Pediatric chest II: Benign tumors and cysts.

Thoracic tumors are rare in children, and metastatic or malignant conditions must be excluded during the diagnostic evaluation. The majority of primary pulmonary neoplasms in children are malignant; this article primarily addresses benign tumors. Surgical resection is the standard treatment for benign thoracic tumors in children. Thoracotomy is a traditional approach, but the thoracoscopic technique for diagnosis and treatment of thoracic tumors is well established. The term benign tumors can be a misnomer in that although their histology is not malignant, these tumors can be locally aggressiv

An Unusual Cause of Extubation Failure: Surgical Fixation of the Endotracheal Tube With a Suture-A Case Report.

Technical difficulties encountered during endotracheal tube (ETT) extubation are rare but can precipitate catastrophic airway complications. We present the case of a 3-month-old male infant weighing 5.4 kg who underwent video-assisted thoracoscopic surgery (VATS) for a bronchogenic cyst excision. Following the procedure, initial attempts at extubation failed. Flexible fiberoptic bronchoscopy (FOB) identified that the ETT had been inadvertently sutured to the tracheal wall. Since this Vicryl suture also secured the surgical repair, extubation was delayed to avoid disrupting the anastomosis. On

[Congenital malformations of the lung, the radiologist's point of view].

Congenital lung malformations include a complex range of developmental abnormalities. Currently, most are diagnosed prenatally or during early childhood. They may, however, be discovered later, incidentally or in connection with non-specific symptoms, sometimes severe. Knowledge of their radiological appearances is necessary for their detection. Proper technique and analysis of cross-sectional imaging, computed tomography and magnetic resonance imaging, allow a definitive diagnosis in most patients and pre-treatment evaluation of surgical cases. This review will describe the radiological aspec

Delayed Hemorrhage Following Needle Aspiration for a Mediastinal Cyst: The Significance of Confronting Upside-Down Video-Assisted Thoracic Surgery Under Two-Lung Ventilation.

INTRODUCTION: Surgical resection remains the gold standard for managing mediastinal cysts, including bronchogenic cysts, whereas needle aspiration serves as an alternative option that can facilitate preoperative volume reduction or, in certain selected cases, serve as a definitive treatment. However, it may lead to rare but potentially life-threatening complications such as mediastinitis; therefore, its indication should be carefully considered. This report details a case of a delayed intracystic hemorrhage 3 days after an endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA), requirin

Walking a tightrope - A multitude of airway challenges in a mediastinal mass excision.

Providing a satisfactory lung isolation during the video-assisted thoracoscopic surgery in infants can be challenging due to the limited options for one lung ventilation. Additional anatomic pathologies/derangements add to the complexity in managing the airway in these tiny infants. We discuss a case of a 10-month infant scheduled for video-assisted excision of a recurrent bronchogenic cyst that was in proximity to recurrent laryngeal nerves, compressing the trachea and bronchus. The associated severe tracheo-bronchomalacia and the risk of an inadvertent recurrent laryngeal nerve injury demand

Cystic adenomatoid change is common to various forms of cystic lung diseases of children: a clinicopathologic analysis of 10 cases with emphasis on tracing the bronchial tree.

CONTEXT: Congenital cystic adenomatoid malformation, intralobar sequestration, extralobar sequestration, bronchopulmonary foregut malformation, bronchial atresia, and lobar emphysema generally are treated as separate entities, but there are commonalities among them. OBJECTIVE: To clarify the identity of congenital cystic adenomatoid malformation in relation to other cystic diseases. DESIGN: We studied 10 consecutive cases of surgically resected cystic lung disease in children. We studied the bronchial and vascular trees by mapping bronchi, pulmonary blood vessels, systemic blood vessels, the

[Carbon dioxide laser in pulmonary surgery].

Carbon dioxide laser (Skalpel-1 laser device) with a power of 30 to 80 Wt was used in operations on the lungs in 42 patients with focusing or defocusing of the beam, depending on the purpose--incision of the pulmonary tissue or bronchus, hemostasis or aerostasis. Among the patients subjected to operation, 21 had echinococcosis, 6 had bronchiectasis, 4 had a chronic abscess, 3 had an acute abscess of the lungs complicated by pyothorax, 3 had bullous disease, 2 had bronchogenic cysts, and 2 patients had hemangiopericytoma and fibroma of the lungs. The authors claim that carbon dioxide laser shor

The helpful support of veno-arterial extracorporeal membrane oxygenation to treat an exceptional systemic-to-pulmonary venous shunt: A case report.

Systemic-to-pulmonary venous shunting is a rare cause of refractory hypoxemia. Its prompt recognition is crucial, as management requires not only etiological treatment but also the restoration of physiological venous return. We report the case of a 40-year-old man presenting with dyspnea, cervicothoracic edema, and hypoxemia. Imaging revealed a mediastinal mass compressing the superior vena cava (SVC), with associated subocclusive thrombosis in its lower segment and right pulmonary artery. Endobronchial ultrasound-guided puncture and video-assisted thoracoscopic marsupialization confirmed a be

The bronchial cast syndrome after the fontan procedure: further evidence of its etiology.

