CT of benign soft-tissue masses of the extremities.

The computed tomographic (CT) scans and medical records of 35 patients with proven benign soft-tissue masses of the extremities were reviewed to assess the contribution of CT in the evaluation of such masses. CT demonstrated the mass in all 35 cases and was able to provide a specific diagnosis in 28 (80%); 25 prospectively, three retrospectively. Correct diagnoses made using CT included hematomas (five), synovial cysts (seven), myositis ossificans (six), fatty tumors (four), aneurysms (three), pseudoaneurysms (two), schwannoma (one), and abscess (one). The CT appearance of a hematoma depends o

Magnetic Resonance Imaging of Internal Derangements and Other Knee Pathologies in Adult Nigerians.

BACKGROUND: Knee joint pathologies/injuries are one of the most common musculoskeletal complaints in adults worldwide. The aetiologies of knee joint disorders are diverse. Magnetic resonance imaging (MRI) is a sophisticated method of detecting and characterizing knee pathologies. This study was conducted to document the clinical presentation and MRI patterns of knee joint abnormalities in a group of adults in Lagos, Nigeria, and to juxtapose it with reports from other climes. METHODOLOGY: A retrospective hospital-based analysis of the knee MRI of 158 adult Nigerians was conducted in a single

Two Cases of Popliteal Cystic Adventitial Disease Treated With Excision and Primary Bypass Graft: A Review of the Literature.

Cystic adventitial disease (CAD) is a rare vascular pathology which predominantly affects peripheral vessels of young otherwise healthy males. Much debate exists regarding its exact etiology. It is characterized by a collection of mucinous material within the adventitial wall layer of the affected vessel, resulting in arterial stenosis and rapidly progressive calf claudication. Treatment is primarily surgical, although radiological interventions have been reported. Its rarity makes evidence-based surgical or radiological management difficult. With this in mind, we report two cases of popliteal

Imbalance of angiogenic factors and avascular edematous cystic villi in a trisomy 13 pregnancy: a case report.

The incidence of pre-eclampsia is significantly higher in trisomy 13 pregnancies than in normal pregnancies. Soluble fms-like tyrosine kinase-1 (sFlt-1), located on chromosome 13, is an anti-angiogenic molecule derived from the placenta and contributes to the pathogenesis of pre-eclampsia. Elevated sFlt-1 and reduced placental growth factor (PlGF) are associated with trisomy 13 pregnancies and may play a pathogenic role in the subsequent development of pre-eclampsia. Here we present a case of a trisomy 13 pregnancy without any signs of pre-eclampsia that showed alterations in circulating angio

What is the clinical and ethical importance of incidental abnormalities found by knee MRI?

INTRODUCTION: Magnetic resonance imaging (MRI) is increasingly used to examine joints for research purposes. It may detect both suspected and unsuspected abnormalities. This raises both clinical and ethical issues, especially when incidental abnormalities are detected. The prevalence of incidental, potentially clinically significant abnormalities identified by MRI and their clinical significance in a population undergoing knee MRI in research studies are unknown. METHODS: We examined the prevalence of such lesions in healthy asymptomatic adults and those with symptomatic knee osteoarthritis (

Ganglion cysts associated with cruciate ligaments of the knee: a possible cause of recurrent knee pain.

Intraarticular ganglia of the knee are uncommon; however, these ganglion cysts may produce knee discomfort without a clear etiology. We present the cases of 10 patients with ganglion cysts arising from cruciate ligaments of the knee joint who underwent arthroscopic excision after MR examination. The MR findings, clinical features and arthroscopic findings were evaluated comparatively. Diagnoses were confirmed by means of a histological study after arthroscopic excision. The cysts were fluid-filled, with low T1-weighted signal intensity and high T2-weighted signal intensity. Except for two pati

Longitudinal ultrasound and clinical follow-up of Baker's cysts injection with steroids in knee osteoarthritis.

This study was conducted to assess ultrasound (US) and clinical changes of Baker's cyst (BC) of patients with knee osteoarthritis (OA) after steroid injection. Patients with knee OA complicated with symptomatic BC (40) were treated with US-guided direct (posterior) aspiration. The injection of 40 mg triamcynolone acetonide was in 20 patients direct into the BC and in other 20 subjects intra-articular (anterior). BC diameters (longitudinal, transverse, and thickness) were measured and followed up with US at baseline, 2, 4, and 8 weeks after injection. Swelling, pain, and range motion were score

The impact of magnetic resonance imaging of the knee on surgical decision making.

