Reactive astrocyte formation in vivo is regulated by noradrenergic axons.

Beta adrenergic receptor antagonists greatly reduce reactive astrocyte formation induced by neuronal degeneration. To test the hypothesis that the density of noradrenergic innervation is a factor in the regulation of astrocytosis, we measured glial fibrillary acidic protein (GFAP) optical density after neuronal injury in central nervous system (CNS) regions with permanent noradrenergic sprouting or norepinephrine (NE) depletion. The injury model employs the injection of Ricinus communis lectin into a cranial or peripheral nerve to destroy CNS neurons without the blood-brain barrier disruption

Postoperative magnetic resonance imaging may predict poor outcome in children with severe traumatic brain injuries who undergo cranial surgery.

OBJECTIVE: Multiple studies have evaluated the use of MRI for prognostication in pediatric patients with severe traumatic brain injury (TBI) and have found a correlation between diffuse axonal injury (DAI)-type lesions and outcome. However, there remains a limited understanding about the use of MRI for prognostication after severe TBI in children who have undergone cranial surgery. METHODS: Children with severe TBI who underwent craniectomy or craniotomy at Primary Children's Hospital in Salt Lake City, Utah, between 2010 and 2019 were identified retrospectively. Of these 92 patients, 43 unde

Intracranial flow patterns at increasing intracranial pressure.

In the course of a pilot study, changes in intracranial pressure were compared with the transcranial Doppler findings of the middle cerebral artery. The cases of five patients were discussed who developed dissociated brain death in spite of intensive therapeutic measures. The studies showed that changes of the intracranial pressure influenced the flow patterns considerably: at increasing intracranial pressure (decreasing cerebral perfusion pressure) a progressive reduction of the systolic and above all diastolic flow velocities and finally a pendular flow occurred. These changes could be recor

Management and outcomes of pediatric traumatic brain injury in Africa: a systematic review.

OBJECTIVE: Traumatic brain injury (TBI) carries a major global burden of disease; however, it is well established that patients in low- and middle-income countries, such as those in Africa, have higher mortality rates. Pediatric TBI, specifically, is a documented cause for concern as injuries to the developing brain have been shown to lead to cognitive, psychosocial, and motor problems in adulthood. The purpose of this study was to investigate the reported demographics, causes, management, and outcomes of pediatric TBI in Africa. METHODS: A literature search was conducted using PubMed, Global

How reliable is the predictive value of SEP (somatosensory evoked potentials) patterns in severe brain damage with special regard to the bilateral loss of cortical responses?

OBJECTIVE: In patients sustaining severe brain damage (SBD), prediction of later outcome is often very difficult, in particular under conditions of therapeutic management like relaxation and intravenous phenobarbital. Somatosensory evoked potentials (SEP) correlated best with later prognosis and expected neurological deficit. In detail, the primary bilateral loss of cortical responses (BLCR) is regarded to be a reliable marker for poor prognosis. The goal of the following prospective study was to reassess the prognostic value of early serial median nerve SEP recording in severe brain damage in

Hemorrhagic Cholecystitis in an Elderly Patient With Fall Syndrome: A Case Report.

Hemorrhagic cholecystitis is a rare, potentially fatal complication of acute cholecystitis that develops with or without gallstones. It tends to be diagnosed late because its clinical presentation overlaps with that of other acute abdominal conditions, and suspicion is often low, as it is first a ruling-out diagnosis due to its low incidence. We present the case of an 81-year-old man with epilepsy, heavy tobacco use, and alcohol consumption, who ambulated with crutches due to a previous left ankle fracture. He was admitted following a ground-level fall caused by a tonic-clonic seizure, sustain

Low Utility and Yield of Routine Outpatient CT Imaging in Patients With Mild Traumatic Brain Injury and Intracranial Hemorrhage.

OBJECTIVE AND BACKGROUND: This study aims to better characterize the utility of CT scan imaging when seeing mild traumatic brain injury (TBI) patients managed non-operatively in a clinic. The benefit of routinely scheduling outpatient head CT for patients discharged with mild TBI and intracranial hemorrhage (ICH) remains unclear. Unselective imaging increases cost, scanner demand, and cumulative radiation exposure. METHODS: We performed a retrospective cohort study of 100 consecutive adults with mild TBI (Glasgow coma scale (GCS) 13-15) and non‑operative ICH who were admitted between January

Evidence-based indications for repeat head CT after mild traumatic brain injury: a systematic review and meta-analysis.