We describe a three-year-old boy who presented with recurrent expectoration of bronchial casts six months following creation of a fenestrated lateral tunnel Fontan circulation for pulmonary atresia with intact ventricular septum. Cardiac catheterization demonstrated elevated central venous pressure with two areas of stenosis within the Fontan circuit, specifically at the junction of the right superior caval vein and the right pulmonary artery, and between the atrial baffle and the right superior caval vein. Insertion of Palmaz stents in these areas resulted in a reduction in central venous pre

[Malignant branchiogenic cyst--fact or fiction?].

A case of a cystic metastasis of squamous cell carcinoma of palatine tonsil in a 54-year-old male is reported, clinically presenting as a branchiogenic cyst. The grade of differentiation of the squamous cell epithelium lining the cyst was very variable: in some areas it was extremely well differentiated, thus resembling common benign branchial cleft cyst; transition to dysplastic areas with features of carcinoma in situ was visible and finally, in one small focus, invasion of the epithelial structures into the lymphoid tissue was observed. Despite its metastatic nature, the tumor fulfilled his

Diagnostic dilemmas of mediastinal cysts.

Children with foregut cysts of the mediastinum can present at any age with nonspecific respiratory symptoms or dysphagia. Chest radiograph and barium esophagram are recommended for initial evaluation, but they fail to identify some lesions. In other cases, an obvious mass may be confused with a solid neoplasm. We have operated upon 34 infants and children with mediastinal bronchogenic cysts and esophageal duplications from 1968 through 1985. This review of their clinical course and radiographic imaging studies emphasizes some of the diagnostic pitfalls that lead to operative delay. Twelve (35%

Robotic resection of benign nonadrenal retroperitoneal tumors: A consecutive case series.

BACKGROUND: The deep location, narrow operative space and proximity to major vessels make minimally invasive resection of nonadrenal retroperitoneal tumors (NTRs) challenging and rarely reported. This study aimed to evaluate the safety and feasibility of robotic resection of benign nonadrenal retroperitoneal tumors. METHODS: The demographics and perioperative outcomes of a consecutive series of patients who underwent robotic NTR resection between January 1, 2015, and August 30, 2017, were analyzed. RESULTS: Twenty-five patients (9 men and 16 women; mean age, 50.5 years) were included in the

Non-neoplastic mediastinal cysts.

OBJECTIVE: The non-neoplastic mediastinal cysts (NNMCs) form a group of uncommon benign lesions of a congenital origin. The significant controversy regarding these cysts is whether to manage with observation or surgical resection. The aim of this study is to analyse the utility of thoracic computed axial tomography (CT) in imaging diagnosis of the NNMCs and the results of surgery in these lesions. PATIENTS AND METHODS: Twenty NNMCs underwent surgery between 1980 and 2000. The preoperative study of mediastinal cystic masses includes a complete blood test, chest radiography (CR) and, for the la

Treatment of tongue base masses in children by transoral robotic surgery.

The feasibility and effectiveness of transoral robotic surgery (TORS) in children with tongue base masses (TBMs) were evaluated. Eight pediatric patients who were treated with TORS for TBMs between January 2010 and January 2016 at a tertiary hospital included in the study. All pathologies were congenital lesions: four were lingual thyroglossal ductus cysts (LTGDCs), one was a minor salivary gland tumor, one was a vallecular cyst, one was a bronchogenic cyst, and one was an ectopic thyroid tissue. TORS was performed successfully in all cases. The mean robotic set-up and exposure time was 13.0 ±

Clinicopathological analysis of small-sized anterior mediastinal tumors.

PURPOSE: The purpose of this study was to determine the clinicopathological findings and prognosis of small-sized anterior mediastinal tumors (SSAMTs). METHODS: A retrospective study was conducted on 43 patients who underwent surgery between January 1989 and December 2011 for SSAMTs. RESULTS: From the preoperative radiological findings, the tumors were classified into solid (n = 28) and cystic lesions (n = 15). The pathological diagnoses of the solid lesions included thymoma (n = 24), thymic carcinoma (n = 1), mucosa-associated lymphoid tissue lymphoma (n = 1), teratoma (n = 1) and neurofibr

[Study of cystic mediastinal lesions--diagnostic usefulness of magnetic resonance imaging].

We have recently operated 17 cases of cystic mediastinal lesions. A study was made here on their findings of magnetic resonance imaging (MRI). The MRI findings of all the cases revealed oval shaped mediastinal lesions. T2-weighted images showed the lesions to have extremely high signal intensity. Findings characteristic of each disease could also be observed. 1. Pericardial cyst. The cystic lesion was pericardial diverticulum located in cephalad recess of azygos vein (CRAzV) in 5 out of 6 cases. T1-weighted images revealed a mass lesion of low signal intensity with thin wall and enhancement ef

Inner cyst approach, remove bronchial cervical cyst.

Although most cystic lesions in young adults represent a branchial cleft cyst, the bronchial cervical cyst is not rare (Acta Oto-Laryngologica 2010;130:300). Surgical excision is both diagnostic and curative and also eliminates the risk of infection or recurrence. As we have all known, it is hard to protect some important structures in an inflamed tissue, especially in infants. However, a correct approach can avoid this kind of iatrogenic injury. An accurate diagnosis is always by pathologic examination. The diagnosis can be made before operation when finding cartilage in the nodule. We presen
