Two contrasting post-zoster dermatomal phenomena.

A 29-year-old, normotensive, nondiabetic man presented with a 9-day history of a scaly, pruritic eruption involving the right chest, axilla, and arm. He had a history of herpes zoster involving the same areas about 4 weeks ago. The present eruption started after the herpetic lesions had healed. Examination revealed scaly, erythematous plaques and papules involving the right side of the chest, axilla, and arm in a dermatomal pattern (figure 1). Removal of the scales revealed underlying bleeding points (positive Auspitz sign). The rest of the body, including scalp, palms, soles, and nails, were

Orofacial pain disorder or trigeminal autonomic cephalalgia? Insights into pathophysiology and implications in medicine and dentistry.

OBJECTIVE: This narrative review highlights the importance of orofacial pain considerations in the differential diagnosis of trigeminal autonomic cephalalgias (TACs)/trigeminal autonomic orofacial pains (TAOPs). In addition, we provide novel insights into the pathophysiology of the presentation of TACs in the maxillary (V2) and mandibular (V3) trigeminal distributions (TAOPs). BACKGROUND: TACs are lifelong disorders characterized by severe unilateral head pain accompanied by cranial autonomic phenomena. However, the head pain experienced may refer to the face, be present solely in the orofaci

Effect of Preoperative Pain on Inferior Alveolar Nerve Block.

The present study tested the hypothesis that the amount and severity of preoperative pain will affect the anesthetic efficacy of inferior alveolar nerve block (IANB) in patients with symptomatic irreversible pulpitis. One-hundred seventy-seven adult volunteer subjects, actively experiencing pain in a mandibular molar, participated in this prospective double-blind study carried out at 2 different centers. The patients were classified into 3 groups on the basis of severity of preoperative pain: mild, 1-54 mm on the Heft-Parker visual analog scale (HP VAS); moderate, 55-114 mm; and severe, greate

Enigmatic pain referred to the teeth and jaws.

A number of obscure syndromes can mimic dental or jaw pain in the absence of pathosis within these structures. This enigmatic dental pain includes conditions such as pretrigeminal neuralgia, complex regional pain syndrome, temporal tendinitis, and carotodynia. Each of these syndromes is described through a pertinent case report to illustrate appropriate diagnosis and treatment.

Evaluation of the Antibacterial and Antioxidant Properties of Chemical Constituents of the Roots of Woodfordia uniflora: An Integrated Approach of Experimental and Computational Study.

Woodfordia uniflora is a medicinal plant used for the treatment of malaria, toothache, and stomach problems. The root parts of the plant are also used for healing liver disorders. Silica gel chromatography separation of CH2Cl2/MeOH (1:1) and MeOH extracts of roots of W. uniflora result in the isolation of three compounds, namely, bergenin (1), β-sitosterol (2), and epigallocatechin 3-gallate (3), reported herein for the first time from the plant. The structure of the isolated compounds was elucidated using NMR (1D and 2D) techniques. Disk diffusion and DPPH assay were used to evaluate the anti

Differential diagnosis of orofacial pain and temporomandibular disorder.

When a patient complains of orofacial pain, health care providers must make a correct diagnosis. Doing this can be difficult, since various signs and symptoms may not be specific for 1 particular problem or disorder. One initially should formulate a broad differential diagnosis that can be narrowed after analysis of the history and examination. In this article, orofacial pain is categorized as being caused by: intracranial pain, headaches, neuropathic pain, intraoral pain, temporomandibular disorder, cervical pain, pain related to anatomically associated structures, referred pain, or mental il

Evidence-based review of clinical studies on pharmacology (non-anesthetic studies).

We have divided the analysis of pharmacology studies into studies on local anesthetics and studies on all other pharmacologic agents. This division reflects the large number of studies focusing on methods for obtaining effective anesthesia in patients and permits an independent focus on those clinical studies evaluating nonanesthetic drugs. In this section, we review that alternative class of agents, with an emphasis this year on clinical studies evaluating oral analgesics.

[A 24 month evaluation of zinc oxide pulpotomy on primary canines].

Pulpotomy is the most performed and controversial therapeutic in pediatric dentistry. Since formocresol is known to have a toxic effect on living tissues, plus a mutagenic and carcinogenic potential with a systemic uptake of formocresol via pulpotomized teeth, other alternative products have been investigated. Twenty-five decayed primary canines and twenty-five healthy primary canines necessitating disking for orthodontic purposes were pulpotomized using non modified Zinc Oxide Eugenol. The effects of this material were evaluated both clinically and radiographically. Post operative control exa

Gender differences in analgesia for endodontic pain.

