Progressive calvarial and upper cervical pneumatization associated with habitual valsalva maneuver in a 70-year-old man.

A 70-year old man with a 15-year-history of chronic daily Valsalva maneuvers for left ear congestion presented with worsening vertigo and calvarial (occipitoparietal) and upper cervical hyperpneumatization. With continued frequent Valsalva maneuvers, subsequent studies demonstrated increased pneumatization with extension of air into the epidural space, causing mass effect on the left parietal lobe. Four months after discontinuing the habitual Valsalva maneuvers, CT demonstrated resorption of the epidural air and partial regression of the calvarial pneumatization.

Occurrence of postoperative pneumoencephalus in posterior fossa surgery using the semi-sitting position: Prevalence, factors and patterns.

BACKGROUND: Posterior fossa (PF) surgery in the semi-sitting position (SSP) offers benefits for the patient, the anesthesiologist and the surgeon. However, in neurosurgery, neuroanesthesiology and related fields, a certain paradigm has been established regarding the potential high risk of this position, including the possibility of developing postoperative pneumoencephalus (PPN). The objective of the present work is to assess the prevalence, factors, and patterns related with PPN in SSP in a heterogeneous group of patients with PF conditions. METHODS: Observational, descriptive, cross-section

Skull thickening, paranasal sinus expansion, and sella turcica shrinkage from chronic intracranial hypotension.

In children or young adults, the morphology of the skull can be altered by excessive drainage of CSF following placement of a ventriculoperitoneal (VP) shunt. In Sunken Eyes, Sagging Brain Syndrome, gradual enlargement of the orbital cavity occurs from low or negative intracranial pressure (ICP), leading to progressive bilateral enophthalmos. The authors report several heretofore unrecognized manifestations of this syndrome, which developed in a 29-year-old man with a history of VP shunt placement following a traumatic brain injury at the age of 9 years. Magnetic resonance imaging showed typic

Pneumatization degree of the anterior clinoid process: a new classification.

The objective of this study is to determine the incidence and degree of anterior clinoid process pneumatization, in addition highlighting to their clinical significance. Multidetector-row CT scans of the skull base were reviewed in 648 subjects between 2007 and 2008. The presence of pneumatized anterior clinoid process and its degree were studied and documented. These data were statistically analyzed. Pneumatization of the ACP was found in 62 of 648 patients (9.6%) including 32 (51.6%) men and 30 (48.4%) women. The age of these patients ranged from 21 to 82 years (mean, 41 +/- 15.7 years). Pne

Skull base CSF leak repair in the outpatient Arena: Pushing the boundaries of modern endoscopic surgery.

BACKGROUND: Skull base defects with cerebrospinal fluid (CSF) leak have been a significant clinical challenge due to the risk of life-threatening complications such as meningitis, pneumocephalus and intracranial abscess formation. Traditionally, these defects have been managed in the operating room under general anesthesia, using either open craniotomy or endoscopic endonasal approaches. However, with advances in high-definition endoscopy, specialized instruments and biomaterials for reconstruction, there is a growing chance of performing selected repairs in an outpatient setting under local a

Safety Profile of Select Perioperative Lumbar Drain Use After Endoscopic Endonasal Surgery.

BACKGROUND AND OBJECTIVES: Lumbar drains (LD) may be selectively used in complex endoscopic endonasal surgery (EES) to promote cerebrospinal fluid (CSF) diversion and decrease CSF leak rates. Perioperative outcomes related to LD insertion have not been described in detail. We aimed to study safety profiles and complications associated with LD placement for EES. METHODS: We retrospectively reviewed patients undergoing EES for anterior skull-base tumors from January 2014 to December 2023. Patients were stratified based on selected use or nonuse of a LD placed perioperatively. Drain-related comp

Spontaneous cerebrospinal fluid rhinorrhea as a primary presentation of idiopathic intracranial hypertension, management strategies, and clinical outcome.

BACKGROUND: Causes of cerebrospinal fluid (CSF) rhinorrhea could be divided into primary (spontaneous) and secondary (head trauma and iatrogenic). Idiopathic intracranial hypertension (IIH) has emerged as a cause for spontaneous CSF rhinorrhea but is still underestimated, may be overlooked and needs special consideration in management. The objective of this study is to demonstrate spontaneous CSF rhinorrhea as the primary presentation of IIH and explore the algorithm of management. METHODS: All patients with spontaneous (primary) CSF rhinorrhea were included with complete clinical and radiolo

A case series of closed head trauma with pituitary stalk disruption resulting in hypopituitarism.

INTRODUCTION: Traumatic brain injury (TBI) is one of the main causes of morbidity and mortality in young trauma patients with resultant multi-organ effects. Hypopituitarism following TBI can be debilitating and life threatening. TBI which causes hypopituitarism may be characterized by a single head injury, such as from a motor vehicle accident, or by chronic repetitive head trauma, as seen in combative supports including boxing, kick-boxing, and football. In the majority of cases, a diagnosis of hypopituitarism can be entirely missed resulting in severe neuro-endocrine dysfunction. We present

Complications of Advanced Kadish Stage Esthesioneuroblastoma: Single Institution Experience and Literature Review.

INTRODUCTION: In esthesioneuroblastoma, greater disease extent and Kadish staging correlate with greater recurrence, complications, and mortality. These advanced stage malignancies require extensive resections and aggressive adjuvant therapy. This increases the risk of complications such as cerebrospinal fluid leak, neurologic deficits, and osteomyelitis. We present our case series and then analyze the literature to ascertain whether advanced stage tumors corresponds to greater rates of complications. METHODS: A retrospective review of consecutive patients with histologically-proven esthesion

Transcutaneous reduction of frontal sinus fracture using bony tapper device.

PURPOSE: Frontal sinus fractures are relatively common injuries presenting to trauma units. The optimal treatment of frontal sinus fractures remains controversial. Bicoronal approach is commonly used to obtain full exposure of both frontal bones and to repair any intracranial trauma; however, this approach requires long operative time and could lead to a long-lasting visible scar, alopecia, and paresthesias. In addition, patients have to stay in hospital for several days with this method. Endoscopic approach, less invasive than bicoronal method, has a steep learning curve. In this article, we

Missile injuries of the anterior skull base.

Missile injuries of the anterior skull base usually occur during war or war-like situations. These injuries may be isolated or associated with multiple traumatic injuries. We report 23 such cases managed during military conflicts and peacekeeping operations. All were adult males. Four of these patients sustained bullet injuries; the rest were injured from shrapnel. Eighteen patients had injury to the visual apparatus with permanent blindness. Proptosis was seen in 16, cerebrospinal fluid (CSF) leak from the wound in seven, and CSF orbitorrhea in three patients. Sixteen had irreparable injury t
