Functional approach to a Class II patient with upper first molar impaction.

The lack of the left first molar maxillary and the left second molar maxillary, caused respectively by impaction and agenesis is a very rare case, which determines an important occlusal imbalance and asymmetrical mandible movement. A gnatologic and functional orthodontic approach were planned to improve the retrognathic mandible and the muscular activity using kinesiograph and electromyography.

Morphologic variability of nonsyndromic operated patients affected by cleft lip and palate: a geometric morphometric study.

INTRODUCTION: In this study, we compared patterns of morphologic variations of the craniofacial skeleton between patients affected by clefts who were operated on and unaffected subjects, aiming to discuss possible morpho-functional consequences of treatment in craniofacial development. METHODS: The lateral cephalograms of 76 subjects, comprising patients with operated unilateral cleft lip and palate (OpC) and a group matched for sex and age without cleft, were used. Thirteen landmarks were used as variables in geometric morphometric tests quantifying and describing overall shape variation, di

The use of a coded healing abutment as an impression coping to design and mill an individualized anatomic abutment: a clinical report.

A coded implant healing abutment makes an impression at the implant level no longer necessary. An impression is made of the healing abutment, which is placed onto the implant directly after implant placement. The codes embedded in the occlusal surface of the healing abutment provide essential information for the computer software to place the implant analog in the definitive cast and to design and mill the definitive abutment.

Predictive value of molar bite force on Class II functional appliance treatment outcomes.

Sagittal intermaxillary changes brought about by functional appliances show large inter-individual variation. One factor that may in part explain these differences is the masticatory musculature and its functional capacity. The aims of this study were to investigate changes in maximal molar bite force during functional appliance treatment and to assess the influence of pre-treatment maximal molar bite force on treatment outcomes with functional appliances used in Class II malocclusion children. Twenty-five children (17 males and 8 females), aged 9-13 years, with a Class II malocclusion and inc

Mandibular distraction using bone morphogenic protein and rapid distraction in neonates with Pierre Robin syndrome.

INTRODUCTION: Mandibular distraction is recognized as a treatment of respiratory distress in neonates with microretrognathia as seen in the Pierre Robin syndrome. However, mandibular distraction is a complex and lengthy treatment involving 2 to 4 weeks of distraction and another 4 to 12 weeks for bone consolidation. This study was performed to establish the safety and effectiveness of rapid protocol distraction osteogenesis with recombinant human bone morphogenetic protein 2 (rhBMP-2) in neonates with the Pierre Robin syndrome. METHODS: A retrospective review of all patients treated in our de

Orthodontic treatment combined with mandibular distraction osteogenesis and changes in stomatognathic function.

We performed an orthodontic treatment combined with mandibular distraction osteogenesis in a 15-year-old patient who wanted a correction of a chin deficiency and a protruding upper lip. The patient had an Angle Class II division 1 malocclusion with mandibular retrusion, a low mandibular plane angle, and scissors bite. First, a quad-helix appliance was applied to the mandibular dentition to correct the scissors bite in the bilateral premolar region. Later, a preadjusted edgewise appliance was applied to the maxillary and mandibular teeth. After 3 days, a mandibular distraction osteogenesis was

Case report: forced eruption and implant site development.

It is now possible to use the orthodontic extrusion of periodontally compromised teeth to facilitate the development of a future implant site in an area where the amount of bone in the inciso-apical dimension would otherwise have been inadequate. The purpose of this case report was to analyze the hard and soft tissue changes when forced eruption is used for implant site development.

Long-term surgical versus functional Class II correction: a comparison of identical twins.

The purpose of this twin case comparison was to assess the short- and long-term effects of nonsurgical treatment vs orthognathic surgical treatment for Class II correction. Two identical twins (age 13 years 3 months) were treated for Class II correction where one patient was treated nonsurgically using a fixed functional appliance, while the other was treated using orthognathic mandibular advancement surgery. The patients were recalled and evaluated 5 years in retention. Comparing changes in the short and long term, surgical treatment led to superior skeletal results compared to the nonsurgica

Modified internal mandibular distraction osteogenesis in the treatment of micrognathia secondary to temporomandibular joint ankylosis: 4-year follow-up of a case.