The effectiveness of routine repeat head computed tomography (CT) for mild traumatic brain injury (mTBI) is a subject of ongoing debate, with considerable variation in clinical practices. This systematic review and meta-analysis aimed to assess the evidence comparing routine and selective repeat CT protocols for mTBI patients. A comprehensive search was performed in PubMed and the Cochrane Library for studies published in the last two decades. The studies included adult mTBI patients who underwent repeat head CT during their hospital stay. The primary outcome measured was the necessity for neu

Selective Inhibitory Circuit Dysfunction after Chronic Frontal Lobe Contusion.

Traumatic brain injury (TBI) is a leading cause of neurologic disability; the most common deficits affect prefrontal cortex-dependent functions such as attention, working memory, social behavior, and mental flexibility. Despite this prevalence, little is known about the pathophysiology that develops in frontal cortical microcircuits after TBI. We investigated whether alterations in subtype-specific inhibitory circuits are associated with cognitive inflexibility in a mouse model of frontal lobe contusion in both male and female mice that recapitulates aberrant mental flexibility as measured by

A Novel Decision-Support Tool (IniCT Score) for Repeat Head Computed Tomography in Pediatric Mild Traumatic Brain Injury.

BACKGROUND: The necessity of computed tomography (CT) has been questioned in pediatric mild traumatic brain injury (mTBI) because of concerns related to radiation exposure. Distinguishing patients with lower and higher risk of clinically important TBI (ciTBI) is paramount to the optimal management of these patients. OBJECTIVE: This study aimed to analyze the imaging predictors of ciTBI and develop an algorithm to identify patients at low and high risk for ciTBI to inform clinical decision making using a large single-center cohort of pediatric patients with mTBI. METHODS: We retrospectively i

Effect of amplitude and duration of impulsive pressure on endothelial permeability in in vitro fluid percussion trauma.

BACKGROUND: Intracranial pressure changes during head impact cause brain injuries such as vasogenic edema and cerebral contusion. However, the influence of impulsive pressure on endothelial function has not yet been fully studied in vitro. In this study, we developed a pressure loading device that produced positive and negative pressures by modifying an in vitro fluid percussion model and examined the effects of the amplitude and duration of the pressures on endothelial permeability. METHODS: Human umbilical vein endothelial cells were subjected to three types of positive pressure (average am

Effect of combined treadmill training and magnetic stimulation on spasticity and gait impairments after cervical spinal cord injury.

Spasticity and gait impairments are two common disabilities after cervical spinal cord injury (C-SCI). In this study, we tested the therapeutic effects of early treadmill locomotor training (Tm) initiated at postoperative (PO) day 8 and continued for 6 weeks with injury site transcranial magnetic stimulation (TMSsc) on spasticity and gait impairments after low C6/7 moderate contusion C-SCI in a rat model. The combined treatment group (Tm+TMSsc) showed the most robust decreases in velocity-dependent ankle torques and triceps surae electromyography burst amplitudes that were time locked to the i

Are we underestimating the burden of traumatic brain injury? Surveillance of severe traumatic brain injury using centers for disease control International classification of disease, ninth revision, clinical modification, traumatic brain injury codes.

BACKGROUND: The epidemiology of traumatic brain injury (TBI) is often studied through the use of International classification of disease, ninth revision, clinical modification (ICD-9-CM), diagnosis codes from the Centers for Disease Control and Prevention TBI Surveillance System. Recent studies suggest that these codes may underestimate the burden of TBI because of inaccuracies and low sensitivity. OBJECTIVE: To determine the sensitivity and specificity of ICD-9-CM codes in a severe TBI population. METHODS: We retrospectively reviewed medical records of all hospital admissions including comp

Serum fibrinogen level and fibrinogen administration in patients with traumatic brain injury: A systematic review and meta-analysis protocol.

INTRODUCTION: Traumatic Brain Injury (TBI) is a leading cause of disability and death globally. It has a significant economic burden. Coagulopathy has been identified as one of the key factors contributing to the poor outcomes observed in TBI patients, and it has been theorised that the management of coagulopathy will improve patient outcomes. Low serum fibrinogen levels denote a coagulopathic state, and the therapeutic administration of fibrinogen has been proposed to correct this state. However, there is no consensus on its efficacy in patients with TBI. Hence, this systematic review and met
