BACKGROUND: Crohn's disease complicated by perianal fistulae is more prevalent and severe in patients of African ancestry.
METHODS: We profiled single cells from diverse patients with Crohn's disease with perianal fistula from colorectal mucosa and fistulous tracts. Immunofluorescence was performed to validate predicted cell-cell interactions. Unstimulated monocytes were chronically cultured in diverse cohorts. A subset was analyzed by single-nucleus RNA + ATAC sequencing.
FINDINGS: Fistulous tract cells from complete proctectomies demonstrated enrichment of myeloid cells compared to paired

INTRODUCTION: Fistula-in-ano can be associated with a number of conditions, including Crohn's disease. The majority, however, are classified as idiopathic or cryptoglandular. The aim of this study was to review the outcome of surgical management of fistula-in-ano in a specialist colorectal unit.
MATERIALS AND METHODS: One hundred and four consecutive patients underwent surgery for anal fistulae between 1st January 2000 and December 2004. Data was analysed in two main groups, according to the aetiology, cryptoglandular (n = 86) and Crohn's disease (n = 18). Follow-up data was available on 91 p

AIM: Alteration of bowel function after delayed coloanal anastomosis (DCAA) might be a limitation to its utilization. Our aim was to assess the long-term bowel function of DCAA in a large multicentric cohort.
METHOD: All patients who underwent DCAA interventions at 29 GRECCAR-affiliated hospitals between 2010 and 2021 were retrospectively included. Low anterior resection syndrome (LARS) score or confection of a stoma due to poor bowel function was assessed in eligible patients. Good bowel function was defined by the preservation of bowel continuity with no LARS or a minor LARS.
RESULTS: Amon

PURPOSE: In recent years, intersphincteric resection has increasingly become a viable alternative to abdominoperineal resection due to its significant preservation of the anus. Previous studies have indicated that intersphincteric resection has provides comparable oncological outcomes to APR, but anal incontinence remains a common postoperative complication. we reassessed identify preoperatively controllable and measurable risk factors that may affect anal function following intersphincteric resection, and to attempt to construct a preoperative risk assessment system for anal function after in

Transanal total mesorectal excision (taTME) has improved the laparoscopic dissection for rectal cancer in the narrow pelvis. Although taTME has more clinical benefits than laparoscopic surgery, such as a better view of the distal rectum and direct determination of distal resection margin, an intraoperative urethral injury could occur in excision ta-TME. This study aimed to determine the feasibility and efficacy of the ta-TME with IRIS U kit surgery. This retrospective study enrolled 10 rectal cancer patients who underwent a taTME with an IRIS U kit. The study endpoints were the safety of acces

AIM: Physiotherapy is an established treatment strategy for low anterior resection syndrome (LARS). However, data on its efficacy are limited. This is in part due to the inherent challenges in study design in this context. This systematic review aims to analyse the methodology of studies using pelvic floor physiotherapy for treatment of LARS to elucidate the challenges and limitations faced, which may inform the design of future prospective trials.
METHODOLOGY: A systematic review of the literature was undertaken through MEDLINE, Embase and Cochrane Library, yielding 345 unique records for sc

BACKGROUND: Abdominal colostomy has been reported as an option with good quality of life for patients requiring abdominoperineal resection (APR) for very low rectal cancer. Some young, compliant patients, nevertheless, are very motivated to avoid abdominal colostomy following APR. Spiral smooth muscle cuff perineal colostomy as neosphincter reconstruction can be a reasonable alternative. We have published before the results of a series of sphincter reconstruction in the conventional technique following APR. As we developed our technique for colorectal resection sphincter reconstruction, we als

AIM: Rectal cancer patients undergoing neoadjuvant chemoradiotherapy (NACRT) experience physical deterioration and reductions in their quality of life. This feasibility study assessed prehabilitation (a walking intervention) before, during and after NACRT to inform a definitive multi-centred randomized clinical trial (REx trial).
METHODS: Patients planned for NACRT followed by potentially curative surgery were approached (August 2014-March 2016) (www.isrctn.com; 62859294). Prior to NACRT, baseline physical and psycho-social data were recorded using validated tools. Participants were randomize

AIM: Mandatory splenic flexure mobilization (SFM) has been debated for rectal cancers. Proponents argue that additional mobilization facilitates a tension-free anastomosis; however, this must be weighed against heightened morbidity. Little is known about the impact of specific techniques on pathology quality metrics. We aim to determine the impact of SFM on pathology quality metrics for patients undergoing rectal resections for colorectal adenocarcinoma.
METHOD: Patients were selected by querying the University of Kansas electronic medical records and the American College of Surgeons National

BACKGROUND: Although previous studies have examined frailty as a potential predictor of adverse surgical outcomes, little is reported on its application. We sought to assess the impact of the 5-item modified frailty index (mFI) on morbidity in patients undergoing combined colorectal and liver resections.
METHODS: Adult patients who underwent combined colorectal and liver resections were identified using the ACS-NSQIP database (2005-2015). The 5-item mFI consists of history of chronic obstructive pulmonary disease, congestive heart failure, hypertension, diabetes, and partial/total dependence.

BACKGROUND: Transanal total mesorectal excision (TaTME) is a technically demanding procedure with a significant learning curve. Operative time is often used as a proxy for procedural proficiency.
OBJECTIVE: To evaluate the impact of the learning curve and selected patient- and procedure-related factors on the operative duration of TaTME procedures.
METHODS: A retrospective analysis was conducted on 250 consecutive patients who underwent TaTME for rectal cancer between 2016 and 2025. Operative time trends were assessed in relation to case sequence, individual surgeon experience, and patient-r

BACKGROUND: Abdominoperineal resection (APR) for low rectal cancer is associated with substantial morbidity and psychosocial burden. This randomized controlled trial evaluated whether integrating resilience-oriented cognitive behavioral therapy and behavioral sleep regulation into enhanced recovery after surgery (ERAS) improves clinical and oncologic outcomes.
METHODS: In this single-center trial, 192 patients undergoing curative APR were randomized to standard ERAS or ERAS plus a structured multidomain behavioral intervention. The primary endpoint was 2-year disease-free survival (DFS). Key

BACKGROUND: Local recurrence remains a major challenge in rectal cancer. Total mesorectal excision is the surgical cornerstone; however, early outcomes after its adoption have not consistently achieved satisfactory local control, partially reflecting the difficulty of maintaining appropriate mesorectal dissection. Advances in the understanding of the perirectal fascia and dissection planes, together with robotic technology, have enabled high-quality, fascia-guided total mesorectal excision. Despite this progress, regional strategies differ, emphasizing either the optimization of upfront surger

BACKGROUND: This study aims to develop an anatomic landmark-based ordinal grading system for surgical difficulty in robot-assisted rectal resection and to assess its preoperative predictability, with the potential to support preoperative assessment of surgical difficulty.
METHODS: We divided the procedure into two phases: mesocolon dissection and mesorectal dissection. Surgical difficulty was defined using an anatomy-based ordinal grading system (grades 1-3) for each phase. A cohort of 158 patients undergoing robot-assisted rectal resection was included. Regression analysis was performed for
