The LNT Model and ALARA in Diagnostic Imaging: A Critical Review of Scientific Evidence in the Low-Dose Range.

BACKGROUND: The Linear No-Threshold (LNT) model and the As Low As Reasonably Achievable (ALARA) principle form the cornerstone of contemporary radiation protection. Although these frameworks were designed as conservative regulatory tools, their extrapolation to the low-dose and dose-rate (LDDR) range commonly encountered in diagnostic imaging remains scientifically contentious. OBJECTIVE: This review critically examines the scientific basis for applying the LNT model and the ALARA principle in the LDDR range (1-50 mGy) routinely encountered in diagnostic imaging, with the aim of evaluating wh

Cancer Incidence and Childhood Residence Near the Coldwater Creek Radioactive Waste Site.

IMPORTANCE: Beginning in the 1940s, radioactive waste from the effort to develop an atomic bomb was stored in the open near the St Louis, Missouri, airport, and over several decades contaminated nearby Coldwater Creek. OBJECTIVE: To evaluate whether living near Coldwater Creek in childhood was associated with self-reported cancer incidence in the St Louis Baby Tooth-Later Life Health Study (SLBT). DESIGN, SETTING, AND PARTICIPANTS: The SLBT cohort study was centered in St Louis and included individuals who donated their baby teeth from 1958 to 1970 to assess exposure to atmospheric nuclear w

Acute intensive care unit management of mustard gas victims: the Turkish experience.

PURPOSE: Sulphur mustard (SM) is an highly toxic and vesicant chemical weapon that was used in various military conflicts several times in the history. The severity of ocular, dermal, and pulmonary symptoms that may appear following a characteristic asymptomatic period are depending on the SM concentration and exposure duration. The aim of this study is to present the clinical features and share the intensive care unit (ICU) experiences for the medical management of mustard gas victims. MATERIALS AND METHODS: Thirteen Free Syrian Army soldiers near Al-Bab region of North Syria were reportedly

A method for determining weights for excess relative risk and excess absolute risk when applied in the calculation of lifetime risk of cancer from radiation exposure.

Radiation-related risks of cancer can be transported from one population to another population at risk, for the purpose of calculating lifetime risks from radiation exposure. Transfer via excess relative risks (ERR) or excess absolute risks (EAR) or a mixture of both (i.e., from the life span study (LSS) of Japanese atomic bomb survivors) has been done in the past based on qualitative weighting. Consequently, the values of the weights applied and the method of application of the weights (i.e., as additive or geometric weighted means) have varied both between reports produced at different times

Genetic effects of radiotherapy for childhood cancer.

Radiation-induced heritable diseases have not been demonstrated in humans and estimates of genetic risks for protection purposes are based on mouse experiments. The most comprehensive epidemiologic study is of the Japanese atomic bomb survivors and their children, which found little evidence for inherited defects attributable to parental radiation. Studies of workers exposed to occupational radiation or of populations exposed to environmental radiation appear too small and exposures too low to convincingly detect inherited genetic damage. In contrast, survivors of childhood cancer form the lar

Penetrating cardiothoracic war wounds.

OBJECTIVE: Penetrating cardiothoracic war wounds are very common among war casualties. Those injuries require prompt and specific treatment in an aim to decrease mortality and late morbidity. There are a few controversies about the best modality of treatment for such injuries, and there are not many large series of such patients in recent literature. METHODS: We analysed a group of 259 patients with penetrating cardiothoracic war wounds admitted to our institutions between May 1991 and October 1992. RESULTS: There were 235 (90.7%) patients with thoracic wounds, 14 (5.4%) patients with cardia

DTI-ALPS as a Biomarker of Small Vessel Disease Progression and Amyloid-β in Normal Aging: A 3-Year Longitudinal Study.

Cerebral small vessel disease (SVD) is a leading cause of stroke, vascular cognitive impairment, and functional decline in older adults. Emerging experimental and translational data implicate glymphatic dysfunction in SVD pathogenesis and in vascular amyloid accumulation. The analysis along the perivascular space (ALPS) index, derived from diffusion tensor imaging (DTI), noninvasively estimates water diffusivity along perivascular pathways and has been proposed as an indirect imaging marker related to glymphatic function. In this study, we aim to determine whether the baseline DTI-ALPS index i

Diiron(I) Bis(cyclopentadienyl) Complexes with Bridging Iminium Ligands: From Foundational Organometallic Chemistry to Unique Reactivity and Biological Potential.

ConspectusDimetallic complexes offer a remarkable platform to probe metal-metal cooperativity, enabling ligand reactivity patterns that are inaccessible to mononuclear systems. Starting from [Fe2Cp2(CO)4] (Fp2, Cp = η5-C5H5), diiron μ-aminocarbyne (iminium) complexes are available through a straightforward multigram-scale procedure. Carbonyl removal is key to enabling selective modification of the ligand set and promoting the formation of uncommon hydrocarbyl ligands involving the carbyne center. In this context, the insertion of terminal alkynes into iron-carbynes bond affords a wide diversit

Organ Dose Reconstruction Applicable for a Japanese Nuclear Worker Cohort: J-EPISODE.

