INTRODUCTION: The aim of this study was to evaluate the deglutitive tongue movements in patients with skeletal Class II malocclusion.
METHODS: Fifty-nine patients (26 male, 33 female) with skeletal Class II relationship were divided into 3 groups according to cephalometric analysis. Group 1 (n = 19) had mandibular retrognathism, group 2 (n = 20) had maxillary prognathism, and group 3 (n = 20) had both mandibular retrognathism and maxillary prognathism. Twenty-two skeletal Class I patients (10 male, 12 female) were also included as the controls.
RESULTS: In the mandibular retrusion group, the

A tight cooperation between the orthodontist and oral surgeon is a key factor for success in orthognathic surgery. The quality of the final result is determined a great deal by the pre-operative model-surgery. The KDMMS is a planning tool which allows, even in the most complex cases, accurate and quick model-surgery. The two main components of the KDMMS are the double base plates and the model positioning system, which resemble a "fixation extérieure". The double base plates enable the operator to reproduce the original, pre-operative situation at all times. This allows multiple set-ups for on

An 8-year-old girl with a skeletal Class III malocclusion was treated in 2 phases. Maxillary expansion and protraction were carried out as the early intervention. However, her maxillary hypoplasia and mandibular hyperplasia deteriorated with age. The phase 2 comprehensive treatment began with proper mechanics when she was 12 years old with growth potential. In the maxillary arch, an auxiliary rectangular wire was used with a round main wire and an opening spring to create space for the impacted teeth and to bodily move the anterior teeth forward. Decompensation of mandibular incisors and corre

Hemifacial microsomia (HFM) is a complex three-dimensional congenital condition that is characterized by mandibular hypoplasia and unilateral or bilateral microtia; although, other facial structures may be affected. Little is known about craniofacial growth and morphology in patients with HFM; therefore, we examined 75 HFM patients by means of a cephalometric analysis in a longitudinal study on serial lateral cephalograms. We hypothesized that the growth of several facial structures on both sides of HFM patients would be different compared to Dutch controls. We determined patients with HFM had

This case report describes the treatment of a boy with both central maxillary incisors impacted because of 2 supernumerary teeth. Therapeutic management of the impacted teeth was combined with orthodontic treatment of a Class II Division 1 malocclusion. A sequential approach of surgical removal of the supernumerary teeth with 2 stages of surgical exposure and orthodontic traction of the impacted teeth resulted in proper incisor positioning. Close monitoring and multidisciplinary cooperation during the various treatment phases led to a successful esthetic result, with good periodontal health an

With the introduction during the last years of new tools such as low friction braces, skeletal anchorage, and especially mini screws, the tremendous evolution of lingual orthodontics, orthodontic care has evolved at a rhythm never known before. Using various clinical examples, the author shows the tremendous impact of the use of mini screws in a great number of treatments, producing particularly favourable facial changes, often going beyond the usual limits of orthodontic treatments.

After sagittal split osteotomy, osteosynthesis for stabilization of the fragments is usually performed by means of rigid or semirigid fixation to avoid maxillomandibular fixation (MMF). Sometimes, this leads to malpositioning of the fragments with resulting 'immediate relapse' or disturbances of the temporomandibular joints (TMJs) such as progressive idiopathic condylar resorption, which is the worst effect. We have therefore developed a new bone fixation system that allows three-dimensional adjustment even after fixation of the fragments. The special configuration of the system avoids intraop

The interaction between craniofacial structure assessed by lateral cephalometry, and tongue, soft palate, and upper airway size determined from computed tomography (CT) scans was examined in 25 control subjects and 80 patients with obstructive sleep apnea (OSA). On the basis of the cephalometric analyses, the patients with OSA had retruded mandibles with larger ANB angle differences, elongated maxillary and mandibular incisors and mandibular molars, and high total upper and lower face heights The computed tomographic evaluations revealed that patients with OSA also had larger tongue, soft pala

The Herbst appliance (HA) is considered an efficient method to treat class II malocclusion characterized by mandibular retrusion. Nevertheless, HA is although hampered by side effects reducing its therapeutic potentialities. The association between HA and TADs in the lower arch has proved to be very effective in controlling lower incisor flaring. This case report shows that the insertion of TADs in both arches offers a satisfactory control of the vertical dimension, by avoiding a maxillary clockwise inclination.

