Treatment outcome of headache after occlusal appliance therapy in a randomised controlled trial among patients with temporomandibular disorders of mainly arthrogenous origin.

The aim of this study was to evaluate headaches before and after treatment with a stabilisation appliance and a control appliance in a randomised controlled trial in patients having temporomandibular disorders (TMD) of mainly arthrogenous origin. The effect of treatment was evaluated both in a short and in long-term perspective. 60 patients (mean age 30 years) with TMD of mainly arthrogenous origin were studied. The patients were selected from patients referred for treatment of TMD during a 3-year period to the Department of Stomathognathic Physiology, Faculty of Odontology, Malmö University.

Mandibular repositioning can be effective in treatment of reducing TMJ disk displacement. A long-term clinical and MR imaging follow-up.

In order to evaluate the long term clinical and morphologic results of recapture of a displaced TMJ disk, we recalled for follow-up MR imaging 75 patients who had been treated by attempted disk recapture based on pre-treatment MR imaging 1-6 years earlier. The treatment included a day appliance with inclines to guide the mandible into the therapeutic position and a telescopic night appliance which prevented retrusion of the mandible during sleep. Appliance treatment was followed by rebuilding or resurfacing the posterior teeth of one arch to permanently support the mandible in the therapeutic

Use and effectiveness of splint appliance therapy: review of literature.

Interocclusal orthopedic appliances of varied design and application have been employed in the treatment of myofascial pain dysfunction (MPD) and temporomandibular joint disorders (TMD). These appliances provide the practitioner with a non-invasive, reversible form of intervention to manage the patient's symptoms. Literature on the use and effectiveness of these appliances has become readily available and now requires retrospective evaluation. However, comparison of results from studies making use of interocclusal orthopedic appliance therapy is difficult due to the employment of various outco

Prosthetic therapy for sleep apnea syndrome.

Twenty patients with sleep apnea syndrome were examined polysomnographically with and without an Esmarch sleep apnea appliance. The relationship between the therapeutic effect of the device and dentofacial morphologic characteristics of the patients were analyzed with the use of diagnostic casts and cephalometric radiographs. The apnea index or number of apneic episodes per hour decreased significantly (p < 0.0001) after insertion of the appliance. The diagnostic casts and cephalometric analyses revealed that the device was most effective for patients who exhibit a tendency toward micrognathia

A 5-year report from a multicentre randomised clinical trial: immediate non-occlusal versus early loading of dental implants in partially edentulous patients.

PURPOSE: To compare peri-implant bone and soft-tissue levels of immediately non-occlusally loaded versus non-submerged early loaded implants in partially edentulous patients 5 years after implant placement. MATERIALS AND METHODS: Fifty-two patients were randomised in five Italian private practices: 25 in the immediately loaded group and 27 in the early loaded group. To be immediately loaded, single implants had to be inserted with a torque of > 30 Ncm, and splinted implants with a torque of > 20 Ncm. Immediately loaded implants were provided with non-occluding temporary restorations within 48

An investigation of the diagnostic value of computerized axiography in evaluating the dynamic function of the temporomandibular joint in patients with disc displacement.

AIM: To explore the diagnostic value of computerized axiography for patients with temporomandibular joint (TMJ) disc displacement. MATERIALS AND METHODS: A total of 17 patients with bilateral temporomandibular disorder (34 TMJs) were treated with a stabilization splint for 7.26 ± 1.82 months. The TMJs were divided into two groups-disc displacement with reduction (DDwR) (n = 20) and disc displacement without reduction (DDwoR) (n = 14)-according to pretreatment MRI. Computer-aided diagnosis axiography (CADIAX) was used to record and analyze the results of the condylography examinations, includi

Comprehensive Three-Dimensional Evaluation of Temporomandibular Joint Changes Following Stabilization Splint Therapy.

INTRODUCTION AND AIMS: While stabilization splints (SSs) have shown promising therapeutic effects for temporomandibular disorders (TMDs), comprehensive studies evaluating temporomandibular joint (TMJ) changes following SS therapy are necessary. This study aimed to assess TMJ structural, positional, and condylar remodelling changes in TMD patients by using advanced three-dimensional assessment and shape correspondence analysis. METHODS: This retrospective study included 80 adult TMD (arthralgia) patients treated with SS. Pre- and post-treatment cone beam computed tomography scans were analysed

Internet-Based Behavioural Therapy for Adolescents With Temporomandibular Disorder Pain: A Feasibility Multicenter Randomised Controlled Trial.

BACKGROUND: Temporomandibular disorder (TMD) pain in adolescents is common, has a negative impact on quality of life, and is a predictor of pain in young adulthood. A common and symptomatic treatment is occlusal appliance therapy (OAT). Internet-based behavioural therapy (IBT) is a novel, multimodal, and patient-centred self-management therapy for adolescents with TMD pain. OBJECTIVE: To evaluate the adherence and effect of IBT compared to OAT in adolescents with TMD pain. METHODS: Participants were recruited from public dental care and specialist clinics in orofacial pain in Sweden between

Diagnosis and treatment of snoring in adults-S2k Guideline of the German Society of Otorhinolaryngology, Head and Neck Surgery.

