Pain management in non-operatively treated patients with multiple single rib fractures: an international comparison.

PURPOSE: Evidence on non-operative management of multiple single rib fractures remains limited, resulting in institutional treatment variation. This study compared outcomes of different pain management strategies. METHODS: A retrospective cohort study at Massachusetts General Hospital (MGH, USA) and Leiden University Medical Center (LUMC, The Netherlands) included patients ≥ 18 years with ≥ 3 single rib fractures between 2018 and 2023. Exclusion criteria included flail chest, surgical fixation, severe extra-thoracic trauma, Glasgow Coma Scale < 15 after 72 h, and transfers. Primary outcomes we

Variation in Prehospital Trauma Triage Protocols.

OBJECTIVES: Trauma patients are a high volume and morbidity population, showing the importance of their prehospital care. This study aimed to evaluate the current status and consistency of prehospital trauma triage protocols in the United States. METHODS: States with statewide emergency medical services (EMS) protocols and trauma triage criteria were included. For each state, EMS protocols were analyzed to assess trauma centers and categories, trauma triage criteria (including physiologic, anatomic, mechanism, and patient factors), and transport mode and destination guidance. RESULTS: Of 31

Comparison of In-Hospital Outcomes of Surgical Stabilization of Rib Fractures with Nonsurgical Management: A Multicenter, Prospective, Cohort Study.

BACKGROUND: Evidence for the efficacy of surgical stabilization of rib fractures in patients with rib fractures is controversial. OBJECTIVE: We aim to compare the clinical outcomes of surgical rib fixation for rib fracture with non-operative treatment. METHODS: Our institutional database from three general hospitals (Viet Duc Hospital, Viet Tiep Friendship Hospital & Cho Ray Hospital) was queried to identify patients with flail chest treated with locked plate fixation between December 2021 and February 2023. A medical record review for demographic, injury, hospital, and surgical data was als

Analysis of isolated transverse process fractures sustained during blast-related events.

BACKGROUND: Personnel have sustained a range of devastating blast injuries during recent conflicts. Previous studies have focused on severe injuries, including to the spine; however, no study has specifically focused on the most common spinal injury-transverse process (TP) fractures. Although their treatment usually requires limited intervention, analysis of TP fractures may help determine injury mechanisms. METHODS: Data were collected from victims with spinal fractures who were injured in improvised explosive device attacks, from the UK's Joint Theatre Trauma Registry. The level and side of

Urgent Surgical Treatment of Blunt Chest Trauma Followed by Cardiac and Pericardial Injuries.

INTRODUCTION: Widespread opinion that penetrating chest injuries are more urgent, in terms of treatment and care, contributed to underestimation of the urgency of blunt chest trauma, which in most cases is treated conservatively. It remains an open question frequency when the injuries of the heart and pericardium are not timely diagnosed and surgically treated. AIM: To demonstrate the importance of well-timed surgical treatment of blunt chest trauma, when coupled with cardiac and pericardial injuries. METHODS: At the Thoracic Surgery Clinic of the University Clinical Centre Banja Luka, Bosni

Nationwide cohort study of independent risk factors for acute respiratory distress syndrome after trauma.

BACKGROUND: The primary objective of this study was to evaluate the effect of specific direct and indirect factors that accounted, in trauma patients, for the development of acute respiratory distress syndrome (ARDS) and mortality in patients with ARDS. METHODS: We performed a retrospective cohort study of patients from the National Trauma Data Bank. Multilevel mixed-effects logistic regression was used with the development of ARDS as the primary and mortality in patients with ARDS as the secondary outcome measures. We compared trauma patients with versus without thoracic (direct) and extrath

Traumatic tricuspid valve insufficiency. Experience in thirteen patients.

From 1964 through June 1993, thirteen patients with traumatic tricuspid insufficiency were treated surgically; all were male, and the ages ranged from 17 to 64 years (median 39 years). The condition was associated with blunt chest trauma in all patients: motor vehicle accidents in twelve and an explosion of a tank of compressed air in one. The median duration between trauma and operation was 17 years (range 1 month to 37 years). Preoperatively, six patients were in sinus rhythm and seven were in atrial fibrillation. At operation, the right ventricular function appeared moderately to severely d

Outcomes in Single Versus Two-Stage Surgical Stabilization of Complex Bilateral Chest Wall Injury: A Single-Center Experience.

