Privacy-preserving adaptive arrhythmia monitoring with wearable ECG, smartphone PPG, and federated on-device learning.

INTRODUCTION: Wearable electrocardiography (ECG) and smartphone technology create new opportunities for arrhythmia monitoring outside clinical environments. However, deployable mobile cardiac systems must address more than just classification accuracy. They must reliably acquire physiological signals under real-world conditions, protect the privacy of sensitive cardiac data, adapt to user- and sensor-specific variability, and minimize false alarms in daily use. We present Cardio-FL, a smartphone-based digital health prototype for privacy-preserving, adaptive arrhythmia monitoring. METHODS: Th

Physiological determinants of mechanical efficiency during advanced ageing and disuse.

This study aimed to determine which physiological factors impact net efficiency (ηnet) in oldest-old individuals at different stages of skeletal muscle disuse. To this aim, we examined ηnet, central haemodynamics, peripheral circulation, and peripheral factors (skeletal muscle fibre type, capillarization and concentration of mitochondrial DNA [mtDNA]). Twelve young (YG; 25 ± 2 years), 12 oldest-old mobile (OM; 87 ± 3 years), and 12 oldest-old immobile (OI; 88 ± 4 years) subjects performed dynamic knee extensor (KE) and elbow flexors (EF) exercise. Pulmonary oxygen uptake, photoplethysmography,

Cardiorespiratory responses to muscle metaboreflex activation in fibrosing interstitial lung disease.

NEW FINDINGS: What is the central question of this study? We determined whether sensory feedback from metabolically sensitive skeletal muscle afferents (metaboreflex) causes a greater ventilatory response and higher dyspnoea ratings in fibrosing interstitial lung disease (FILD). What is the main finding and its importance? Ventilatory responses and dyspnoea ratings during handgrip exercise and metaboreflex isolation were not different in FILD and control groups. Blood pressure and heart rate responses to handgrip were attenuated in FILD but not different to controls during metaboreflex isolati

Use of a Smart Watch for Early Detection of Paroxysmal Atrial Fibrillation: Validation Study.

BACKGROUND: Wearable devices with photoplethysmography (PPG) technology can be useful for detecting paroxysmal atrial fibrillation (AF), which often goes uncaptured despite being a leading cause of stroke. OBJECTIVE: This study is the first part of a 2-phase study that aimed at developing a method for immediate detection of paroxysmal AF using PPG-integrated wearable devices. In this study, the diagnostic performance of 2 major smart watches, Apple Watch Series 3 and Fitbit (FBT) Charge HR Wireless Activity Wristband, each equipped with a PPG sensor, was compared, and the pulse rate data outp

Evidence for a physiological role of pulmonary arterial baroreceptors in sympathetic neural activation in healthy humans.

KEY POINTS: In an anaesthetised animal model, independent stimulation of baroreceptors in the pulmonary artery elicits reflex sympathoexcitation. In humans, pulmonary arterial pressure is positively related to basal muscle sympathetic nerve activity (MSNA) under conditions where elevated pulmonary pressure is evident (e.g. high altitude); however, a causal link is not established. Using a novel experimental approach, we demonstrate that reducing pulmonary arterial pressure lowers basal MSNA in healthy humans. This response is distinct from the negative feedback reflex mediated by aortic and ca

Reduced arterial vasodilatation in response to hypoxia impairs cerebral and peripheral oxygen delivery in hypertensive men.

KEY POINTS: Hypoxaemia evokes a repertoire of homeostatic adjustments that maintain oxygen supply to organs and tissues including the brain and skeletal muscles. Because hypertensive patients have impaired endothelial-dependent vasodilatation and an increased sympathetic response to arterial oxygen desaturation, we investigated whether hypertension impairs isocapnic hypoxia-induced cerebral and skeletal muscle hyperaemia to an extent that limits oxygen supply. In middle-aged hypertensive men, vertebral and femoral artery blood flow do not increase in response to isocapnic hypoxia, limiting bra

Cerebrovascular response to the cold pressor test - the critical role of carbon dioxide.

What is the central question of this study? What is the role of carbon dioxide in the cerebral blood flow (CBF) response to the cold pressor test (CPT)? What is the main finding and its importance? The CBF response was elevated during the isocapnic (controlled CO2 ) CPT in the middle cerebral artery and the internal carotid artery compared with the poikilocapnic (uncontrolled CO2 ) CPT, owing to ventilation-associated reductions in end-tidal CO2 . Furthermore, the common carotid artery vasodilated to a greater extent during the isocapnic compared with the poikilocapnic CPT, whereas the interna

Orthostatic hypotension and subjective sleep quality in older people.