The purpose of this prospective clinical trial was to investigate the analgesic efficacy of three oral medication groups on postoperative endodontic pain in male and female dental patients, with an emphasis on analgesic differences between the sexes. Forty-three patients were administered ibuprofen 600 mg, placebo, or pentazocine 50 mg/0.5 mg naloxone in a randomized, double-blinded manner. Beginning immediately after endodontic treatment, patients took the assigned medication every 6 hours for 24 hours and recorded their degree of discomfort on a 100-mm visual analog scale. Endodontic applications of cone-beam volumetric tomography.

The ability to assess an area of interest in 3 dimensions might benefit both novice and experienced clinicians alike. High-resolution limited cone-beam volumetric tomography (CBVT) has been designed for dental applications. As opposed to sliced-image data of conventional computed tomography (CT) imaging, CBVT captures a cylindrical volume of data in one acquisition and thus offers distinct advantages over conventional medical CT. These advantages include increased accuracy, higher resolution, scan-time reduction, and dose reduction. Specific endodontic applications of CBVT are being identified

Anesthetic efficacy of the supplemental intraosseous injection of 2% lidocaine with 1:100,000 epinephrine in irreversible pulpitis.

The purpose of this study was to determine the anesthetic efficacy of a supplemental intraosseous injection of 2% lidocaine with 1:100,000 epinephrine in teeth diagnosed with irreversible pulpitis. Fifty-one patients with symptomatic, vital maxillary, and mandibular posterior teeth diagnosed with irreversible pulpitis received conventional infiltrations or inferior alveolar nerve blocks. Pulp testing was used to determine pulpal anesthesia after "clinically successful" injections. Patients who were positive to the pulp tests, or were negative to the pulp tests but felt pain during endodontic a

Early management of dental pain in children and adolescents.

OBJECTIVES: There is little information regarding the initial management of dental pain in children and adolescents. The present study investigated the early management of 155 children and adolescents with dental pain prior to definitive treatment in the Eastman Dental Institute and Hospital for Oral Health Care Sciences, London, UK. METHOD: Prior to the child being examined by a qualified dentist in the casualty department, the accompanying parents or guardians were requested to complete a self-administered questionnaire regarding the duration and severity of pain, presence of swelling, prof

A clinical study of direct pulp capping applied to carious-exposed pulps.

Direct pulp capping of carious-exposed pulp was performed on 44 teeth. We evaluated the success rates of these cases, and analyzed the relationships between the success rates and their clinical findings. Furthermore, we examined the length of time necessary for adequate postoperative follow-up. The success rate in this study was 81.8%. Age of the patients, type of teeth, responses to thermal stimuli and percussion, and the diameter of pulpal exposure had no bearing on the success rate. However, the degree of bleeding on pulpal exposure was related to the success rate (p = 0.042). The success r

The mouth in health and disease.

Medical, dental and psychiatric conditions may cause dysfunction of the mouth in health and disease. The principle of clinical assessment of the patient, which includes a thorough assessment of the physical, social and psychological factors, is of paramount importance. This paper has attempted to describe some of the more common conditions affecting the mouth in general practice, and to underline important causes of pain which can occur in the face arising from conditions in the mouth.

Longitudinal evaluation of the impact of dental caries treatment on oral health-related quality of life among schoolchildren.

This study aimed to evaluate the impact of dental caries treatment on oral health-related quality of life (OHRQoL) among schoolchildren and the responsiveness of the Child Perceptions Questionnaire (CPQ8-10 ) instrument. Brazilian schoolchildren, 8-10 yr of age, were randomly selected and assigned to two groups--dental caries treatment (DCT) and caries-free (CF)--according to their caries experience [decayed, missing, or filled primary teeth (dmft) and decayed, missing or filled secondary teeth (DMFT) values of ≥ 0]. The CPQ8-10 instrument was administered at baseline and at 4 wk of follow-up

Pain remapping in migraine: a novel characteristic following trigeminal nerve injury.

The location of pain during the headache phase of migraine varies between individuals as well as between attacks in some individuals. We have observed a "remapping" or a change in the location of migraine pain following injury to the trigeminal system that is a novel characteristic to migraine and has not been described in other trigeminal pain syndromes of the head, neck, and face. Recognition of this clinical feature implies that the pathophysiology of migraine is impressionable and may be why diagnosis and treatment are often delayed.