Micrognathia and obstructive sleep apnoea syndrome (OSAS) are problems subsequent to temporomandibular joint ankylosis (TMJa) in growing patients. For patients with micrognathia and OSAS secondary to TMJa, it is important to restore proper mandibular form and dimension, achieve occlusal stability and recover satisfactory joint movement. We report a 4-year follow-up of a patient with micrognathia and OSAS secondary to bilateral TMJa. The treatment of this patient involved (1) a modified internal mandibular distraction osteogenesis without altering the pre-existing occlusion; (2) TMJ arthroplast

Bone scintigraphy of human temporomandibular joints during Herbst treatment: a case report.

Bone scintigraphy results were compared with changes shown on orthopantomographic radiographs in a patient with facial asymmetry, before, during, and after Herbst treatment, and followed up with control of growth activity in the temporomandibular joints (TMJs) after long-term retention. The present study showed that new bone formation (modelling) was initiated asymmetrically in TMJs during treatment. The results indicate that late development of right/left asymmetry in the occlusion can be corrected and normalized using the Herbst appliance therapy, stimulating a differentiated 'catch up' grow

Maxillofacial biomodelling.

The authors report the clinical applications of biomodelling with the stereolithography apparatus, a computer-controlled manufacturing technique that builds anatomically accurate skeletal models from sectional radiological data. Reference to several individual cases demonstrates how pre-operative 3-D modelling can refine the accuracy of diagnostic information, facilitate preoperative planning and surgical technique, and reduce operating time.

Characteristics of Morphology in Older Adult Patients with Obstructive Sleep Apnea: A Retrospective Cross-Sectional Study.

Objective: The prevalence and severity of obstructive sleep apnea (OSA) increase with age, suggesting that age-related factors are etiological factors for OSA in older adults. In addition to anatomic contractions of the upper respiratory tract, such as those caused by obesity and retrognathia, sleep is impaired in older OSA patients due to aging. Although aging has long been associated with structural changes in the upper airway potential, specific age-related anatomical differences in patients with OSA are not established. This study aimed to examine age-related morphological differences in O

[Mandibular repositioning in adult patients. An alternative to surgery in some patients? A two-year follow-up].

INTRODUCTION: A number of adult patients with skeletal discrepancies refuse surgical intervention. AIMS: The aim was to assess the reaction to mandibular repositioning in simulating a skeletal correction in such patients. MATERIALS AND METHODS: 32 consecutive patients without any signs of temporo-mandibular dysfunction (TMD) were offered mandibular repositioning, as a non-invasive alternative and it was explained to them that the approach was based on the results described in case reports. Before initiating any treatment initial records, headfilms, study casts and photos were taken (T0) and

Retrospective 25-year follow-up of treatment outcomes in Angle Class III patients : Success versus failure.

OBJECTIVES: Despite recommendations for early treatment of hereditary Angle Class III syndrome, late pubertal growth may cause a relapse requiring surgical intervention. This study was performed to identify predictors of successful Class III treatment. MATERIALS AND METHODS: Thirty-eight Class III patients treated with a chincup were retrospectively analyzed. Data were collected from the data archive, cephalograms, and casts, including pretreatment (T0) and posttreatment (T1) data, as well as long-term follow-up data collected approximately 25 years after treatment (T2). Cartilage changes link retrognathic mandibular growth to TMJ disc displacement in a rabbit model.

Recent experimental research demonstrated that non-reducing temporomandibular joint (TMJ) disc displacement in growing rabbits impaired mandibular growth. TMJ disc displacement is also shown to induce histological changes of the condylar cartilage. The authors hypothesized that the severity of these changes would correlate to the magnitude of mandibular growth. Bilateral non-reducing TMJ disc displacement was surgically created in 10 growing New Zealand White rabbits. Ten additional rabbits constituted a sham operated control group. Aided by tantalum implants, growth was cephalometrically dete
