Persistent and recurrent conductive deafness following stapedotomy.

BACKGROUND: Stapedotomy produces the best hearing results of any otological operation. However, in a small number of cases the air-bone gap is not successfully closed, or conductive hearing loss recurs. OBJECTIVE: To investigate the proportion of cases in the senior author's series which required revision surgery and to determine the medium to long term success rate of surgery, taking into account the results of revision surgery. METHODS: Review of a series of 233 primary stapedotomy operations and 100 revision procedures carried out by one surgeon over a 20-year period. RESULTS: Closure of

Histopathology of ossicular implants.

Ossicular and cortical bone grafts maintain their morphologic structure for long periods of time and show varying amounts of replacement of nonviable bone by new bone through a slow process of creeping substitution. Cartilage grafts develop chondromalacia, lose stiffness, and tend to be resorbed over time. Plastipore prostheses elicit foreign body giant cell responses with microscopic biodegradation of the implants. There is a great need for the study of well-documented human temporal bone cases with in situ ossicular implants.

Effects of middle-ear disorders on power reflectance measured in cadaveric ear canals.

OBJECTIVE: Reflectance measured in the ear canal offers a noninvasive method to monitor the acoustic properties of the middle ear, and few systematic measurements exist on the effects of various middle-ear disorders on the reflectance. This work uses a human cadaver-ear preparation and a mathematical middle-ear model to both measure and predict how power reflectance R is affected by the middle-ear disorders of static middle-ear pressures, middle-ear fluid, fixed stapes, disarticulated incudostapedial joint, and tympanic-membrane perforations. DESIGN: R was calculated from ear-canal pressure m

On the use of 3D modeling, reconstruction and printing techniques for the development of a total ossicular replacement prosthesis: a case study of cholesteatoma.

The middle ear, which lies between the external auditory canal and the inner ear (cochlea), comprises the tympanic membrane, the ossicular chain (i.e. malleus, incus, and stapes), as well as the associated muscles, ligaments, and the middle ear cavity. Its primary function is to transmit vibratory energy (sound pressure) from the air to the cochlear fluids via the ossicular chain. This part of the ear can be damaged by cholesteatoma, which can affect all three ossicles, necessitating ossiculoplasty to restore sound transmission. Ossiculoplasty is the preferred intervention for restoring the me

The impact of tympanic membrane perforations on middle ear transfer function.

PURPOSE: Injury or inflammation of the middle ear often results in the persistent tympanic membrane (TM) perforations, leading to conductive hearing loss (HL). However, in some cases the magnitude of HL exceeds that attributable by the TM perforation alone. The aim of the study is to better understand the effects of location and size of TM perforations on the sound transmission properties of the middle ear. METHODS: The middle ear transfer functions (METF) of six human temporal bones (TB) were compared before and after perforating the TM at different locations (anterior or posterior lower qua

Auditory ossicles in ScanningElectron Microscopy.

<b>Introduction: </b>Knowledge about the physiology of a healthy middle ear is essential for understanding the activity and mechanics of the ear as well as the basics of ossiculoplasty. Trauma of the epithelial lining of the tympanic cavity as well as the ossicular chain may be the result of chronic inflammation and surgery. Depending on the observed changes of the middle ear lining, there are several types of distinguished chronic inflammatory changes: simple, with cholesteatoma, with the formation of inflammatory granulation tissue, in course of specific diseases. <br><b

The role of micro-noise trauma in the etiology of aging-related changes in the inner ear.

Eleven chinchillas between 1 and 2.4 years of age had the malleus/incus complex removed from one middle ear and then lived in the Washington University animal facilities for 4 years post-surgery. Each animal had one ear (termed ambient-noise) in which the conductive apparatus was intact; the other ear (termed noise-protected) had a 50-60 dB conductive hearing loss. The background sound level in the animal facility was 59 dBA with periodic brief sounds up to 102 dBA. After the 4-year experimental period, both ears were fixed, embedded in plastic and dissected for microscopic examination as flat

The mechanics of the middle-ear at static air pressures: the role of the ossicular joints, the function of the middle-ear muscles and the behaviour of stapedial prostheses.

In an experimental study, the mechanical behaviour of the ossicle-chain during changes of the static air pressure was analyzed microscopically and with a radiographic magnification technique in temporal bone preparations. Several preliminary experiments served to pinpoint methodological problems, like exsiccation-artifacts, storage procedures, preparation defects, missing air-cushion effect with the opened middle-ear cavity, absent labyrinthine pressure, relation of the optical axis to middle-ear structures and statistical reproducibility of the measured values. Variations of the static air pr
