The value of a negative antepartum test: contraction stress test and modified biophysical profile.

OBJECTIVE: To examine the outcome of pregnancies in high-risk patients whose last antepartum fetal assessment was a negative contraction stress test (CST) or a negative modified biophysical profile. METHODS: Twenty-nine hundred ninety-four women who received modified biophysical profiles were compared with 2450 who had CSTs during the preceding 3 years. Pregnancy outcomes were evaluated in patients whose last test was negative. RESULTS: Seventeen hundred fifty-three patients had negative modified biophysical profiles as the last test before delivery, and 1337 had negative CSTs as the last te

Cerebral regional tissue Oxygen Saturation to Guide Oxygen Delivery in preterm neonates during immediate transition after birth (COSGOD III): an investigator-initiated, randomized, multi-center, multi-national, clinical trial on additional cerebral tissue oxygen saturation monitoring combined with defined treatment guidelines versus standard monitoring and treatment as usual in premature infants during immediate transition: study protocol for a randomized controlled trial.

BACKGROUND: Transition immediately after birth is a complex physiological process. The neonate has to establish sufficient ventilation to ensure significant changes from intra-uterine to extra-uterine circulation. If hypoxia or bradycardia or both occur, as commonly happens during immediate transition in preterm neonates, cerebral hypoxia-ischemia may cause perinatal brain injury. The primary objective of the COSGOD phase III trial is to investigate whether it is possible to increase survival without cerebral injury in preterm neonates of less than 32 weeks of gestation by targeting cerebral t

Spontaneous fetal heart rate deceleration: an ominous sign for fetal outcome.

BACKGROUND: The aim of this study was to establish the prognostic import of spontaneous fetal heart-rate deceleration, a sign of fetal distress. METHODS: This retrospective study enrolled 169 patients with fetal heart-rate deceleration treated at Shin-Kong Wu Ho Su Memorial Hospital from 1 January 1998 to 24 November 2000. Reviewed variables included type of fetal heart-rate irregularity (including early, variable, late, and spontaneous variants), neonatal outcome, Doppler results, gestational age and weight at birth, and amniotic fluid index. RESULTS: The spontaneous form of fetal heart-rat

Uncertain value of electronic fetal monitoring in predicting cerebral palsy.

BACKGROUND: Electronic monitoring of the fetal heart rate is commonly performed, in part to detect hypoxia during delivery that may result in brain injury. It is not know whether specific abnormalities on electronic fetal monitoring are related to the risk of cerebral palsy. METHODS: Among 155,636 children born from 1983 through 1985 in four California counties, we identified singleton infants with birth weights of at least 2500 g who survived to three years of age and had moderate or severe cerebral palsy. The children with cerebral palsy were compared with randomly selected control children

Evaluation and Association of Meconium-Stained Amniotic Fluid With Fetal Distress.

Background Meconium-stained amniotic fluid (MSAF) often leads to complicated deliveries ranging from instrumental delivery, cesarean delivery, and neonatal complications like fetal distress, neonatal intensive care unit (NICU) admission, and neonatal death. Therefore, this study aimed to evaluate the prevalence of MSAF in fetal distress, to determine the clinical profile of newborns in terms of risk factors, and to study the association between antenatal or intra-natal risk factors with MSAF, between umbilical cord pH and MSAF, between stage of labor and MSAF, and between fetal distress and MS

Perinatal risk factors. With special reference to intrauterine growth retardation and neonatal respiratory adaptation.

Intrauterine growth retardation (IUGR) and neonatal respiratory disorders (RD) are two dominating problems in perinatal medicine. The aims of this study were to analyze the possibilities of selecting groups of pregnant women with increased risk of either problem, to analyze the relationship between IUGR and neonatal respiratory disorders and to evaluate the influence on neonatal respiratory adaptation of different events during delivery. For these purposes epidemiological methods were mainly used but in paper V an animal model was applied. In the whole population of pregnant women attending th

Antenatal surveillance of fetal growth restriction.

Fetal growth restriction is a complex problem in modern obstetrics. It is a condition of suboptimal fetal growth based on a genetically predetermined potential and affects approximately 5% to 10% of pregnancies. It is traditionally defined as an estimated fetal weight less than the 10th percentile. Those pregnancies that are affected by growth restriction are associated with increased risk of perinatal morbidity and mortality. Because of this increased risk, these pregnancies are monitored more closely to try to identify those fetuses at the greatest risk of fetal demise and initiate delivery

The IDvIP trial: a two-centre randomised double-blind controlled trial comparing intramuscular diamorphine and intramuscular pethidine for labour analgesia.

BACKGROUND: Intramuscular pethidine is routinely used throughout the UK for labour analgesia. Studies have suggested that pethidine provides little pain relief in labour and has a number of side effects affecting mother and neonate. It can cause nausea, vomiting and dysphoria in mothers and can cause reduced fetal heart rate variability and accelerations. Neonatal effects include respiratory depression and impaired feeding. There are few large studies comparing the relative side effects and efficacy of different opioids in labour. A small trial comparing intramuscular pethidine with diamorphin

[Kinetocardiotocographic and Doppler ultrasound findings before termination of risk pregnancies].