Although magnetic resonance imaging (MRI) of the knee is known to be an accurate technique for diagnosing soft tissue disorders of the knee, the value of MRI in improving patient outcome is controversial. The purpose of this project was to study the value of MRI in a subgroup of patients with knee pain and disability whose diagnosis was uncertain after standard orthopaedic evaluation. An extensive database was recorded prospectively for each of 208 patients providing clinical data as well as diagnoses from clinical, MRI, and surgical observations. These data were analyzed to determine the effe

Arthroscopic all-inside suture of symptomatic Baker's cysts: a technical option for surgical treatment in adults.

Synovial fluid caused by repeated effusions may replete the gastrocnemius-semimembranosus bursa (GSB) communicating with the knee joint. Fluid trapped inside the GSB through an alleged unidirectional valve-like mechanism forms a so-called Baker's cyst. Since a significant association of Baker's cysts with knee joint disorders has been reported, treatment should primarily address articular lesions causing recurrent effusions. Arthroscopic surgery provides an effective treatment in that both the cyst and associated joint disorders can be optimally visualized and accordingly treated. In the prese

[The value of ultrasound for correction of clinical diagnostic errors in soft tissue tumors].

In doubtful clinical diagnosis, ultrasound investigation may represent a workable diagnostic means in revealing the nature of soft tissue alterations, as will be demonstrated by 6 clinical examples; in all cases a wrong clinical diagnosis delayed the necessary therapy and possibly deteriorated the prognosis of disease. In these patients correction of the clinical diagnosis was of decisive importance for consecutive diagnostics and therapy.

Prevalence and associations of incidental nonvenous duplex findings discovered during lower extremity venous imaging.

OBJECTIVE: Venous duplex imaging defines venous pathology (VP). Unexpected clinically relevant findings are also found but rarely mentioned in the literature. This study aims to define the prevalence of ancillary findings (nonvenous duplex) by study type and venous outcome and subgroup associations with primary study indication and risk factors. METHODS: Our vascular laboratory database was queried for lower extremity venous duplex studies with comments regarding ancillary findings and associated patient demographics, primary study indication, associated conditions, and venous study outcome.

Magnetic resonance imaging of knee injuries in children.

The appearances of knee injuries on MR imaging are less well documented in children than adults. Some patterns of injury are shared by both groups of patients, e. g. meniscal damage. The frequency of specific injuries may differ, e. g. anterior cruciate ligament (ACL) tear. Congenital abnormality, coexistent pathology and previous treatment of the knee appear to be associated with meniscal problems. Discoid menisci are seen most frequently in children and have unique features on MR scans. Cruciate ligament tears are difficult to diagnose in the smallest children. The ACL may not be identified

Clinical effectiveness and safety of intra-articular injection of HYALGO in the management of knee osteoarthritis symptoms: A multicenter prospective study.

BACKGROUND: The reduced concentration of hyaluronic acid in the synovial fluid, leading to impairment of joint function and painful symptomatology during knee osteoarthritis (OA), can be restored by using injectable formulations of hyaluronic acid (HA) and chondroitin sulfate (CS), variable for relative composition, HA/CS molecular modifications, and injection protocols. The present study aims to assess the safety and performance of the intra-articular (IA) viscosupplementing agent HYALGO, a formulation combining 40 mg/mL HA (>1700 kDa) and 40 mg/mL CS, in the treatment of patients suffering f

[Radiosynoviorthesis of the knee joint: Influence on Baker's cysts].

AIM: The study examines the influence of a radiosynoviorthesis of the knee joint on the Baker's cyst volume over time. METHODS: Patients with radiosynoviorthesis of the knee joint with simultaneous Baker's cyst were retrospectively selected and asked for a sonographic control. The presence of a pre-therapeutic sonography with imaging of the Baker's cyst in longitudinal and cross-sectional view to determine the volume was necessary. Exclusion criterion was an intermediate therapy with influence on the volume of the Baker's cyst. Groups were formed with a time interval of 3 months (2 to 4 month

Prevalence of Baker's cyst in patients with knee pain: an ultrasonographic study.

The objectives of this study are to investigate the prevalence of Baker's cyst (BC) in patients with knee pain, and to assess the correlation between BC and severity of osteophytes and joint effusion. A retrospective study was conducted on a group of patients with knee pain referred to our outpatient clinic for ultrasonography of the knee between January 2010 and February 2011. Patients underwent an ultrasonographic exam of the knees to assess the presence of marginal femorotibial osteophytosis, joint effusion and BC. A dichotomous score was assigned to each item (1 present, 0 absent) and seve
