Recent trends in the treatment of posttraumatic stress disorder and other mental disorders in the VHA.

OBJECTIVE: This study proposed to evaluate Veterans Health Administration (VHA) specialty mental health care workload for treating posttraumatic stress disorder (PTSD) and other mental disorders between 2005 and 2010 in comparison with results from 1997 to 2005. The 2005-2010 time frame represents a period of increased utilization of services by recently returning veterans and of program expansion within VHA. METHODS: VHA administrative databases were queried for all veterans receiving specialty mental health treatment annually between 2005 and 2010. Veterans were categorized by military serv

A multi-component cognitive-behavioral intervention for sleep disturbance in veterans with PTSD: a pilot study.

STUDY OBJECTIVES: A significant portion of US military personnel are returning from deployment with trauma-related sleep disturbance, and disrupted sleep has been proposed as a mechanism for the development of medical conditions in those with posttraumatic stress disorder (PTSD). Although individuals with PTSD may realize improved sleep with either PTSD treatment or CBT for insomnia, many continue to experience residual sleep difficulties. Newly developed interventions designed to address nightmares are effective to this end, but often do not fully remove all aspects of PTSD-related sleep diff

Reproductive health of male Australian veterans of the 1991 Gulf War.

BACKGROUND: Since the 1991 Gulf War concerns have been raised about the effects of deployment to the Gulf War on veterans' health. Studies of the reproductive health of Gulf War veterans have reported varied findings. METHODS: We undertook a cross-sectional study of male Australian Gulf War veterans (n = 1,424) and a randomly sampled military comparison group (n = 1,548). The study was conducted from August 2000 to April 2002. A postal questionnaire included questions about difficulties achieving pregnancy, pregnancy outcomes including live births, stillbirths, miscarriages and terminations;

Frostbite: diagnosis, treatment, prognosis, and future directions.

From cold-weather training to deployments in high altitude and arctic conditions, frostbite injury and its sequelae remain a serious concern in military operations. Frostbite harms tissue in two distinct ways. The first involves ice crystal formation within tissue, resulting in mechanical damage and ischemia. The second occurs secondary to the inflammatory and prothrombotic state caused by reperfusion from rewarming frozen tissue. Frostbite injuries can be classified in a number of ways, but no perfect system exists. Initial work-up and diagnosis are primarily clinical. However, in equipped tr

Sulfur dioxide exposure of mice induces peribronchiolar fibrosis-A defining feature of deployment-related constrictive bronchiolitis.

Deployment-related constrictive bronchiolitis (DRCB) has emerged as a health concern in military personnel returning from Southwest Asia. Exposure to smoke from a fire at the Al-Mishraq sulfur enrichment facility and/or burn pits was reported by a subset of Veterans diagnosed with this disorder. DRCB is characterized by thickening and fibrosis of small airways (SA) in the lung, but whether these are related to toxin inhalation remains uncertain. The aim of this study was to determine whether sulfur dioxide (SO2) exposure can induce histopathological features of DRCB. C57BL/6J mice were exposed

Humanitarian Medical Aid Mission in the Middle of a Pandemic - Israeli Experience in Equatorial Guinea.

INTRODUCTION: In March 2021, a series of explosions shook a military base in Bata, Equatorial Guinea. As a response to government officials' request, the Israel Defense Forces Medical Corps (IDF-MC) deployed an emergency aid team that faced two major challenges: (1) understanding the scenario, the injury patterns, and the needs of the local medical system; and (2) minimizing the coronavirus disease 2019 (COVID-19) outbreak threats. This report describes the team design, the activities performed before and during the deployment, analyzes the pathology encountered, and shares lessons learned fro

Periodontal Disease and Tooth Wear in a Sample of German Soldiers with Posttraumatic Stress Disorder.

PURPOSE: Dental symptoms of post-traumatic stress disorder (PTSD) patients include a majority of painful temporomandibular joint and masticatory muscle findings, restricted mouth opening, and pronounced attritions. Traumatic occlusal force resulting in injury of the teeth and/or the periodontal attachment apparatus may exceed the adaptive capacity of the individual person or site. This observational cross-sectional study in soldiers with PTSD and a non-PTSD control group after military deployments aimed to evaluate a possible relationship between bruxism and periodontal diagnosis. The Impact of Hazardous Alcohol Use on Behavioral Healthcare Utilization Among National Guard Service Members.

BACKGROUND: Prior research suggests that both posttraumatic stress disorder (PTSD) and alcohol abuse affect behavioral healthcare utilization among combat-exposed military populations. However, their interactive effect is not well documented, especially after experiencing psychological trauma. OBJECTIVE: This study examined the role of hazardous alcohol use (i.e. repeated patterns of drinking which lead to harmful consequences) on behavioral healthcare utilization among service members stratified by past-year combat exposure. METHOD: This study utilized a sample of National Guard service mem

Association of War Zone-Related Stress With Alterations in Limbic Gray Matter Microstructure.

IMPORTANCE: Military service members returning from theaters of war are at increased risk for mental illness, but despite high prevalence and substantial individual and societal burden, the underlying pathomechanisms remain largely unknown. Exposure to high levels of emotional stress in theaters of war and mild traumatic brain injury (mTBI) are presumed factors associated with risk for the development of mental disorders. OBJECTIVE: To investigate (1) whether war zone-related stress is associated with microstructural alterations in limbic gray matter (GM) independent of mental disorders commo

Acute Mild Traumatic Brain Injury Assessment and Management in the Austere Setting-A Review.

INTRODUCTION: Traumatic brain injury (TBI) continues to be a major source of military-related morbidity and mortality. The insidious short- and long-term sequelae of mild TBIs (mTBIs) have come to light, with ongoing research influencing advances in patient care from point of injury onward. Although the DoDI 6490.11 outlines mTBI care in the deployed setting, there is currently no standardized training requirement on mTBI care in the far-forward deployed setting. As the Joint Trauma System (JTS) is considered to be one of the leaders in standard of care trauma medicine in the deployed environm

Online versus telehealth delivery of a parenting program for postdeployed military families: Child internalizing and externalizing symptoms as moderators of response.

OBJECTIVE: Families could benefit from remote parent training programs. However, the comparative effectiveness of online self-directed versus facilitator-delivered telehealth for families with varying characteristics remains unclear. This study examined whether baseline child mental health symptoms moderate parental response to two remote delivery formats of a parenting intervention for military families. METHOD: A sample of 149 families with a 4- to 12-year-old child and at least one parent deployed post-9/11 participated in a 14-week After Deployment, Adaptive Parenting Tools parenting inte

Training in the Shadow of War: Exertional Heat Illness During the "Iron Swords" War.

INTRODUCTION: High-intensity physical exertion in combat zones under harsh environmental conditions is associated with exertional heat illness (EHI). However, the impact of a full-scale war on the incidence and severity of EHI in training settings remains insufficiently investigated. MATERIALS AND METHODS: We analyzed EHI data obtained from the IDF Institute of Military Physiology database for three years (October 2022 to September 2025). The cohort included 174 soldiers diagnosed with EHI, stratified into three phases: Pre-War, Acute-War, and Late-War. RESULTS: Despite massive deployment to