Poor sleep quality and orthostatic hypotension are common complaints in an older population, and both are related to factors such as polypharmacy and depression. However, it is not known whether there is a direct association between the two. Our objective is to investigate a potential association between orthostatic blood pressure response and subjective sleep quality in older people. A within-subjects, cross-sectional design embedded in a larger longitudinal study design. Participants were recruited from the community to visit the TRIL clinic at St James's Hospital, where they underwent a str

Development of an efficient deep learning model for arrhythmia detection from consumer smartwatches.

INTRODUCTION: Cardiac arrhythmias often progress without clear symptoms and may remain undetected until severe complications occur. Continuous monitoring through consumer smartwatches offers a scalable and non-invasive opportunity for real-time rhythm screening in free-living conditions. However, wrist-based photoplethysmography (PPG) signals are highly susceptible to motion artifacts and inter-subject variability, limiting traditional signal-processing techniques. In this work, we compare classical machine learning and deep learning approaches for multi-class arrhythmia classification from re

Longitudinal sex differences in cerebrovascular ageing in older adults: results from the brain in motion study.

Older females are at an increased risk of dementia, cardiovascular disease and stroke. However sex differences in cardiovascular and cerebrovascular ageing remain poorly understood. We examined longitudinal changes in vascular function in 82 adults (mean age: 65.4 ± 5.1 years; range: 55-75; 39 females) at two timepoints (T1 and T2) 6.1 ± 1.4 years apart. We measured middle cerebral artery velocity (MCAv) and pulsatility (PI) using transcranial Doppler ultrasound, mean arterial blood pressure (MAP) with finger photoplethysmography and cerebrovascular conductance and resistance (CVCi, CVRi) at r

A comprehensive accuracy assessment of Samsung smartwatch heart rate and heart rate variability.

BACKGROUND: Photoplethysmography (PPG) is a low-cost and easy-to-implement method to measure vital signs, including heart rate (HR) and pulse rate variability (PRV) which widely used as a substitute of heart rate variability (HRV). The method is used in various wearable devices. For example, Samsung smartwatches are PPG-based open-source wristbands used in remote well-being monitoring and fitness applications. However, PPG is highly susceptible to motion artifacts and environmental noise. A validation study is required to investigate the accuracy of PPG-based wearable devices in free-living co

Exploring the Abnormal Modulation of the Autonomic Systems during Nasal Flow Limitation in Upper Airway Resistance Syndrome by Hilbert-Huang Transform.

UNLABELLED: Patients with nasal flow limitation and upper airway resistance syndrome (UARS) during sleep can present with low blood pressure and disturbing symptoms associated with hypervagotony. We hypothesized that the dynamic changes of the autonomic system related to inspiratory flow limitation can be quantified by the developed analytic technique applied on beat-to-beat heart rate (RR intervals) and finger photoplethysmography (PPG). METHOD: A breath-by-breath investigation based on the Hilbert-Huang transform was performed to explore autonomic nervous system changes observed during insp

Characteristics and effectiveness of vasodilatory and pressor compensation for reduced relaxation time during rhythmic forearm contractions.

What is the central question of this study? Reduced relaxation time between contractions in exercise requires increased vasodilatation and/or pressor response to prevent hypoperfusion and potential compromise to exercise tolerance. However, it remains unknown whether and to what extent local vasodilatation and/or systemic pressor compensation occurs and whether the efficacy of compensation is exercise intensity dependent. What is the main finding and its importance? We demonstrate that in a forearm exercise model vasodilatory but not pressor compensation occurs and is adequate to prevent hypop

Continuous measurement of pulse arrival time for identification of blood pressure changes in patients undergoing hemodialysis-a feasibility study.

BACKGROUND AND HYPOTHESIS: Intradialytic hypotension (IDH) is a common complication during hemodialysis, associated with immediate and long-term risks. Current monitoring methods lack timely warning signals for preemptive intervention. Continuous non-invasive measurement of blood pressure providing real-time insights into hemodynamic changes might enable the early detection of impending IDH. Pulse arrival time (PAT), which can be measured non-invasively and continuously by electrocardiogram (ECG) and photoplethysmography (PPG), is inversely correlated with blood pressure and might serve as suc

Cerebral hemodynamic changes induced by a lumbar puncture in good-grade subarachnoid hemorrhage.

BACKGROUND: Patients with good-grade subarachnoid hemorrhage (SAH) are those without initial neurological deficit. However, they can die or present severe deficit due to secondary insult leading to brain ischemia. After SAH, in a known context of energy crisis, vasospasm, hydrocephalus and intracranial hypertension contribute to unfavorable outcome. Lumbar puncture (LP) is sometimes performed in an attempt to reduce intracranial pressure (ICP) and release headaches. We hypothesize that in good-grade SAH patients, a 20-ml LP releases headaches, reduces ICP and improves cerebral blood flow (CBF)