42 pregnancies complicated by severe intrauterine growth retardation (IUGR) were monitored between 1991 and 1992 with the kinetocardiotocogram (K-CTG) comparing them with normal pregnancies of the same gestational age. We evaluated the foetal movements and the heart rate patterns establishing a correlation with pulsed Doppler ultrasound findings. There was a significant reduction of foetal activity in cases of IUGR and pathological Doppler sonography (p < 0.01). The reduction of foetal movements was noticed several days prior to pathological FHR patterns. There was a significant reactivity aft

[Delivery after 2 previous cesarean sections. A series of 41 uterine trials].

It has been possible to consider how delivery should be carried out in view of the progress that has been made handling scarred uteruses. Over 21 months 41 tests of uterine function have been authorized in our department out of 67 cases where there were two scars in the uterus (67%). 26 patients delivered vaginally (63.4%) and 5 had Caesarean sections because of failure of the test of the scar. When the relationship between the fetus and the pelvis was satisfactory, the fact that the cervix was not ripe and the presenting part was not engaged, did not prevent carrying out a trial of scar in 78

Intraoperative Neuromonitoring for Spinal Surgery in a Pregnant Patient: Case Report and Literature Review.

We report the strategy of anesthesia and intraoperative neurophysiological monitoring (IONM) in a 29-year-old, 22 weeks pregnant patient posted for surgery for aggressive vertebral body hemangioma. We used propofol and fentanyl-based anesthesia for IONM. Motor-evoked potentials (MEP) and somatosensory-evoked potentials (SSEP) were used to monitor the neural tracts during surgery. Fetal heart rate monitoring was done preoperatively and postoperatively. Train of 8, 75 μs duration pulse, 250-500 Hz stimulus was used for MEP and 30 mA, 200-400 μs, 3-5 Hz was used for SSEP. No new motor or somatose

[Induction of Labour In Growth Restricted and Small for Gestational Age Fetuses - A Historical Cohort Study].

PURPOSE: Induction of labour for small-for-gestational-age (SGA) foetus or intrauterine growth restriction (IUGR) is common, but data are limited. The aim of this trial was therefore to compare labour induction for SGA/IUGR with cases with normal fetal growth beyond the 10th percentile. MATERIAL AND METHODS: This historical multicentre cohort study included singleton pregnancies at term. Labour induction for SGA/IUGR (IUGR group) was compared with cases of fetal growth beyond the 10th percentile (control group). Primary outcome measure was caesarean section rate. RESULTS: The caesarean secti

The association of histologic placental inflammation with category II fetal heart tracings.

The present study assessed whether placentas in women delivered by cesarean for category II fetal heart tracings (FHT) exhibit a higher incidence of acute inflammation than those of women delivered by cesarean for labor arrest. This case control study included singleton pregnancies ≥36 weeks of gestation delivered by cesarean for an FHT indication (cases) or because of labor arrest (controls) 2005-2009 at Prentice Women's Hospital. Exclusions were maternal diabetes, hypertension, known thrombophilia, connective tissue disorders, clinical evidence of chorioamnionitis, placental abruption, fetal

Diastolic zero-flow in the umbilical artery in twin pregnancies.

Clinical arterial blood flow measurements in single pregnancies can not be precisely estimated yet. ARED (absent or reverse end diastolic) flow of the umbilical artery (UA) commonly indicates a symptom of fetal jeopardy. The interpretation of blood flow measurement in twin pregnancies is still controversial. On one hand, no differences in a single pregnancies are found, and on the other hand, increased resistance indices have been reported. In the feto-fetal transfusion syndrome mostly there are normal blood flow measurements. When pathological blood flow occurs, usually it affects the donor.

Death of one twin followed by extremely variable flow velocity waveforms in the surviving fetus.

Intrauterine death of one fetus after the second trimester in a twin pregnancy, with continuation of the pregnancy is a rare complication. The risks of morbidity and mortality for the surviving fetus are high. A 32-year-old woman was admitted to the antenatal ward at 27 weeks gestation because of intrauterine death of one twin. During the first 24 h after the death of one twin, Doppler ultrasound assessment showed a remarkable variability in flow velocity waveforms in the umbilical artery of the surviving fetus. Changes from reversed to normal end-diastolic flow velocities were recorded within

Cutoffs that Count: Insights into Receiver Operating Characteristic Curves for Cord Blood pH, Lactate, and Base Excess from an Indian Tertiary Centre.

BACKGROUND: Reading heart rate patterns during labor (cardiotocography) can be difficult and does not always predict when babies develop acid buildup in their blood, especially when oxygen levels drop quickly during delivery. In India, we lack enough research comparing these heart rate patterns with blood chemistry tests from the umbilical cord, making it harder for doctors to make the best decisions during childbirth. OBJECTIVE: We wanted to study how abnormal heart rate patterns during labor relate to blood chemistry levels in the umbilical cord. Our goal was to find better ways to use lact
