[Diagnosis in persistent vertigo after stapes surgery].

One hundred sixty-two otosclerotic patients who underwent a stapedotomy between October 1991 and April 1994 were examined after experiencing postoperative vertigo. Twenty-eight complained of dizziness. In sixteen patients, we observed vertigo and nystagmus only within the first six postoperative days; in these cases, special treatment was not necessary. However, in twelve patients in whom vertigo persisted even after their stay in the hospital, the problem seriously affected their quality of life and their return to work. In these cases, an individualized diagnostic and therapeutic procedure w

Facial nerve monitoring in surgery for congenital auricular atresia.

Surgery for congenital auricular atresia places the facial nerve at risk because of its potential aberrancy; iatrogenic facial paralysis is a well-known potential risk. Five cases of congenital auricular atresia are presented. All patients had intraoperative monitoring of the facial nerve. The facial nerve was found to be abnormally displaced, usually due to hypoplasia of the tympanic ring. Preoperative and postoperative facial nerve function was unchanged by surgery. Facial nerve monitoring is an important adjunct in congenital ear surgery because it allows for early and precise identificatio

Comparison of hearing performance in patients with borderline indication for active middle ear or cochlear implants: clinical outcomes to guide preoperative counseling and decision making.

PURPOSE: The aim of the presented study was to compare the audiological benefit achieved in cochlear implant (CI) patients who, in principle, could still have been treated with an active middle ear implant (AMEI) with a group of AMEI users. METHODS: Results of 20 CI patients with a pure-tone average (PTA) of 70 dB HL prior to surgery were compared with a group of 12 subjects treated with a Vibrant Soundbridge (VSB). Pre-surgical comparison included PTA for air conduction and bone conduction, maximum speech recognition score for monosyllabic words (WRSmax), and aided monosyllabic word recognit

Surgery or implantable hearing devices in children with congenital aural atresia: 25 years of our experience.

OBJECTIVES: Congenital aural atresia and ear deformities have been the subject of serious discussions for centuries. These malformations are associated with significant aesthetic and functional problems. Outcome of the surgical solution is rarely optimal. Despite the gradual improvement of surgical techniques the surgery still remains associated with very limited short-term and mainly long-term functional outcome. Therefore, the priority treatment in modern otology becomes implantable devices--BAHA, Bonebridge and active middle ear implants. METHODS: The functional and aesthetic outcomes of a

Active middle ear implants in patients undergoing subtotal petrosectomy: new application for the Vibrant Soundbridge device and its implication for lateral cranium base surgery.

OBJECTIVE: The functional outcome of ossiculoplasties in chronic ear and lateral cranium base surgery depends on the presence of a ventilated middle ear space and is guided by the existence or absence of ossicular remnants. In patients with poorly ventilated middle ears, after multiple previous operations, missing stapes suprastructure, or after partial temporal bone resection for tumor removal, restoration of conductive hearing is not possible. The direct placement of a vibrating floating mass transducer (FMT) onto the round window membrane with obliteration of the surgical cavity is a new op

First experience with a new stapes clip piston in stapedotomy.

OBJECTIVE: Hearing results after 23 implantations of a newly designed titanium-clip stapes piston prosthesis (the àWengen Clip Piston prosthesis) in patients with otosclerosis were evaluated. This new type of stapes piston was designed to avoid the crimping onto the incus in stapedotomy. This one clip fits all designs and enables solid fixation by clicking the prosthesis onto the long process of the incus without crimping. STUDY DESIGN: A retrospective pilot study was carried out by microcomputer of the preoperative and postoperative audiological results of patients in whom the titanium-clip

[Morphological and functional results of Palisade Cartilage Tympanoplasty].

BACKGROUND AND OBJECTIVE: The application of cartilage in tympanoplasty has been generally accepted, because cartilage as a bradytrophic tissue allows stable and functionally reliable reconstruction of the eardrum even in difficult pathological conditions (such as subtotal defects, tympanosclerosis etc.). A special surgical technique using small cartilaginous chips for the reconstruction of the eardrum has been developed by J. Heermann, who introduced it as Palisade Cartilage Tympanoplasty (PCT). Although being increasingly applied in otosurgery, this technique has to date neither been evaluat

Bone pate repair of the eroded incus--five years on.

Seventy-four patients undergoing bone pate repair of the long process of the incus as part of a tympanoplasty are presented. Mean follow-up time was 66 months (range 12-143 months). Hearing results are averaged over the frequencies 500, 1000, 2000 and 4000 Hz. Thirty-two per cent of the patients had hearing thresholds better than 30 dB pre-operatively. A significant improvement in hearing was seen (p < 0.001). The data were also analysed to assess the effect of the pre-operative state of the tympanic membrane on the final result. The pre-operative condition of the tympanic membrane was either

Vibrant Soundbridge Round Window Vibroplasty: Safety, Coupling Efficiency, and Speech Outcome of the Most Common Coupling Modalities.

UNLABELLED: <p>Introduction: The floating mass transducer (FMT) of the Vibrant Soundbridge (VSB) is successfully used for reverse stimulation of the round window membrane (RWM) since 2005, enabling the treatment of conductive and mixed hearing loss. The FMT can be placed directly to the RWM or indirectly by using an interposed material such as fascia and Tutopatch, or a coupler, e.g., the round window coupler (RWC), the round window soft (RWS) coupler, or the custom-made Hannover coupler version 2 (HCV2). The aim of the study was to retrospectively evaluate the long-term outcome of the most co

Stapes surgery in osteogenesis imperfecta: a clinical study of 16 patients.

Osteogenesis Imperfecta (OI), is a genetic disease of connective tissue, with the main feature of bone fractures, accompanied by blue sclerae and hearing loss. Hearing loss affects about 50% of the patients, beginning in the second and third decade of life. The hearing loss is progressive, starting with a conductive loss, then a mixed and later on a sensorineural loss. There are similarities between otosclerosis, although OI is a distinct entity. The diseases are treated in the same surgical way. The result is, however, not as good in OI as in otosclerosis according to different reports. A scr

[The Vibrant Soundbridge as an active implant in middle ear surgery].

Implantable hearing aids are not only gaining importance for the treatment of sensorineural hearing loss, but also for treatment of mixed hearing loss. The most frequently used active middle ear implant is the Vibrant Soundbridge (VSB) system (Fa. MED-EL, Innsbruck, Österrreich). Following widening of the spectrum of indications for the VBS, various new coupling systems have been established. Based on the literature, available petrosal bone investigations and finite element model (FEM) calculations, this article summarizes the current knowledge concerning mechanical excitation by the VSB.
