UNLABELLED: Long-term results of surgical treatment for congenital talipes equinovarus are documented in the literature but mainly in correlation with clinical and radiographic objective criteria. We present a retrospective study of 48 patients who underwent surgical correction (extended Cincinnati type "a la carte" approach) in our department with an average of 17 years' follow-up. Patients were interviewed and completed a validated disease specific instrument, which evaluates foot function as well as patients' satisfaction, the use of orthotics, shoe size, and the impact of this operation in

This study compared functional and physiologic measures of ambulation and upright mobility with functional electrical stimulation (FES) versus knee-ankle-foot-orthoses (KAFO) in an 11-year-old boy with a T-10 level spinal cord injury. The child was a limited community ambulator with bilateral KAFO and loftstrand crutches. The FES system consisted of percutaneous intramuscular electrodes controlled by a portable stimulator and thumbswitch, an AFO for ankle and foot support, and loftstrand crutches. The subject used a swing-through gait pattern with both modes of mobility. The Functional Indepen

Forefoot pain in the adult often alters mobility and has a negative impact on quality of life. Metatarsalgia describes pain localized to the forefoot. Forefoot pain may be caused by conditions of the lesser toes themselves (eg, hammertoes, mallet toes, claw toes). The pathophysiology of lesser toe deformities is complex and is affected by the function of intrinsic and extrinsic muscle units. In addition to lesser toe and metatarsal abnormality, forefoot pain can be attributed to interdigital neuritis, disorders of the plantar skin, and gastrocsoleus contracture. Treatment of these conditions m

Pain is a frequent symptom in people with inflammatory arthritis (IA), which has substantial impact on their quality of life. Analyses of electronic health record data indicate that UK pain care in people with IA often involves prescribing long-term opioids and gabapentinoids, despite absent trial evidence for efficacy. Patient survey data suggest that non-pharmacological pain care with supportive trial evidence is underused. A UK-specific guideline on pain management for people with IA is required to address this. This comprehensive life-course guideline is the first British Society for Rheum

Fifty patients with spastic diplegic cerebral palsy were included in this retrospective study which compared visual assessment of gait to three-dimensional (3D) gait analysis. Inter-observer variability was evaluated as well. Inclusion criteria comprehended independent ambulation (i.e. without assistive devices or orthoses). All subjects went through 3D gait analysis at the Gait Analysis Laboratory of the AACD Hospital. Four observers, viewing videotaped gait cycles, evaluated 10 specific points of interest of the cycle: hip flexion at terminal stance; knee flexion at initial contact; knee ext

BACKGROUND: Cerebral palsy is the commonest cause of severe physical disability in childhood. For many years treatment has centred on the use of physiotherapy and orthotics to overcome the problems of leg spasticity, which interferes with walking and can lead to limb deformity. Intramuscular botulinum toxin (BT-A) offers a targeted form of therapy to reduce spasticity in specific muscle groups.
AIMS: To determine whether intramuscular BT-A can improve walking in children with cerebral palsy.
DESIGN: Randomised, double blind, placebo controlled trial.
METHODS: Forty patients with spastic dip

A personal prospective study of 98 consecutive patients presenting with neurological impairment and fractures or dislocations between the 9th thoracic and 2nd lumbar vertebrae bodies. Fifty-three patients underwent Harrington instrumentation, and 45 patients were treated recumbently. Neurological improvement was much better following Harrington rods in the complete paraplegia group but there was no difference in neurological recovery between the two groups in those with incomplete paraplegia. Forty-two patients who had been stabilised with Harrington rods underwent post-operative myelography o

UNLABELLED: The aim of this study was to explore the efficacy of a minimalist stiff insole in limiting/reducing first metatarsophalangeal joint (1MTP) range of motion and to explore the impact on the ankle joint during walking. It was hypothesized that the minimalist stiff insole would significantly reduce 1MTP kinematic and kinetic parameters whilst causing compensatory effects at the ankle joint.
METHODS: Fourteen healthy participants were recruited. Three-dimensional lower limb and forefoot kinematics were measured during walking at a self-selected speed with a stiff and control insole. Di
