Lumbar intradural extramedullary bronchiogenic cyst.

The pathological findings of an intradural and extramedullary cyst at the L1 level of the spinal canal are described in a 28-year-old male who presented with chronic lumbago and progressive weakness and numbness in both lower limbs. Histopathological examination revealed the diagnosis of bronchiogenic cyst. Bronchiogenic cysts in the spinal canal are uncommon, and cysts at the L1 level are extremely rare. Their pathogenesis is still poorly understood. They are thought to be a malformation arising from a split notochordal syndrome. We conclude that intradural extramedullary bronchiogenic cysts

Pleuropulmonary blastoma and the challenge of distinguishing the cystic form (type I) from other congenital lung malformations.

Pleuropulmonary blastoma is a rare pediatric lung tumor of mesenchymal origin, with prognosis varying by subtype. While types II and III are usually identifiable on imaging due to their solid tumor components, the purely cystic type I represents a particular diagnostic challenge. In imaging, it is often difficult to distinguish from benign congenital lung malformations such as congenital pulmonary airway malformation (CPAM, formerly congenital cystic adenomatoid malformation or CCAM), sequestration, or bronchogenic cyst. Accurate differentiation, however, is clinically relevant, as misclassifi

Surgical treatment of a cystic neoplasm of the posterior mediastinum: Case report and literature review.

INTRODUCTION AND IMPORTANCE: The study presents a clinical case of removal of a paravertebral tumour by thoracoscopic method and a review of the literature on the diagnosis and treatment of posterior mediastinal tumours. CASE PRESENTATION: A 26-year-old female patient presented with an extradural, paravertebral posterior mediastinal tumour and complaints of low back pain. The mass was diagnosed by computer tomography (CT) and magnetic resonance imaging (MRI). The mass was mobilised from the adjacent vital structures without rupturing the capsule by thoracoscopic approach. The histological exa

Extralobar pulmonary sequestration showing high CA 19-19 levels.

We report a case of 40 year-old woman with left thoracic pain who was diagnosed as having a cystic mass located posteriorly at the basis of the left pleural cavity. The preoperative serum CA 19-9 level was increased (2 900 IU/mL). Further investigations did not find neoplastic lesions in the gastrointestinal tract. The intraoperative finding of an anomalous systemic arterial supply to the mass suggested the diagnosis of extralobar sequestration, confirmed at the histopathological examination. The association between pulmonary sequestration and increased tumor markers levels is overlooked in we

A case of a transient phrenic nerve paralysis after resection of a giant lymphangioma evaluated by dynamic digital radiography.

BACKGROUND: Lymphangioma is a relatively rare benign congenital malformation composed of dilated cystic lymphatic vessels; however, its occurrence in adults and manifestation as mediastinal lymphangioma are even rarer. In general, the incidence of mediastinal lymphangiomas has been reported to range from 0.01 to 4.5% of all mediastinal tumors. Mediastinal lymphangiomas tend to increase in size and can sometimes stretch the phrenic nerve that are present in the mediastinum, resulting in phrenic nerve palsy. Dynamic digital radiography (DDR) allows for quantitative evaluation over time and has r

Role of CT in the evaluation of children with foregut cyst.

The presentation of a foregut cyst may vary from an asymptomatic mass discovered as an incidental finding in a chest radiograph, to signs or symptoms secondary to airway compression by the cyst. The radiographic evaluation of a child with a possible foregut cyst usually consists of a chest radiograph and barium esophagogram. Although this approach is often sufficient, in other instances delay of treatment occurs or unnecessary workups are performed due to the inadequacy of the approach. CT is useful for depicting cryptic foregut cysts, clarifying abnormal radiographs, avoiding unnecessary work

Congenital cystic lung disease: contemporary antenatal and postnatal management.

Congenital cystic lung disease comprises a broad spectrum of rare but clinically significant developmental abnormalities, including congenital pulmonary adenomatoid malformations, bronchopulmonary sequestrations, bronchogenic cysts, and congenital lobar emphysema that result from perturbations in lung and airway embryogenesis. As congenital lung lesions are now more commonly recognized antenatally, mothers require accurate prenatal counseling and appropriate perinatal management. In light of long-term complications of infection and malignancy, there is growing consensus that infants with asymp

Anaesthesia and Marfan's syndrome: case report.

