Tight stapes prosthesis fixation leads to better functional results in otosclerosis surgery.

HYPOTHESIS: Tight fixation of stapes prostheses yields better functional results because sound transmission from the incus to the prosthesis is improved. BACKGROUND: The optimal prosthesis to use for otosclerosis surgery is still a matter of debate. It has been proposed that using prostheses made of Nitinol, a shape-memory metal, produces better functional results with less variability and reduced risk for middle and inner ear damage. This is thought to be because heat activation rather than manual crimping of the prosthesis loop forms a tighter fixation. METHODS: Functional results of two g

Comprehensive analysis of predictors and outcomes following Vibrant Soundbridge implantation - part 1 of a prospective study.

Active middle ear implants such as the Vibrant Soundbridge (VSB) represent effective treatment options for patients with conductive or mixed hearing loss when conventional hearing aids are insufficient. Nevertheless, postoperative speech intelligibility varies considerably, even among patients with comparable audiometric profiles. This first part of a two-part prospective observational study aimed to analyze outcomes and identify clinically accessible predictors of postoperative speech intelligibility. Twenty adults who underwent unilateral VSB implantation were included. Audiological assessme

Evolution of techniques of stapedectomy from the total stapedectomy to the small fenestra stapedectomy.

No consensus exists among surgeons about how much of the footplate to remove in stapes surgery. By comparing ideal cases with identical surgical techniques, it may be possible to select an optimal amount of the stapes footplate to remove. This article details the evolution of techniques of stapedectomy from the total stapedectomy to the small fenestra stapedectomy.

Optimization of surgical interventions in auditory rehabilitation for chronic otitis media: comparative between passive middle ear implants, bone conduction implants, and active middle ear systems.

INTRODUCTION: In otology consultations, patients with chronic otitis media (COM) often present as candidates for various hearing rehabilitation options. Selecting the most suitable approach requires careful consideration of patient preferences and expectations, the risk of disease progression, and the integrity of the bone conduction pathway. This study aims to evaluate and compare postoperative hearing outcomes in COM patients undergoing tympanoplasty (with or without passive middle ear implants), bone conduction systems (BCI), or active middle ear implants (AMEI). The objective is to assess

Impact of Imaging in Management of Otosclerosis.

The use of imaging in otosclerosis for diagnosis, preoperative assessment, and follow-up has the potential to give the clinician an additional tier of patient evaluation and validation of diagnosis. Before stapes surgery, imaging may help avoid unnecessary middle ear explorations in nonotosclerotic cases, prevent potential complications, and assist in appropriate patient counseling regarding management expectations. Postoperatively, following unsuccessful air-bone gap closure in stapes surgery or conductive hearing deterioration following initial successful closure of the air bone gap, imaging

A single-ossicle ear: Acoustic response and mechanical properties measured in duck.

To date, the single-ossicle avian middle ear (ME) is poorly understood, despite its striking resemblance to the design of many currently used ossicular replacement prostheses. This study aims to improve comprehension of this system. The acoustic response and the mechanical properties of the mallard middle ear were studied by means of optical interferometry experiments and finite element (FE) simulations. A finite element model was constructed based on μCT data and validated using the experimental results. Stroboscopic holography was used to measure the full-field displacement of the tympanic m

[A new method for the fixation of sound-conducting elements of the middle ear during ossiculoplasty].

The objective of the present work was to estimate the possibilities of the application of platelet-enriched plasma as a filtering material for ossiculoplasty. The results of surgical interventions on 120 patients presenting with chronic suppurative otitis media are reported. All the patients underwent closed-type sanation surgery (differential atticoanthrotomy) including either complete or partial restoration of the sound conduction mechanism in the middle ear. Platelet-enriched plasma was applied into the tympanic cavity of 55 patients as a fixing material for the reconstruction of the chain

A prospective multicentre otology database.

UNLABELLED: WHY IS AN INTERNATIONAL OTOLOGY DATABASE IMPORTANT? There are many reports on the outcome of otological interventions in the literature. However, it is difficult to make direct comparisons between these reports because of the lack of uniformity in reporting outcomes and patient selection. The aim is to create an interactive otology database for surgeons in the UK and Europe. METHODOLOGY: The proposed project is the creation of a common database on a website. Two levels of data entry are available: (1) level I: minimum database (only main outcomes are included; for any otolaryngolo

[Cochlear protection and surgical precision in stapedotomy with Er:YAG laser].

BACKGROUND: Using laser in stapedotomy has attracted a lot of attention since the nineties. It aims at minimizing complications due to hand-operated equipment, especially inner ear lesions, and achieving higher precision. We analyzed the clinical effect of Er:YAG laser with the question whether the postulated cochlea protection and foot plate perforation, optimized for better sound conduction, are achieved, compared to conventional stapedotomy. PATIENTS AND METHODS: We evaluated retrospectively audiometrical data including pre- and postoperative bone and air conduction thresholds of 114 conse
