Bridge-Enhanced Anterior Cruciate Ligament Repair versus Autograft Reconstruction in Anterior Cruciate Ligament Surgery.

INTRODUCTION: Surgical repair of the anterior cruciate ligament (ACL) using an autograft is the accepted standard treatment for symptomatic ACL tears, but morbidity associated with the harvest of the autograft, residual laxity, and the possibility of a second injury and post-traumatic osteoarthritis have brought about renewed interest in biologic repair. Bridge-enhanced ACL repair (BEAR) employs an extracellular matrix, resorbable implant, and autologous blood to promote healing of the native ligament. MATERIALS AND METHODS: The PubMed, Embase, Cochrane Library, and Scopus databases were quer

Effects of proprioceptive training on gait and plantar pressure after anterior cruciate ligament reconstruction: study protocol for a randomized controlled trial.

BACKGROUND: People who have undergone anterior cruciate ligament reconstruction have an increased risk of osteoarthritis. Abnormality of lower limb kinematics will occur after the operation. This may be related to lower limb muscle strength, the co-excitation of hamstrings and quadriceps femoris, and the weakness of proprioception. Proprioceptive training can improve the proprioception of lower limbs and promote the recovery of lower limb kinematics. Our research objective is to observe whether proprioceptive training can improve the proprioception of lower limbs within 3 months after surgery

A new technique in double-bundle anterior cruciate ligament reconstruction with implant-free tibial fixation.

PURPOSE: This case-series outcome study presents a surgical technique for anatomic double-bundle anterior cruciate ligament (ACL) reconstruction with 4-tunnel using two interference screws. There was a 2-year minimum follow-up. METHODS: From January to December 2009, an ACL 4-tunnel, anatomic, double-bundle reconstruction was performed on 27 patients. Double-strand hamstring tendon grafts were used in each femoral tunnel as well as two interference screws. Tibial fixation was insured through manual tension, by tying non-absorbable sutures on the bone bridge between the two tunnels at 20° of k

Thigh muscle size and strength after anterior cruciate ligament reconstruction and rehabilitation.

It is the hypothesis of the senior author (GAA) that high circumference measurements are not an accurate reflection of thigh muscle cross-sectional area or muscle strength after standard rehabilitation following anterior cruciate ligament reconstruction. Likewise, normal quadriceps femoris strength is not achieved in these patients despite aggressive rehabilitation. The purpose of our study was to quantify thigh muscle size and strength and correlate thigh circumference, muscle cross-sectional area by magnetic resonance imaging (MRI), and isokinetic strength in our patients. Thirty-three patie

Second-look arthroscopic assessment of arthroscopic single-bundle posterior cruciate ligament reconstruction: comparison of mixed graft versus achilles tendon allograft.

BACKGROUND: Although controversy continues over the choice of graft tissue, including autografts, allografts, and synthetic ligaments, for posterior cruciate ligament (PCL) reconstruction, the use of a mixed graft consisting of a hamstring (semitendinosus and gracilis) autograft plus tibialis anterior allograft tendon has not been studied in detail. HYPOTHESIS: Outcomes of PCL reconstructions performed with a mixed graft would be superior to those using solely an Achilles tendon allograft in terms of functional knee scores, posterior stability, and the graft appearance. STUDY DESIGN: Cohort

Twelve-Month Maturation Timeline of Quadriceps Tendon Autograft Signal Intensity Ratio After ACL Reconstruction Surgery Using Magnetic Resonance Imaging.

BACKGROUND: Anterior cruciate ligament (ACL) injuries are among the most common sports-related injuries, accounting for about half of all knee injuries, and most athletes opt to undergo ACL reconstruction (ACLR). The quadriceps tendon (QT) ACLR has only recently been gaining popularity, and therefore few studies exist for evaluating the healing, or ligamentization, timeline of the QT autograft. PURPOSE: To evaluate the signal intensity (SI) ratio of the tendon QT autograft after ACLR during the first postoperative year. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: A prospective

Our Preferred Technique for Bone-Patellar Tendon-Bone Allograft Preparation.

Anterior cruciate ligament (ACL) reconstruction is an increasingly common procedure as orthopaedic patients seek to remain active longer in life, resulting in more ligamentous knee injuries. Graft selection is at the forefront of decision making in knee reconstruction, with advantages and disadvantages to various grafts, including allograft to autograft. Although the gold standard for the ACL reconstruction of elite athletes and highly active patients has traditionally been bone-patellar tendon-bone autograft (BTB), this graft is not without its disadvantages, such as increased operative time,

No difference in postoperative efficacy and safety between autograft and allograft in anterior cruciate ligament reconstruction: a retrospective cohort study in 112 patients.

BACKGROUND: Arthroscopic anterior cruciate ligament reconstruction (ACLR) is the best treatment choice for returning to pre-injury activities following ACL rupture. Although allografts are considered an effective alternative to autografts, there is still controversy regarding the safety and effectiveness of this procedure, especially concerning the risk of postoperative infection and disease transmission. The purpose of this study was to compare the efficacy outcomes and safety between allografts and autografts in primary ACLR. METHODS: The retrospective analysis involved 112 patients (58 pat

Treatment outcome of chronic patellar tendon rupture using autogenous semitendinosus graft: a retrospective case series.