An 18-year-old male with Marfan's syndrome underwent surgery for repair of an ascending aortic dissection. The clinical features of this congenital syndrome and the acute management of its complications are described. As elective surgery is frequently required for ocular, musculo-skeletal or cardiovascular problems in people with Marfan's syndrome, patients must be carefully assessed preoperatively for specific complications. Significant pulmonary problems include restriction of lung function due to pectus excavatum or kyphoscoliosis and intrinsic pulmonary involvement with emphysema, bronchog

Laryngopyocoele in an adult female mimicking an infected branchial cyst.

The incidence of laryngopyocoele is very unusual and only 41 cases have been documented in the world literature. Although hoarseness and stridor are the predominant symptoms associated with laryngocoele, patient can present with a recurrent neck abscess misleading to an infected branchial cyst. Complete excision of the cyst can be effectively achieved by the transcervical approach, especially in case of a large lateral cyst in a stable patient.

[Sonography with color and pulsed Doppler in fetal pulmonary malformations].

OBJECTIVE: Evaluate the advantages of color Doppler and pulsed Doppler in the diagnosis and prognosis of fetal pulmonary malformations. MATERIALS AND METHOD: This retrospective study of 24 fetuses explored pulmonary malformations using sonography. A classification into four groups was made based on the presence of cysts and the echogenicity of the lesion, then analysis of video recordings including Doppler with an interpretation chart: afferent vessel, color chart, spectrum shape (afferent vessel and within the malformation) to determine the contribution of Doppler within each group. Minimally invasive pediatric surgery: Our experience.

AIM: Departmental survey of the pediatric laparoscopic and thoracoscopic procedures. MATERIALS AND METHODS: It is a retrospective study from January 1999 to December 2007. The various types of surgeries, number of patients, complications and conversions of laparoscopic and thoracoscopic procedures were analyzed. RESULTS: The number of minimally invasive procedures that had been performed over the past 9 years is 734, out of which thoracoscopic procedures alone were 48. The majority of the surgeries were appendicectomy (31%), orchiopexy (19%) and diagnostic laparoscopy (16%). The other advanc

[Thoracoscopic resection of mediastinal tumors].

Thoracoscopic surgery was performed in nine patients with benign mediastinal tumors: four neurogenic tumors, three bronchogenic cysts, a foregut cyst and a mature teratoma. Four cystic masses located in para-esophageal lesion in inferior mediastinum and four neurogenic tumors located in posterior mediastinum. A mature teratoma was adjacent to thymus in anterior mediastinum. Under general anesthesia with unilateral ventilation, the patients were turned in the lateral decubitus position and we tilt the operative table to allow gravity assistance for the displacement of the lung. Congenital Cystic Lung Lesions: Redefining the Natural Distribution of Subtypes and Assessing the Risk of Malignancy.

Asymptomatic cystic lung lesions-congenital pulmonary airway malformations (CPAMs), sequestrations, and bronchogenic cysts-are commonly diagnosed prenatally. Indications to resect are to eliminate risk of malignancy or infection. CPAMs consist of a spectrum of malformations, with type 1 historically considered the most common. Mucinous cell clusters, seen almost exclusively in type 1, are premalignant lesions at risk for progression to mucinous adenocarcinoma. We reviewed and classified 2.5 years of consecutive, prenatally diagnosed lesions as extralobar sequestration, intralobar sequestration

Thoracoscopic resection of bulky intrathoracic benign lesions.

BACKGROUND: Video-assisted thoracic surgery (VATS) is used for the diagnosis and treatment of some mediastinal lesions. However, large-size tumours are usually approached by thoracotomy or sternotomy. We report our experience of a full thoracoscopic approach for bulky intrathoracic lesions. METHODS: From November 2002 to March 2007, 14 patients with a bulky intrathoracic mass were referred for resection. The study group consisted of eight females and six males with a mean age of 44 years (range: 13-74). We defined as bulky a mass with a minimal cross-sectional diameter equal to or larger than