INTRODUCTION: Surgical stabilization of rib fractures (SSRF) continues to show benefit for severe chest wall injury (CWI) with improvement in morbidity and mortality following stabilization. Complex bilateral injuries pose a clinical and technical challenge. Currently, there is no formal recommendation for an approach to repair such injuries, whether that be bilateral SSRF within the index case or staged repair. We sought to describe our institutional experience with this subset of patients, identify factors that may influence the decision to pursue single- versus two-stage procedures, and ass

Age-Neutral Surgical Stabilization of Rib Fractures: Breaking the Ageism Barrier.

INTRODUCTION: Surgical stabilization of rib fractures (SSRF) is associated with lower rates of mortality and fewer complications. This study evaluates whether the decision to undergo SSRF is associated with age, race, ethnicity, and insurance status and assesses associated clinical outcomes. METHODS: This retrospective analysis included patients ≥45 y old with rib fractures who underwent SSRF in the Trauma Quality Improvement Program from 2016 to 2020. Race, ethnicity, and insurance statuses were collected. Age in years was dichotomized into two groups: 45-64 and 65+. Outcomes included ventil

Rib Plating as an Effective Approach to Managing Traumatic Rib Injuries: A Review of the Literature.

Timely repair is essential to maximizing outcomes in patients with traumatic rib injuries, whether in the presence or absence of flail chest (FC) or existing as single or multiple rib fractures (MRF), due to its high morbidity and mortality rate. This review focuses primarily on the plating system as an effective surgical approach to stabilizing these injuries. Literature was surveyed using the Google Scholar, PLOS One, and PubMed search engines between August 2021 and April 2022. A total of 34 articles were included herein, and primary and secondary outcomes were assessed. The primary outcome

Nonrheumatic mitral incompetence.

This review clearly indicates that a clinical picture of pure, isolated mitral insufficiency constitutes an interesting diagnostic challenge. In adult patients especially, this common valvular lesion is often nonrheumatic and may be found in a variety of cardiac conditions. The following provides a general orientation for their differential diagnosis. The first clue to the presence of papillary muscle dysfunction, a "spontaneous" chordal rupture, or a congenital or traumatic lesion which may account for the mitral insufficiency, may be derived from the patient's case history. A history suggest

Are rib fractures stable? An analysis of progressive rib fracture offset in the acute trauma setting.

BACKGROUND: Rib fractures serve as both a marker of injury severity and a guide for clinical decision making for trauma patients. Although recent studies have suggested that rib fractures are dynamic, the degree of progressive offset remains unknown. The purpose of this study was to further characterize the change that takes place in the acute trauma setting. METHODS: A 4-year (2016-2019) retrospective assessment of adult trauma patients with rib fracture(s) admitted to a level I trauma center was performed. Initial and follow-up computed tomography scans were analyzed to determine the magnit

Pain as an indication for rib fixation: a bi-institutional pilot study.

BACKGROUND: In trauma patients, open reduction and internal fixation of rib fractures remain controversial. We hypothesized that patients who have open reduction and internal fixation of rib fractures would experience less pain compared with controls and thus require fewer opiates. Further, we hypothesized that improved pain control would result in fewer pulmonary complications and decreased length of stay. METHODS: This is a retrospective bi-institutional matched case-control study. Cases were matched 1:2 by age, injury severity Score, chest abbreviated injury severity score, head abbreviate

Association Between Adherence to Evidence-Based Practices for Treatment of Patients With Traumatic Rib Fractures and Mortality Rates Among US Trauma Centers.

IMPORTANCE: Rib fractures are sustained by nearly 15% of patients who experience trauma and are associated with significant morbidity and mortality. Evidence-based practice (EBP) rib fracture management guidelines and treatment algorithms have been published. However, few studies have evaluated trauma center adherence to EBP or the clinical outcomes of each practice within a national cohort. OBJECTIVE: To examine adherence to 6 EBPs for rib fractures across US trauma centers and the association with in-hospital mortality. Evaluation of Trauma Triage Criteria Performance in a Regional Trauma System.

Objective: We evaluated the performance of individual trauma triage criteria using data from a regional trauma registry. Methods: Los Angeles County (LAC) paramedics use triage criteria adapted from the 2011 Center for Disease Control (CDC) guidelines to triage injured patients to Trauma Centers (TCs). TCs report outcomes to a LAC EMS registry. We abstracted data for patients 15 years or older from 2013 to 2015 and identified all trauma triage criteria that were met for each encounter. Study outcomes were: (1) "clear need" for a TC, defined as receiving a non-orthopedic operative intervention
