Stroke often leads to hemiplegia, significantly impairing walking ability. Traditional rehabilitation methods commonly use mechanical knee-ankle-foot orthoses (KAFOs) to stabilize gait, but these can restrict knee flexion and reduce walking efficiency. This study aimed to evaluate the feasibility and effectiveness of an Electric KAFO (E-KAFO) for stroke survivors with hemiplegia, using the Berg Balance Scale to determine its impact on walking function. Fourteen individuals with hemiplegia (mean age: 55.1 ± 13.7 years; gender: 12 males, 2 females; height: 168.2 ± 7.7 cm; weight: 67.1 ± 11.5 kg;

Stroke-induced hemiplegia is a major cause of long-term disability, often leading to lower limb deformities and abnormal gait. Ankle-foot orthoses (AFO) have shown effectiveness in improving these conditions, but limited research explores the benefits of combining AFO therapy with balance training. This study aimed to explore the effects of combining balance training exercises with orthotic intervention on various gait characteristics in stroke patients with lower limb paralysis. This randomized controlled trial (RCT) involved 32 patients, 12-18 weeks post-stroke, randomized into two groups: b

Functional electrical stimulation of the ankle dorsiflexor (DF-FES) may have advantages over ankle foot orthoses (AFOs) in managing pediatric cerebral palsy (CP). This study assessed the functional benefit and orthotic effect of DF-FES in children with hemiplegic CP. We conducted an open-label prospective study on children with hemiplegic CP ≥ 6 years who used DF-FES for five months. The functional benefit was assessed by repeated motor function tests and the measurement of ankle biomechanical parameters. Kinematic and spatiotemporal parameters were assessed by gait analysis after one and five

The ability to navigate stairs step-over-step is an important functional outcome following severe lower leg injury and is difficult for many patients. Ankle-foot orthoses, such as the Intrepid Dynamic Exoskeletal Orthosis (IDEO), are often prescribed to improve function. This study compared stair climbing mechanics between IDEO users and able-bodied control participants. Thirteen IDEO users who sustained severe lower leg injury and 13 controls underwent biomechanical gait analysis. Participants ascended and descended a 16-step instrumented staircase without handrail use at a controlled cadence

BACKGROUND: It has previously been discussed that treatment of the hemiplegic arm in patients with cerebral palsy can improve gait parameters in the lower body. Our question was whether improving the ankle rocker with an orthosis has an effect on the upper body during walking. The main aim was to investigate, which trunk and arm kinematics of toe walking children with hemiplegic cerebral palsy are changed by wearing a hinged ankle-foot orthosis, restoring an initial heel contact.
METHODS: Specific parameters of the pelvis, thorax, and arm kinematics were investigated. Differences in the hemip

Ponseti treatment for clubfoot has been successful, but recurrence continues to be an issue. After correction, patients are typically braced full time with a static abduction bar and shoes. Patient compliance with bracing is a modifiable risk factor for recurrence. We hypothesized that the use of Mitchell shoes and a dynamic abduction brace would increase compliance and thereby reduce the rate of recurrence. A prospective, randomized trial was carried out with consecutive patients treated for idiopathic clubfeet from 2008 to 2012. After casting and tenotomy, patients were randomized into eithe

Gait abnormalities are common in the older population owing to aging- and disease-related changes in physical and neurological functions. Differentiating the causes of gait abnormalities is challenging because various abnormal gaits share a similar pattern in older patients. Herein, we propose a deep neural network (DNN) model to classify disease-specific gait patterns in older adults using commercialized instrumented insoles. This study included 150 patients aged ≥ 65 years, divided into the following five groups (N = 30 in each group): healthy older individuals (HI), patients with Parkinson'

BACKGROUND: New technologies from the field of mobile health (mHealth) are increasingly used to improve patient monitoring during rehabilitation. While in recent years, mobile phones, health apps, personal digital assistants, and smartwatches opened up new diagnostic and monitoring opportunities for patients, the development of innovative sensor devices, such as instrumented insoles, has now reached a sufficient level of usability with promising opportunities for clinical practice. According to research on the best method for monitoring recovery after musculoskeletal injury or surgery, the Pat

Adjusting the range of motion (ROM) and spring stiffness of ankle-foot orthoses (AFOs) for individuals post-stroke enables customized functionality and targeted support during specific phases of the gait cycle. Modifications to dorsiflexion ROM or spring stiffness theoretically influences the second and third rockers of gait. Understanding these effects is crucial for optimizing gait in individuals post-stroke. This study investigated the impact of dorsiflexion ROM adjustments in multi-function articulated AFOs on ankle, knee, and hip kinematics during gait in individuals post-stroke. Nine par

BACKGROUND: Motor coordination concerns are estimated to affect 5%-6% of school-aged children. Motor coordination concerns have variable impact on children's lives, with gait and balance often affected. Textured insoles have demonstrated positive impact on balance and gait in adults with motor coordination disorders related to disease or the ageing process. The efficacy of textured insoles in children is unknown. Our primary aim was to identify the feasibility of conducting a randomised controlled trial involving children with motor control issues. The secondary aim was to identify the limited
