Three-Flap Mastopexy with Reduction or Augmentation.

BACKGROUND: Many authors have studied breast ptosis and made contributions to the search for a procedure that allows its correction with long-lasting results and minimal scars. Collective evolution has allowed us to reach the point where we are today and will allow us to continue improving techniques in the future. OBJECTIVES: The mastopexy procedure that we have used for the last 11 years, resulting from our surgical practice and countless observations at conferences, is a versatile procedure, applicable to different types of breasts and mastopexies with and without reduction or augmentation

Anatomic Characteristics of Tissues Attached to the Fifth Metatarsal Bone.

BACKGROUND: Two types of stress, bending stress and traction stress, have been reported to be involved in the mechanism of Jones fracture. However, little is known about the risk factors for traction stress. PURPOSE: To classify the attachment position of the peroneus brevis muscle (PB), peroneus tertius (PT), lateral band of the plantar aponeurosis (LB), and the long plantar ligament (LPL), focusing on the zone where a Jones fracture occurs (zone 2), and to compare the footprint area of each tissue type. STUDY DESIGN: Descriptive laboratory study. METHODS: This study examined 102 legs from

The importance of the greater occipital nerve in the occipital and the suboccipital region for nerve blockade and surgical approaches--an anatomic study on cadavers.

OBJECTIVE: Greater occipital nerve (GON) blockade is an effective method for treatment of occipital neuralgias. Occipital neuralgia or neuropraxis of this region may be seen particularly as a result of compression of the GON. This study shows the relationship between the GON and its external bone landmarks, in order to prevent complications and to perform nerve blockades safely. The study also defines the points where the GON pierces the semispinalis capitis (SSC) and the trapezius, and where the GON passes the obliquus capitis inferior (OCI), and identifies bone landmarks for places where the

Surgical management of carpal tunnel syndrome in neurosurgery practice in Ethiopia and review of literature.

Carpal tunnel compression of the median nerve is a common condition with an estimated incidence of 0.1% to 10% and a direct medical cost of above dollars 1 billion per year in the USA. Between the years 1995 to 2002, 28 cases, 6 (21.4%) males and 22 (87.6%) females of mild to severe forms of carpal tunnel syndrome (CTS) were seen and treated by the author in Tikur Anbessa Hospital (TAH) Neurosurgery Unit and a private hospital in Addis Ababa. Of the 28 cases, 22 (87.6%) patients, 7/22 (31.8%) mild and 15/22 (68.2%) severe forms, were surgically treated with the open surgery method. The male to

Proposal for an Expanded Classification of the Superficial Musculoaponeurotic System (SMAS) in the Human Forehead, Based on Anatomical and Microscopic Study.

INTRODUCTION: The superficial musculoaponeurotic system (SMAS) is fundamental for facial soft tissue support and surgical rejuvenation. Although its morphology in the midface and neck is well characterized, the structure of its cranial extension to the forehead remains a subject of terminological uncertainty. The aim of this study was to conduct a detailed histological and immunohistochemical examination of the forehead supporting structures to characterize their morphology and propose an expanded, region-specific classification of the SMAS. MATERIAL AND METHODS: Full-thickness tissue specime

An anatomical basis for endoscopic cubital tunnel release and associated clinical outcomes.

PURPOSE: To study the ulnar nerve in fresh-frozen cadavers as related to endoscopic release of the cubital tunnel and to present a retrospective review of patients treated with decompression via endoscopic visualization. METHODS: To further our understanding of relevant anatomy, we dissected 26 cadaver limbs. We paid special attention to fascial membranes as potential sites of constriction as well as the position of nerves, vessels, and aberrant anatomy. These findings facilitated our understanding of the extent of release in 80 patients (92 cases) with endoscopic cubital tunnel simple decomp

Late presentation of fractures of the lateral condyle of the humerus in children.

BACKGROUND: The current controversy regarding the management of fractures of the lateral condyle of the humerus presenting between 3 to 12 weeks prompted us to evaluate our results of open reduction and internal fixation of such fractures. PATIENTS AND METHODS: Twenty-one patients operated between March 1995 and February 2001 qualified for this study. Five patients presented between 3-4 weeks, nine between 5-8 weeks and seven between 9-12 weeks post injury. Ten fractures were classified as stage II and eleven as stage III (Jacob et al. criteria). The mean age was 8 years (range: 4-14 years).

Elastic properties of muscle-tendon complex in long-distance runners.

The purpose of this study was to investigate the elastic properties of muscle-tendon complex (MTC) in knee extensor muscles and the capacity for elastic energy utilization in long-distance runners (LDR) by comparing with data obtained from untrained individuals (CON). The elongation (L) of the tendon and aponeurosis of vastus lateralis muscle during isometric knee extension was determined by real-time brightness mode ultrasonography, while the subjects developed a gradually increasing torque from 0 (relaxed) to maximal effort (MVC) within 7 s. In addition, performances in two kinds of maximal

Loss of the knee-ankle coupling and unrecognized elongation in Achilles tendon rupture: effects of differential elongation of the gastrocnemius tendon.

PURPOSE: The biarticular anatomy of the gastrocnemii is an important mechanism of knee-ankle coupling and differential elongation may affect this function leading to weakness of the push-off phase during the gait. Achilles tendon ruptures may cause detachment of the gastrocnemius tendon from the soleus aponeurosis with subsequent differential elongation of the individual subtendons. This study investigated the effects of such detachment by investigating tendon fusion levels of the two muscle groups, and the effect of sequential differential elongation of the gastrocnemius on the Achilles tendo

Minimal invasive para-rectus approach for limited open reduction and percutaneous fixation of displaced acetabular fractures.

INTRODUCTION: Minimal invasive fixation has been reported as an alternative option for treatment of acetabular fractures to avoid blood loss and complications of extensive approaches. Closed reduction and percutaneous lag screw fixation can be done in minimally displaced acetabular fractures. Open reduction is indicated, if there is wide displacement. In this study, we report the use of a mini-open anterior approach to manipulate and reduce anteriorly displaced transverse acetabular fractures combined with percutaneous lag screw fixation. METHODS: This report included eight patients. All had

[The striated sphincter of the urethra. 1: Recall of knowledge on the striated sphincter of the urethra].

The first part of this report is devoted to the anatomy, nerve supply, and physiology of the striated sphincter, three subjects which greatly aroused a great deal of controversies. The conclusions of the authors are as follows. The striated sphincter of the urethra is a muscle of the perineum, arising from the cloacal sphincter. The deep transverse, bulbo-cavernous, anal sphincter, ischio-cavernous and superficial transverse muscles have the same embryological origin and together with the sphincter of the urethra form the accessory pelvic diaphragm. The embryological origin of the levator ani

Differential Plantaris-Achilles Tendon Motion: A Sonographic and Cadaveric Investigation.

BACKGROUND: Differential motion between the plantaris and Achilles tendons has been hypothesized to contribute to pain in some patients presenting with Achilles tendinopathy. However, objective evidence of differential Achilles-plantaris motion is currently lacking from the literature. OBJECTIVE: To determine whether differential, multidirectional motion exists between the plantaris tendon (PT) and Achilles tendon (AT) as documented by dynamic ultrasound (US) and postdissection examination in an unembalmed cadaveric model. DESIGN: Prospective, cadaveric laboratory investigation. SETTING: Pr