OBJECTIVES: This guideline aims to promote high-quality care by medical specialists for subjects who snore and is designed for everyone involved in the diagnosis and treatment of snoring in an in- or outpatient setting. DISCUSSION: To date, a satisfactory definition of snoring is lacking. Snoring is caused by a vibration of soft tissue in the upper airway induced by respiration during sleep. It is triggered by relaxation of the upper airway dilator muscles that occurs during sleep. Multiple risk factors for snoring have been described and snoring is of multifactorial origin. The true incidenc

Cervical external root resorption: 3-year follow-up of a case.

Cervical external root resorption can be present in natural teeth, involving one or more teeth in the same patient. The incidence of these lesions appears random and the etiology remains unclear. Cervical external root resorption has been considered a difficult clinical situation, and its diagnosis and treatment of the defect challenging. The present report describes a case of multiple external cervical resorption lesions involving four teeth, including the history, and the clinical and radiographic findings. The treatment included surgical intervention and restoration of the defect without sa

Conservative treatment of craniomandibular asymmetries during growth. A long term study.

OBJECTIVES: Previous studies on growing patients with structural cranio-mandibular asymmetries have shown that it is possible to correct the structural asymmetry, by means of an orthopaedic (positioning splint) -orthodontic approach. The aim of the present study was to ascertain the degree to which the results of such approach are stable over a long time. MATERIAL AND METHODS: Sixteen growing patients with non hereditary craniomandibular asymmetry treated according to the methodology previously described were examined. The degree of mandibular symmetry was assessed with Computer Aided Design

[Long term compliance with an oral protrusive appliance in patients with obstructive sleep apnoea].

BACKGROUND AND OBJECTIVE: Oral protrusive devices (OPD) are increasingly used in primary snoring and mild-to-moderate obstructive sleep disordered breathing. Due to evidence of reduced compliance with the well established standard treatment of nasal positive airway pressure (CPAP) and reports of patient preference for OPD treatment, particularly in mild cases, OPD may be considered another treatment alternative. PATIENTS AND METHODS: We contacted 192 patients suffering from obstructive sleep breathing disorder, who were treated between May 1996 and September 2001 with an OPD. The patients" us

Different Treatments in Patients with Temporomandibular Joint Disorders: A Comparative Randomized Study.

BACKGROUND AND OBJECTIVES: Temporomandibular joint disorders (TMJDs) are associated with pain and reduced jaw mobility. The aim of this study was to compare the outcome of patients with TMJ arthralgia when submitted to four different treatment modalities, in some cases using intra-articular injections of substances with anti-inflammatory properties and in others, a more conservative approach consisting only of a bite splint. Materials and Methods: The sample was made up of 80 patients, randomly distributed into 4 groups of 20 patients each. Each patient was given a nocturnal bite splint. One o

Does drug-induced sleep endoscopy change the treatment concept of patients with snoring and obstructive sleep apnea?

PURPOSE: Success rates of non-ventilation therapies for sleep disordered breathing (SDB) remain hardly acceptable. Drug-induced sleep endoscopy (DISE) tends to show the level and mechanism of obstruction and helps to specify therapy individually. Therefore, increasing success rates are expected. The objective of this study is to detect whether locations of treatment recommendations given after DISE are different to those made after clinical basic ENT (ear, nose, throat) examination (CBE). METHODS: This study included patients with obstructive sleep apnea (OSA) and primary snoring who wish or

Evidence for dental and dental specialty treatment of obstructive sleep apnoea. Part 1: the adult OSA patient and Part 2: the paediatric and adolescent patient.

Until recently, obstructive sleep apnoea was a largely unknown condition. Because of the well-publicised death of some high-profile people resulting from untreated obstructive sleep apnoea, now mostly everyone has heard of the condition. Following diagnosis, several medical treatment modalities are available to patients. However, the role that dentistry and its various specialties can play in successful treatment for obstructive sleep apnoea should not be overlooked. The common causes for adult and paediatric obstructive sleep apnoea will be presented as well as a review of the more successful

Treatment of temporomandibular disorders among adolescents: a comparison between occlusal appliance, relaxation training, and brief information.

In a randomized trial the effects of occlusal appliance and relaxation therapy, each combined with brief information, were compared with brief information only, in adolescents with temporomandibular disorder (TMD) pain. One-hundred-and-twenty-two adolescents (93 F and 29 M aged 12-18 years) were randomly assigned to one of the following 3 groups: brief information + occlusal appliance (BI + OA), brief information + relaxation therapy (BI + RT), or brief information (BI). Included were subjects reporting pain once a week or more often, in addition to receiving a diagnosis of TMD according to th

Treatment outcome in patients with TMD--a survey of 123 patients referred to specialist care.

Temporomandibular disorders (TMD) are a group of functional disorders of the masticatory system. Signs and symptoms of TMD are very common in different age groups in both genders. Treatment need varies greatly, ranging from 5 to 27% of the population. The treatment of TMD patients is generally initiated using reversible conservative methods and if necessary, irreversible and surgical methods are used. Altogether 123 patients (93 females, 30 males, mean age 39 years, range 12 to 72 years) were referred to the Oral and Maxillofacial Department of Oulu University Hospital between the years 2005 a