INTRODUCTION: The treatment options for adults with increased overbite are limited to dentoalveolar changes that camouflage the condition. Because of high relapse tendency, defining the problem area is important when creating a treatment plan. This study aimed to evaluate dentoskeletal morphology in skeletal Class I and II anomalies associated with Angle Class I, Class II Division 1 (Class II/1), and Class II Division 2 (Class II/2) malocclusions with increased overbite compared with normal occlusion.
METHODS: Pretreatment cephalograms of 306 patients (131 men, 175 women; overall ages 18-45 y

BACKGROUND: The use of functional appliances for the correction of retrognathic mandible is very common in orthodontics. Similar appliances known as oral appliances are also frequently used in adults for the treatment of mild to moderate obstructive sleep apnea (OSA). Many studies have reported improvement of pharyngeal airway passage (PAP) dimensions following functional appliance therapy in children and oral appliance therapy in adults. There is only one study in the literature that discussed the effect of oral appliance therapy on posterior pharyngeal wall thickness (PPWT) among subjects wi

OBJECTIVE: The purpose of this study was to identify a surgical reference point on the mandibular ramus that can be used during ramus osteotomy to prevent injury to the inferior alveolar nerve.
MATERIALS AND METHODS: A total of 125 subjects' mandibles were analyzed and compared on a three-dimensional (3D) model constructed from computed tomography (CT). 25 volunteer subjects with normal class I occlusion (group I, control), 50 consecutive subjects (25 females and 25 males) diagnosed with mandibular retrognathism (group II), and 50 consecutive subjects (25 females and 25 males) with prognathis

PURPOSE: Anterior alveolar osteodistraction is a common method for enlargement of the dentoalveolar process, and bone-borne distraction devices are hypothesized to avoid the risk of dental tipping and periodontal impairment during distraction. The aim of this study was to objectify this thesis and to determine the reliability of bone-borne osteodistraction of the anterior alveolar process.
PATIENTS AND METHODS: The study group consisted of 18 consecutive patients who underwent anterior alveolar segmental distraction with a bone-borne distraction device for the treatment of dental crowding or

A 10-year-old girl with maxillary protrusion and crowding of the maxillary and mandibular anterior teeth was treated in 2 phases. In the first phase of treatment, maxillary anterior growth was restrained and mandibular anterior growth promoted for skeletal improvement. In the second phase, at age 13, 4 first premolars were extracted, and an edgewise appliance was used for alveolar improvement. Because the patient had a high mandibular plane angle with a large Frankfort-mandibular plane angle, special care was taken from the beginning to prevent mandibular clockwise rotation. The peroperative management of a 4 1/2 yr old child having bilateral recurrent temporomandibular joint (TMJ) ankylosis and obstructive sleep apnoea (OSA) is presented here. From the anaesthesiologist's point of view, the patient demands special considerations as regards difficulty in intubation, high sensitivity to all central depressant drugs, awake extubation, and post-operative maintenance of a clear airway. It is to be concluded that these conditions call for strict adherence to the basic principles of airway management and close observation extending into the post-operative period.

Few studies of dentofacial and orthodontic structural relationships relative to temporomandibular joint (TMJ) dysfunction have been reported. We undertook this investigation to determine any correlation of orthodontic and dentofacial characteristics with TMJ bilateral disc displacement. The population of patients was selected from a TMJ clinic where a control group of asymptomatic volunteers had been previously established and standardized. Differences in skeletal structural features were determined among three study groups: (1) asymptomatic volunteers with no TMJ disk displacement, (2) sympto

Orthodontic and orthopaedic effects of hyperpropulsors have been studied, evaluating the possible adaptation of the temporo mandibular joint. Before the event of MR imaging, the positional modification of the disc could not be seen. The study has been carried out on 15 subjects, skeletal Class II, with a mean age of 11.6 years (SD = 1 year). A bilateral MR investigation has been done before and after the wear of an activator (Herbst or Monobloc with Head Gear). All patients underwent bilateral imaging in sagittal and modified coronal planes in closed and opened mouth. The qualitative analysis
