A NEW TECHNIQUE FOR COMBINED ANTERIOR CRUCIATE AND ANTEROLATERAL LIGAMENT RECONSTRUCTION USING QUADRICEPS AND PLANTARIS TENDONS.

Anterior cruciate ligament (ACL) reconstructions with quadriceps tendon (QT) grafts are gaining popularity, both in primary and revision procedures. Recently, the role of the anterolateral ligament (ALL) of the knee in improving rotational knee stability has been emphasised and concurrent ACL and ALL reconstruction is advocated. In this paper, a new technique utilising the QT and the plantaris tendon (PLT) for combined ACL and ALL reconstruction is analysed. Patients that underwent combined ACL and ALL reconstruction using QT and PLT grafts in a 3-years period were prospectively analysed. A to

Vertical jump impulse deficits persist from six to nine months after ACL reconstruction.

Later-stage rehabilitation following anterior cruciate ligament (ACL) reconstruction (ACLR) provides a valuable opportunity to target performance deficits before return to sport. This study aimed to: (1) evaluate bilateral counter-movement jump (CMJ) phase-specific impulse and isokinetic strength inter-limb asymmetry progression from 6 to 9 months post-ACLR; and (2) examine the extent to which individual changes in strength asymmetry could explain changes in impulse asymmetry. Male athletes (n = 44) with a hamstring tendon or bone-patellar tendon-bone autograft were tested 6 and 9 months post-

Combined medial patellofemoral and medial patellotibial reconstruction for patellar instability: a PRISMA systematic review.

BACKGROUND: The medial patellofemoral ligament (MPFL) works in association with the medial patellotibial ligament (MPTL) and the medial patellomeniscal ligament (MPML) to impart stability to the patellofemoral joint. The anatomy and biomechanical characteristics of the MPFL have been well described but little is known about the MPTL and MPML. Several reconstruction procedures of the MPFL with semitendinosus, gracilis, patellar and quadriceps tendons, allografts and synthetic grafts have been described. No clear superiority of one surgical technique over another is evident. METHODS: A systemat

One-stage posterior cruciate ligament inlay reconstruction combining anterior cruciate ligament reconstruction following knee dislocation.

Knee dislocation is a rare but severe injury that involves damage to the anterior cruciate ligament (ACL), posterior cruciate ligament (PCL), lateral or medial ligamentous structures, and other soft tissues or bony structures surrounding the knee joint. No consensus exists regarding the best treatment method. This work presents a method of 1-stage ACL and PCL reconstruction in which a contralateral quadriceps tendon-bone autograft with tibial inlay technique is used for the PCL and contralateral hamstring tendon autograft with suspension fixation is used for the ACL. After harvesting grafts, t

Gross osteolytic tibia tunnel widening with the use of Gore-Tex anterior cruciate ligament prosthesis: a radiological, arthrometric and clinical evaluation of 17 patients 13-15 years after surgery.

BACKGROUND: Postoperative widening of the bone tunnels have been found after anterior cruciate ligament reconstruction using autologus bone-patellar tendon-bone or hamstring tendon grafts. These changes seem to be of no clinical significance in a short to midterm follow-up. We investigated if a synthetic graft evokes the same bone tunnel widening and if it is of clinical significance in a longterm follow-up. METHODS: We examined 17 patients, 13-15 years after their anterior cruciate ligament reconstruction using a Gore-Tex ligament prosthesis. The follow-up consisted of clinical examination,

Shear wave elastography for assessing lower extremity muscle stiffness after anterior cruciate ligament reconstruction: A systematic review.

BACKGROUND: Anterior cruciate ligament reconstruction (ACLR) restores passive joint stability, yet athletes who meet strength benchmarks may still land stiffly, hesitate to push off, and re-injure. Muscle stiffness governs how rapidly force is transmitted to the joint, and shear wave elastography (SWE) quantifies it muscle by muscle, but SWE findings after ACLR have not been synthesized. METHODS: Scopus, PubMed, SPORTDiscus, and CINAHL were searched through November 2025 following PRISMA 2020. Eligible studies compared SWE-derived lower-extremity muscle stiffness after ACLR with contralateral

Anterior Cruciate Ligament Reconstruction: A 2015 global perspective of the Magellan Society.

BACKGROUND/OBJECTIVE: The Magellan Society is composed of > 150 high-volume fellowship-trained sports surgeons from four continents. These surgeons represent their regional sports organisations in travelling fellowships and are considered to be opinion leaders in their respective organisations. Prior to the 2014 Magellan Meeting in Arima, Japan, a survey was conducted to understand how Magellan members perform anterior cruciate ligament (ACL) reconstruction. This study aims to better understand how ACL reconstruction is performed by sports surgeons worldwide and to determine differences in sur

Hamstring muscle shortens actively during the late swing phase of high-speed running.

