Long-term results of the metaphyseal-loading anterolaterally flared anatomic femoral stem for total hip arthroplasty.

BACKGROUND: Patients with secondary hip osteoarthritis due to developmental dysplasia of the hip (DDH) often have abnormal femoral morphology, making stem design critical for long-term outcomes. The FMS-anatomic stem previously demonstrated favourable mid-term results. Its successor, the Anatomic Fit stem, was developed with a reduced hydroxyapatite-coated area to enhance proximal load transfer and a narrower lateral flare to facilitate insertion. This study aimed to analyze the long-term clinical and radiographic outcomes of these stems. METHODS: The one-hundred seventy-eight hips underwent

An Investigation of a New Parameter Based on the Plastic Strain Gradient to Characterize Composite Constraint around the Crack Front at a Low Temperature.

Stress corrosion cracking (SCC) is an important destruction form of materials such as stainless steel, nickel-based alloy and their welded components in nuclear reactor pressure vessels and pipes. The existing popular quantitative prediction models of SCC crack growth rate are mainly influenced by fracture toughness values KJc or Jc. In particular, the composite constraint, containing the in-plane constraints and out-of-plane constraints around the crack front, has a significant influence on the fracture toughness of structures in nuclear power plants. Since the plastic strain gradient is a ch

Should systemic sclerosis be recognised as an occupational disease in Switzerland?

Systemic sclerosis is a rare autoimmune disease characterised by a multifactorial aetiology involving a gene–environment interaction. Despite the growing epidemiological arguments for odds ratio (OR) data showing an association with occupational exposure, systemic sclerosis is not currently included in the list of recognised occupational diseases in Switzerland, unlike other northern European countries. Future recognition will be conditional on the demonstration of a strong association between the disease and occupational exposure in the scientific literature. The present article’s

Does Proximally Coated Single-Wedge Cementless Stem Work Well in Dorr Type C Femurs? Minimum 10-year Followup.

BACKGROUND: Total hip arthroplasty (THA) with a proximally coated single-wedge (PSW) cementless stem had been generally considered not to be suitable for Dorr Type C femurs. This study compares the long term outcomes of PSW stem according to the type of proximal femoral geometry. MATERIALS AND METHODS: 307 primary THAs in 247 patients were performed with PSW cementless stem and followed up for over 10 years in this retrospective study. According to Dorr's criteria, 89 femurs were classified as Type A, 156 as Type B, and 62 as Type C. They were followed up for an average of 13.2 years (range 1

Wear of XLPE liner against zirconium heads in cementless total hip arthroplasty for patients under 40 years of age.

INTRODUCTION: Total hip arthroplasty (THA) in the young is challenging. The purpose of this study was to retrospectively determine the clinical, radiographic, and polyethylene wear rate of relatively young patients. METHODS: We evaluated the outcome of consecutive patients receiving primary THA who were under 40 years of age with a minimum 10-year follow-up. Indications for THA in these patients were osteoarthritis due to developmental dysplasia of the hip joint (9 hips), osteonecrosis of the femoral head (7 hips), juvenile idiopathic arthritis (2 hips), and osteoarthritis due to Perthes dise

Management of an extremely displaced maxillary canine.

CASE REPORT: Aligning a displaced maxillary canine into the dental arch is one of the most complicated problems in orthodontics. In cases of extremely high displacement, the tooth is frequently removed surgically. Because of the upper canines' significance to dental esthetics and functional occlusion, such a decision is a very serious one. This case report illustrates the treatment of an extremely high displaced maxillary canine. CLINICAL FINDINGS: The main diagnosis was the displacement and the retention of tooth 13 (in nearly horizontal position, apical to the neighboring teeth); further di

Increased resistance to acute compression injury in chronically compressed spinal nerve roots. An experimental study.

STUDY DESIGN: Nerve conduction velocity was studied in the dog cauda equina subjected to chronic and, 1 week later, additional acute compression. OBJECTIVES: To model the situation of spinal stenosis by first inducing a controlled, chronic compression injury and then to add compression at various pressure levels to the already compressed cauda equina. SUMMARY OF BACKGROUND DATA: Previous models on nerve root and cauda equina compression have analyzed the effects of compression only on noninjured nerves. Clinically, the nerves that are compressed in spinal stenosis already are exposed to chro

From manganism to manganese-induced parkinsonism: a conceptual model based on the evolution of exposure.

Manganism is a distinct medical condition from Parkinson's disease. Manganese exposure scenarios in the last century generally have changed from the acute, high-level exposure conditions responsible for the occurrence of manganism to chronic exposure to much lower levels. Such chronic exposures may progressively extend the site of manganese deposition and toxicity from the globus pallidus to the entire area of the basal ganglia, including the substantia nigra pars compacta involved in Parkinson's disease. The mechanisms of manganese neurotoxicity from chronic exposure to very low levels are no

Do toenail manganese and iron levels reflect brain metal levels or brain metabolism in welders?