An evaluation of cancer risk based on organ-absorbed dose is underway for the Japanese Epidemiological Study on Low-Dose Radiation Effects (J-EPISODE), which has analyzed health effects in association with radiation exposure evaluated with the personal dose equivalent Hp(10). Although the concept of effective dose and its operational definition of Hp(10) are widely used for radiological protection purposes, effective dose is not recommended for epidemiological evaluation. Organ-absorbed dose was instead adopted for the IARC 15-Country Collaborative study (15-Country study), the International N

Is there a place for quantitative risk assessment?

The use of ionising radiations is so well established, especially in the practice of medicine, that it is impossible to imagine contemporary life without them. At the same time, ionising radiations are a known and proven human carcinogen. Exposure to radiation in some contexts elicits fear and alarm (nuclear power for example) while in other situations, until recently at least, it was accepted with alacrity (diagnostic x-rays for example). This non-uniform reaction to the potential hazards of radiation highlights the importance of quantitative risk estimates, which are necessary to help put th

Radiation in the workplace-a review of studies of the risks of occupational exposure to ionising radiation.

Many individuals are, or have been, exposed to ionising radiation in the course of their work and the epidemiological study of occupationally irradiated groups offers an important opportunity to complement the estimates of risks to health resulting from exposure to radiation that are obtained from other populations, such as the Japanese survivors of the atomic bombings of Hiroshima and Nagasaki in 1945. Moreover, workplace exposure to radiation usually involves irradiation conditions that are of direct relevance to the principal concern of radiological protection: protracted exposure to low le

Chemical or biological terrorist attacks: an analysis of the preparedness of hospitals for managing victims affected by chemical or biological weapons of mass destruction.

The possibility of a terrorist attack employing the use of chemical or biological weapons of mass destruction (WMD) on American soil is no longer an empty threat, it has become a reality. A WMD is defined as any weapon with the capacity to inflict death and destruction on such a massive scale that its very presence in the hands of hostile forces is a grievous threat. Events of the past few years including the bombing of the World Trade Center in 1993, the Murrah Federal Building in Oklahoma City in 1995 and the use of planes as guided missiles directed into the Pentagon and New York's Twin Tow

Residential Radon Levels and Ovarian Cancer Among Postmenopausal Women.

IMPORTANCE: Few environmental risk factors for ovarian cancer have been discovered. Women exposed to ionizing radiation from the atomic bomb during World War II experienced an increased risk of ovarian cancer. Today, the largest source of ionizing radiation is radon gas in the home, but whether ionizing radiation is associated with increased risk of ovarian cancer more broadly is unknown. OBJECTIVE: To evaluate whether higher home radon levels are associated with increased risk of ovarian cancer. DESIGN, SETTING, AND PARTICIPANTS: This prospective cohort study included 127 547 women from the

Radiation Dose and Solid Cancer Mortality Risk in the Techa River and East Urals Radioactive Trace Cohorts in 1950-2016.

The objective of the work was to estimate the dose dependence of mortality risk from solid cancers in a cohort that includes members of two cohorts of residents of the Southern Urals who received chronic environmental low-dose, low-dose-rate radiation exposure from releases of the Mayak Plutonium Production Association. These analyses use dose and dose uncertainty estimates from a recently developed Monte-Carlo dosimetry system. The 47,950 members of the cohort include the Techa River Cohort of people who lived in the villages on the Techa River between 1950 and the end of 1960 and the East Ur

Characterizing major and trace element concentrations, and radioactivity in bulk glass samples from the trinity atomic test.

Glass fragments (16 green glasses and 2 red glasses) were handpicked from crushed Trinitite. X-ray diffraction studies revealed that these samples were essentially pure glass with the exception of minor amounts (less than 4 wt%) of quartz (which acts as a diluent) in some samples. The concentrations of 45 elements in the Trinity glasses were determined using Instrumental Neutron Activation Analysis. Plutonium-239 concentrations were determined by measuring the activity of fission products produced during thermal neutron irradiation of the glasses. The activity of various radionuclides was dete

Heart Disease Mortality in the Life Span Study, 1950-2008.

Based on the findings from the Radiation Effects Research Foundation's studies of the cohort of Japanese atomic bomb survivors, it has been reported that total-body irradiation at 0.5-1.0 Gy could be responsible for increased rates of mortality from broad-based categories of cardiovascular disease (CVD), i.e., stroke and heart disease. However, CVD consists of various subtypes that have potentially different radiation dose responses, as well as subtype-specific risks that have not been fully evaluated. Potential problems with changes in the coding rules for the International Classification of

[Epidemiologic study of Creutzfeldt-Jakob disease from autopsy-confirmed cases].

We investigated epidemiologic data from 61 cases of autopsy-confirmed sporadic Creutzfeldt-Jakob disease (CJD). Dura mater-associated CJD cases and familial CJD cases were excluded. There were 34 male and 27 female cases, with an average age at onset of 66.0 +/- 10.5 years (range 27 to 89). At onset of CJD, 1 case was aged in the 20's and 1 in 30's, but there were no cases aged in the 40's. In 6 cases, age at onset was in the 80's. There was no significant difference in relation to the age at onset between males and females, averaging 66.2 +/- 12.4 years (range 27 to 89), and 65.9 7.5 years (r
