Are suspensory ligaments important for middle ear reconstruction?

As the resolution of 3D printing techniques improves, the possibility of individualized, 3-ossicle constructions adds a new dimension to middle ear prostheses. In order to optimize these designs, it is essential to understand how the ossicles and ligaments work together to transmit sound, and thus how ligaments should be replicated in a middle ear reconstruction. The middle ear ligaments are thought to play a significant role in maintaining the position of the ossicles and constraining axis of rotation. Paradoxically, investigations of the role of ligaments to date have shown very little impac

Use of the flexible fiber CO2 laser in pediatric transcanal endoscopic middle ear surgery.

We describe 4 pediatric patients (age 6-11 years) who underwent transcanal endoscopic ear surgery (TEES) with the assistance of a flexible fiber CO2 laser over a period of 6 months. Three of these individuals suffered from densely adherent cholesteatoma, where the laser permitted one-handed dissection while preserving endoscopic visualization by limiting bleeding. In the fourth patient, TEES ossiculoplasty was performed for a congenital stapes bar, with subsequent hearing improvement. Advantages and disadvantages of the flexible fiber CO2 laser in the setting of TEES are discussed. Use of the

Comparison of auditory responses determined by acoustic stimulation and by mechanical round window stimulation at equivalent stapes velocities.

Active middle ear implants (AMEIs) have been studied to overcome the limitations of conventional hearing aids such as howling, occlusion, and social discrimination. AMEIs usually drive the oval window (OW) by means of transmitting vibrational force through the ossicles and the vibrational force corresponding to sound is generated from a mechanical actuator. Recently, round window (RW) stimulation using an AMEI such as a floating mass transducer (FMT) to deliver sound to the cochlea has been introduced and hearing improvement in clinical use has been reported. Although previous studies demonstr

The mechanism of direct stimulation of the cochlea by vibrating the round window.

BACKGROUND: Active middle ear implants such as the vibrant sound bridge (VSB) have been placed on the round window (RW) in patients with conductive or mixed hearing loss, with satisfactory hearing results. Several observations show that the mechanism of RW stimulation is not completely understood. The purpose of the present study was to compare different coupling procedures between the transducer and the RW in order to contribute to an understanding of the mechanism of RW stimulation. METHODS: Five fat sand rats underwent ablation of the left ear and opening of the right bulla, followed by ba

An easy method to determine crucial AMEI performance parameters from clinical routine data in individuals - Part 1: maximum output.

OBJECTIVE: The frequency specific maximum output (MO) of active middle ear implants is the most crucial parameter for speech intelligibility. We determined individual MO from clinical routine data in round window (RW) coupling of the Vibrant Soundbridge (VSB). DESIGN: Monocentric, retrospective analysis. STUDY SAMPLE: 68 ears implanted with the VSB at the RW were analysed. Using bone conduction and direct threshold, MO was determined for combinations of implants (VORP502, VORP503) and processors (Samba, Amadé). Coupling modes were: (A) without coupler (N = 28), (B) spherical coupler (N = 19)

The Merits of Bilateral Application of Middle Ear Implants in Patients With Bilateral Conductive and/or Mixed Hearing Loss.

This study investigated sound localization abilities in patients with bilateral conductive and/or mixed hearing loss (BCHL) when listening with either one or two middle ear implants (MEIs). Sound localization was measured by asking patients to point as quickly and accurately as possible with a head-mounted LED in the perceived sound direction. Loudspeakers, positioned around the listener within a range of +73°/-73° in the horizontal plane, were not visible to the patients. Broadband (500 Hz-20 kHz) noise bursts (150 ms), roved over a 20-dB range in 10 dB steps was presented. MEIs stimulate the

Laser stapedotomy: a comparative study of prostheses and seals.

During the past 13 years, a number of prostheses of differing design and tissue seals have been used in laser stapedotomy for otosclerosis. This study compares the results of three different configurations of prostheses and tissue seals in a series of 53 patients. In 19, a platinum wire Teflon piston was placed in the laser stapedotomy fenestra and crimped on the long process of the incus; autologous venous blood was infiltrated into the oval window niche as a sealing mechanism. In 8 patients, a stainless steel bucket-handle-type prosthesis was used with a blood tissue seal. In 26 patients, a

[The results of operational otosclerosis treatment after stapedotomy].

INTRODUCTION: There are many ways of presenting the treatment's results for otosclerosis. Because of loss of the uniform standard, it is impossible to compare these results between various health centres. But the most important is, which way of presenting seems to be the best. AIM: The aim of the study was to propose a scheme of presenting the treatment's results for otosclerosis. MATERIAL AND METHODS: In order to devise method, medical documentation of 81 patients, treated for otosclerosis in Otolaryngological and Oncological Laryngology Clinic with Audiology and Phoniatrics Department, was

The expanding domain of implantable hearing devices: an update on current status in the United States.

Several implantable devices are now approved in the United States for rehabilitation of conductive, mixed, and sensorineural hearing loss. This paper will review current rehabilitative options including middle ear implants, used for mild to severe sloping sensorineural loss, and bone-anchored hearing aids, for conductive or mixed hearing loss. The indications for these devices differ, as do the bases for exclusion. Audiologic evaluation parameters will be reviewed as a guideline to selection, and as a baseline to determine potential rehabilitative impact. We will also describe emerging applica

Surgery for congenital stapes ankylosis with an associated congenital ossicular chain anomaly.

The surgical findings and results are presented on 32 ears with congenital stapes ankylosis with an associated congenital anomaly of the ossicular chain. One third of the patients had a syndromal diagnosis. In 26 ears, stapedectomy could be performed. In 2 other ears, stapes ankylosis to the bony facial canal was mobilized successfully. In the 4 remaining ears, surgical intervention had to be limited to an exploratory tympanotomy for various reasons. The average hearing gain was 23 dB for the 28 ears on which stapes surgery had been performed. A substantial hearing gain of at least 15 dB was a