The purpose of this study was to investigate the muscle-tendon unit (MTU) and fascicle behavior of the biceps femoris long head (BFlh) along with the activation during the swing phase of high-speed running. Nine active males ran on a treadmill at 70, 80, and 90 % of their maximum running speed. Kinematics of the hip and knee joints were collected during running. The fascicle length of the BFlh was monitored using a B-mode real-time ultrasound imaging system. Muscle activation of the BFlh was simultaneously recorded using surface electromyography. During the swing phase of running, the changes

Ischiofemoral impingement: Clinical perspectives for enhancing diagnosis, and rehabilitation.

INTRODUCTION: Buttock pain often presents a diagnostic dilemma for healthcare professionals, with misdiagnosis resulting in poor treatment outcomes. Ischiofemoral impingement (IFI) - impingement of the quadratus femoris between the ischium and lesser trochanter - is one of the most overlooked causes of buttock pain. Ischiofemoral impingement is most common in females and results in pathology within the quadratus femoris muscle, with additional changes sometimes present within the iliopsoas and proximal hamstring tendons, and the adjacent sciatic nerve. Structural risk factors that narrow the i

A biomechanical comparison of tibial back side fixation between suspensory and expansion mechanisms in trans-tibial posterior cruciate ligament reconstruction.

Soft tissue grafts have attracted increasing attention in anterior cruciate ligament (ACL) reconstruction and offer a number of advantages Therefore, it can also be attractive for trans-tibial PCL reconstruction, if four-strand hamstring or two-strand tibialis grafts could be converted for this use. We intended to investigate the biomechanical properties of fixation devices that are frequently used for the soft tissue graft in the trans-tibial PCL reconstruction.Thirty-six fresh adult porcine knees were used in this study. Porcine digital extensor tendons were used as two-stranded soft tissue

Anterior cruciate ligament injuries in patients with open physes.

From July 1988 to August 1989, six children with open physes and injuries to the anterior cruciate ligament were treated operatively. All injuries involved twisting episodes during sports activities. All six patients had meniscal abnormalities, and, additionally, one patient sustained a Grade III medical ligament tear and had a lateral patellar dislocation. Operative reconstruction used hamstring tendons and place a groove over the front of the tibia and a groove over the top of the femur without violation of the growth plates. The anterior cruciate ligament was primarily repaired (three patie

Indirect transfer of the sternal head of the pectoralis major with autogenous semitendinosus augmentation to treat scapular winging secondary to long thoracic nerve palsy.

BACKGROUND: Scapular winging resulting from long thoracic nerve palsy is uncommon but debilitating, and the choice of surgical treatment is inconsistent. The autogenous semitendinosus tendon plays a key role as an interposed tendon graft, although its use in the indirect transfer of the sternal head of the pectoralis major during the treatment of scapular winging has rarely been reported. MATERIALS AND METHODS: A retrospective review was performed during a 9-year period from the clinical data of 26 cases (28 shoulders) with indirect transfer of the sternal head of the pectoralis major with th

Does Knee Flexion Influence the Relationship between the Femoral Tunnel and the Lateral Anatomic Structures During ACL Reconstruction?

BACKGROUND: In ACL reconstruction, the relationship of the femoral tunnel outlet to the anatomy of the lateral knee is clinically important, but whether that relationship is influenced by knee flexion using a transportal technique is unclear. QUESTIONS/PURPOSES: The purposes of this study were to (1) to describe the relationship between the outlet of the femoral tunnel and the lateral anatomic structures of the femur, including the lateral epicondyle, lateral collateral ligament, popliteus tendon, and lateral head of the gastrocnemius, as a function of knee flexion angle when the tunnel was c

Geometric topology of multi-strand ACL graft constructs: A mechanical framework and exploratory clinical analysis.

BACKGROUND: Hamstring graft preparation in anterior cruciate ligament (ACL) reconstruction varies widely, with strand number often determined by tendon length or surgeon preference. The influence of internal strand geometry on graft mechanical behaviour and postoperative stability remains poorly understood. METHODS: A geometric-mechanical framework was developed to characterize ACL graft constructs composed of two to six strands within a circular femoral tunnel. Spatial strand coordinates were used to derive four theoretical geometric descriptors: Symmetry Index (SI), effective polar moment o