PURPOSE: This study highlights the pattern of presentation, treatment, early functional outcome, and complications observed in the management of chronic patellar tendon ruptures using our preferred technique of autogenous semitendinosus graft reconstruction. METHODS: This was a retrospective case series involving consecutive patients who underwent patellar tendon reconstruction and met the inclusion criteria. The outcome measures were determined by the post-operative knee range of motion (R.O.M), the post-operative International Knee Documentation Committee (IKDC) score, and pattern of post-o

Demographic and surgical factors affect quadriceps strength after ACL reconstruction.

PURPOSE: To investigate the effects of graft source, time since surgery, age, and sex on unilateral and symmetry-based measures of knee extension strength among individuals with ACL reconstruction (ACLR). METHODS: Three hundred and eight individuals aged 13-40 years old with primary, unilateral ACLR in the last 60 months were enrolled in this multi-site clinical measurement study. Participants completed bilateral knee extension maximal voluntary isometric contraction (MVIC) torque assessments which were normalized to body mass (Nm/kg) and limb symmetry indices (LSI) were calculated. Surgical options for chronic patellar tendon rupture in total knee arthroplasty.

PURPOSE: The purpose of this study was to compare mid-term results of three different reconstructive techniques for chronic patellar tendon disruption after total knee arthroplasty (TKA). Several surgical techniques have been proposed, but to date it is still unclear which is the best solution. The hypothesis was that allografts provide better functional results than autografts in restoring a correct joint function. METHODS: Twenty-one reconstructions were performed in twenty-one patients (three groups of seven patients) with chronic patellar tendon lesion following TKA. Group I underwent rec

Semitendinosus force-joint angle relationship after anterior cruciate ligament reconstruction with semitendinosus tendon.

BACKGROUND: The semitendinosus (ST) tendon is among the most frequently used autografts for anterior cruciate ligament reconstruction (ACLR). Persistent deficits in knee flexion strength at flexed knee angles after ACLR may be attributable to a shift in the force-joint angle relationship of the ST caused by post-operative shortening of the muscle belly. This study aimed to investigate the ST muscle morphology and its force-joint angle relationship following ACLR using the ST tendon by comparing the operated and contralateral non-operated limbs. METHODS: Thirteen young adults who had undergone

Comparable Mid- to Long-term Outcomes of Semitendinosus and Semitendinosus/Gracilis Grafts in ACL Reconstruction: Median 8-Year (Range, 5-10 Years) Results From a Randomized Controlled Study.

BACKGROUND: The hamstring tendon autograft used for anterior cruciate ligament reconstruction commonly entails both the semitendinosus and gracilis (ST/G) tendons for sufficient graft thickness, but they may impair function. To mitigate this, a tendon-sparing ST-only graft has been explored. However, less is known of the mid- to long-term outcomes comparing these configurations. PURPOSE/HYPOTHESIS: The purpose was to investigate differences between the quadrupled ST tendon graft and the doubled ST/G graft configuration. The primary hypothesis was that there would be no difference in the Inter

A New Anatomical Approach to T-Junction Hamstring Injuries.

BACKGROUND: Hamstring injuries are the most common muscle injuries in sport, particularly in football. Among these, injuries to the distal musculotendinous T-junction (DMTJ) of the biceps femoris remain poorly characterized and are frequently associated with prolonged recovery and high reinjury rates. The DMTJ is a complex, anatomically variable structure formed by the confluence of the long and short heads of the biceps femoris. OBJECTIVE: To assess whether the anatomical configuration of the DMTJ-specifically the extent of its deep connective tissue limb-is associated with return-to-play (R

Biomechanical Testing of Three Alternative Quadrupled Tendon Graft Constructs With Adjustable Loop Suspensory Fixation for Anterior Cruciate Ligament Reconstruction Compared With Four-Strand Grafts Fixed With Screws and Femoral Fixed Loop Devices.

BACKGROUND: Quadrupled semitendinosus (ST) grafts for anterior cruciate ligament (ACL) reconstruction have advantages of greater graft diameter and gracilis (G) preservation compared with doubled ST-G grafts. However, a paucity of biomechanical data are available regarding different preparation techniques for these constructs. PURPOSE: To biomechanically analyze 3 alternative tendon constructs fixed with adjustable suspensory fixation devices on the femur and tibia compared with a matched 4-strand construct fixed with a tibial screw and femoral fixed loop device. Soft Tissue Reconstruction of the Proximal Tibiofibular Joint by Using Split Biceps Femoris Graft with 5-Year Clinical Follow-up.

Instability of the proximal tibiofibular joint (PTFJ) is a rare clinical condition that presents unique challenges to treatment. We present the case of an active 26-year-old woman with a 4-year history of recurrent PTFJ subluxations, treated surgically at our institution using a split biceps femoris tendon graft for PTFJ reconstruction. She underwent several attempts at nonoperative management until we decided to proceed with surgical intervention. A split biceps femoris graft was used to restore stability of the PTFJ. Approximately 5 years postoperatively, she achieved full range of motion as
