitis of the Mandible.

The nonsuppurative osteomyelitis of the mandible is a rare condition that can occur in children due to low-grade inflammatory processes, dental cavities, periodontal lesions as well as the eruption process of the teeth. We submit a case report involving the orthodontic management of a 9-year-old female patient who presented in our service in the mixed dentition period with diagnosed Garre's sclerosing osteomyelitis of the entire mandibular body. After a full work-up, the following symptoms and signs were noted: bilateral temporomandibular joint (TMJ) pain, loss of the leeway space, anterior op

Principal direction of inertia for 3D trajectories from patient-specific TMJ movement.

Accurate simulation and evaluation of mandibular movement is fundamental for the analysis of functional changes and effects of the mandible and maxilla before and after surgical treatments. We applied principal axes of inertia to the three-dimensional (3D) trajectories generated by patient-specific simulations of TMJ movements for the functional evaluations of mandible movement. Three-dimensional movements of the mandible and the maxilla were tracked based on a patient-specific splint and an optical tracking system. The dental occlusion recorded on the sprint provided synchronization for initi

Comparison of device-supported sensorimotor training and splint intervention for myofascial temporomandibular disorder pain patients.

This study was to compare the short-term therapeutic efficacy of device-supported sensorimotor training with that of standard splint therapy for patients with myofascial temporomandibular disorder (TMD) pain over a treatment period of 3 months. We tested the hypothesis that both types of intervention are equally effective for pain reduction. In addition, the electromyographic (EMG) activity of the temporal and masseter muscles was recorded under conditions of force-controlled submaximum and maximum biting in intercuspation. Of consecutive patients seeking treatment for non-odontogenic oro-faci

Negative association between self-reported jaw symptoms and apnea-hypopnea index in patients with symptoms of obstructive sleep apnea syndrome: a pilot study.

OBJECTIVES: Prior to oral appliance therapy for snoring and obstructive sleep apnea syndrome (OSAS), patients are screened for jaw symptoms (e.g., pain). However, the presence of jaw symptoms in a large spectrum of OSAS patients remains unknown. This study aimed to assess the distribution of subjective jaw symptoms in patients with symptoms of OSAS. METHODS: Five hundred and eleven consecutive patients (66 female, 445 male; mean age 49.6 ± 12.6 years) with clinical symptoms of OSAS were enrolled for cardiorespiratory evaluation. Self-administered questionnaires were used to assess jaw symptom

Long-term efficacy of resilient appliance therapy in TMD pain patients: a randomised, controlled trial.

The aim was to investigate long-term efficacy of a resilient appliance in patients with pain due to temporomandibular disorders (TMD). A randomised, controlled trial was performed in 80 recruited TMD pain patients. They were randomly allocated to one of two groups: treatment with a resilient appliance or treatment with a hard, palatal, non-occluding appliance. The primary treatment outcome was judged positive when patients' characteristic pain intensity decreased by at least 30%. Additional treatment outcomes were physical functioning, emotional functioning and headache. At the 12-month follow

Oromandibular dystonia treatment following a loss of vertical dimension.

UNLABELLED: Oromandibular dystonia is a rare condition of hyperkinetic motor dysfunction in the orofacial area. It is a centrally mediated disorder of unknown aetiology and manifested by a sustained contraction of masticatory muscles, involving one or both lateral pterygoids, and pushing the mandible forward. The prognosis is poor and the treatment is symptomatic. This paper describes a case of a 38-year-old woman referred for a locked left TMJ who was diagnosed as having an oromandibular dystonia. A step-by-step diagnosis and treatment procedure is described. CLINICAL RELEVANCE: Knowledge of

A dual-laminate snore reduction appliance: a report of four cases.

This article describes a snore reduction appliance that can be constructed to advance and hold the mandible in a comfortable prognathic position. The tongue is advanced concomitantly, producing more space in the pharynx while changing the relative positions of the soft palate, posterior tongue, and pharyngeal walls to reduce the incidence of snoring and ameliorate sleep apnea. A dual-laminate appliance, soft inside and hard outside, is vacuformfitted to casts of the maxillary and mandibular teeth and luted in a protrusive relation with cold pressure-cured acrylic. The appliance maintains the m

A systematic review of continuous positive airway pressure for obstructive sleep apnoea-hypopnoea syndrome.

We conducted a systematic review of current evidence on the effectiveness of continuous positive airway pressure (CPAP) for treatment of obstructive sleep apnoea-hypopnoea syndrome (OSAHS). The primary outcomes were subjective sleepiness, using Epworth Sleepiness Scale (ESS) and objective sleepiness using Maintenance of Wakefulness Test (MWT) and Multiple Sleep Latency Test (MSLT). Mean difference (MD) in endpoints was used to compare CPAP to usual care, placebo and dental devices. The analysis was stratified by symptom and disease severity at baseline. CPAP significantly reduced ESS score com

Imaging for the snoring and sleep apnea patient.

Upper airway imaging techniques have significantly advanced the understanding of the pathogenesis of obstructive sleep apnea and the biomechanical mechanisms by which therapeutic interventions for this disorder exert their effects. Both static and dynamic imaging studies have been used to examine the structure and function of the upper airway during wakefulness and sleep. These studies have highlighted the importance of the lateral pharyngeal walls in addition to the tongue and soft palate in modulating changes in upper airway caliber. Upper airway imaging has also been used to understand the

Thirty-year follow-up of a TMD case treated based on the neuromuscular concept.

AIM: Occlusal therapy is employed to alleviate the symptoms of a temporomandibular disorder (TMD) at times. However, the long-term effect of occlusal therapy in the masticatory system is not well understood. This case study aims to present a 30-year follow-up of a TMD case. METHODOLOGY: The patient developed TMD with intermittent closed lock of the left temporomandibular joint (TMJ). Chief complaints included trismus, pain, and noise of the left TMJ during function. The patient's occlusal disharmony was assessed with use of electronic instruments and corrected based on the neuromuscular conce

Migraine and tension-type headache reduction through pericranial muscular suppression: a preliminary report.

Migraine and tension-type headaches have always plagued mankind. In spite of all the research dollars spent trying to determine the etiologies of these headaches, the neurology community still has not established a known cause of migraine and tension type headaches. This paper describes a study that was conducted for the U.S. Food and Drug Administration in which the efficacy of the Nociceptive Trigeminal Inhibition Tension Suppression System was evaluated and proved safe and efficacious in the reduction of medically diagnosed migraine and tension-type headache.

Clinical audit of subjects with snoring & sleep apnoea/hypopnoea syndrome fitted with mandibular repositioning splint.

Snoring and obstructive sleep apnoea/hypopnoea syndrome (OSAHS) are often treated with mandibular repositioning splints (MRS), but the efficacy and satisfaction of them has not been comprehensively addressed. A survey on the use of and satisfaction with MRS was posted to 177 patients referred by a hospital orthodontic department for custom-fitting of a MRS. Data were analysed using non-parametric techniques. The response rate was 81% (n=144). Responders (30F, 114M) had mean (SD) age of 51 (11) years, apnoea+hypopnoea index (AHI) of 24 (21) per hr and Epworth Score of 10 (5) at diagnosis, and h