Inhalation of welding fumes can cause metal accumulation in the brain, leading to Parkinsonian-like symptoms. Metal accumulation and altered neurochemical profiles have been observed using magnetic resonance imaging (MRI) in highly exposed welders, being associated with decreased motor function and cognition. While MRI is impractical to use as a health risk assessment tool in occupational settings, toenail metal levels are easier to assess and have been demonstrated to reflect an exposure window of 7-12 months in the past. Yet, it is unclear whether toenail metal levels are associated with bra

Burden and predictors of chronic obstructive pulmonary disease occurrence and severity among an occupational cohort of United States Department of Energy former workers.

BACKGROUND: Chronic Obstructive Pulmonary Disease (COPD) is a chronic inflammatory lung disease that reduces lung function and primarily affects older adults. Evidence suggests that occupational exposures like diesel exhaust, cadmium, welding fumes, and silica increase the risk of COPD. Some United States Department of Energy (DOE) workers may be exposed to these noxious substances as they execute their job responsibilities. Assessment of the burden of COPD among these workers and identification of the potential associations between the condition and the above occupational exposures is importa

A Rare Case of Total Talar Replacement for Talar Osteonecrosis Due to Fragility Fracture Secondary to Transient Osteoporosis of the Talus.

We report the case of a 44-year-old male patient without a notable medical history who developed transient osteoporosis following surgery for a medial malleolus fracture, which led to a stress fracture and osteonecrosis, ultimately requiring total talar replacement. The patient sustained the fracture spraining his ankle while welding. Osteosynthesis was performed using two cannulated cancellous screws, whereas bone union was achieved 3.5 months postsurgery. Five months after surgery, ankle joint pain worsened without new trauma episodes. Magnetic resonance imaging (MRI) showed diffuse hyperint

Topical anesthetic abuse keratopathy: an overlooked occupational eye health concern.

BACKGROUND: Topical anesthetic abuse keratopathy is a rare but potentially devastating complication that may lead to irreversible vision loss. In occupational settings, workers who sustain frequent ocular injuries may self-administer topical anesthetics without prior prescription, consequently increasing the risk of severe complications. This study evaluated the clinical findings and visual outcomes of patients with this condition. METHODS: All patients diagnosed with topical anesthetic abuse keratopathy who presented to Khalili Hospital, Shiraz, Iran, from March 2023 to March 2025 were inclu

Lower trapezius myocutaneous flap repairs adjacent deep electrical burn wounds.

BACKGROUND: Local tissue damage caused by electrical burns is often deep and severe. High-voltage electrical burns are common in the head, neck and torso areas. These are mostly caused by direct contact with the power supply and are often accompanied by deep injuries of the nerve, blood vessel, muscle, tendon, and bone. We must pay great attention to the clinical treatment of these parts injured by electrical burn. CASE PRESENTATION: The first case involved a migrant worker who touched a 6-kV high-tension wire when welding steel; this electric shock caused burns in many places. Deep electrica

Nerve transection repair using laser-activated chitosan in a rat model.

OBJECTIVES/HYPOTHESIS: Cranial nerve transection during head and neck surgery is conventionally repaired with microsuture. Previous studies have demonstrated recovery with laser nerve welding (LNW), a novel alternative to microsuture. LNW has been reported to have poorer tensile strength, however. Laser-activated chitosan, an adhesive biopolymer, may promote nerve recovery while enhancing the tensile strength of the repair. Using a rat posterior tibial nerve injury model, we compared four different methods of nerve repair in this pilot study. STUDY DESIGN: Animal study. DESIGN: Animals under

Occupational and environmental scleroderma. Systematic review and meta-analysis.

The etiology of systemic sclerosis (SSc) remains unknown; however, several occupational and environmental factors have been implicated. Our objective was to perform a meta-analysis of all studies published on SSc associated with occupational and environmental exposure. The review was undertaken by means of MEDLINE and SCOPUS from 1960 to 2014 and using the terms: "systemic," "scleroderma," or "systemic sclerosis/chemically induced" [MesH]. The Newcastle-Ottawa Scale was used for the qualifying assessment. The inverse variance-weighted method was performed. The meta-analysis of silica exposure

Epidemiologic study on work-related eye injuries in Kaohsiung, Taiwan.

To describe the epidemiologic features of work-related eye injuries in Kaohsiung, a hospital-based study was performed. Four hundred and eighty-six patients who were treated at emergency service or were admitted to the ophthalmology ward over a 4-year period were reviewed. Among these, 38.9% of eye injuries in the study were work-related. Male workers had a 3.99 higher odds ratio (OR) than females to suffer from eye injuries (95% confidence interval [CI], 1.99-8.04). Most of the work-related eye injuries occurred in subjects who were 30-49 years old (OR, 3.02, and 95% CI, 1.56-5.82, when compa

Case-control study on renal cell cancer and occupational exposure to trichloroethylene. Part I: Exposure assessment.

A method for a semi-quantitative retrospective assessment of exposure to trichloroethylene (TCE) was implemented for a case-control study conducted in the Arve valley (France), an area with a widely developed screw-cutting industry, where teams of occupational physicians have collected a large quantity of well-documented measurements. A task-exposure matrix was developed to link the main working circumstances in a screw-cutting workshop to corresponding TCE-exposure levels: a 'basic level' was assigned to each task, standing for usual working procedures; exposure circumstances, such as duratio
