Comparing Composite Scores for the ANAM4 TBI-MIL for Research in Mild Traumatic Brain Injury.

OBJECTIVE: The Automated Neuropsychological Assessment Metrics (Version 4) Traumatic Brain Injury Military (ANAM4 TBI-MIL) is commonly administered among U.S. service members both pre-deployment and following TBI. The current study used the ANAM4 TBI-MIL to develop a cognition summary score for TBI research and clinical trials, comparing eight composite scores based on their distributions and sensitivity/specificity when differentiating between service members with and without mild TBI (MTBI). METHOD: Male service members with MTBI (n = 56; Mdn = 11 days-since-injury) or no self-reported TBI

Large-scale analysis of the prevalence and geographic distribution of HIV-1 non-B variants in the United States.

The genetic diversity of human immunodeficiency virus type 1 (HIV-1) has significant implications for diagnosis, vaccine development, and clinical management of patients. Although HIV-1 subtype B is predominant in the United States, factors such as global travel, immigration, and military deployment have the potential to increase the proportion of non-subtype B infections. Limited data are available on the prevalence and distribution of non-B HIV-1 strains in the United States. We sought to retrospectively examine the prevalence, geographic distribution, diversity, and temporal trends of HIV-1

Influence of mindfulness and coping flexibility in the early phases of burnout development in intensive care unit healthcare workers during the COVID-19 pandemic.

BACKGROUND: Caregivers in intensive care units are exposed to high levels of stress, which were exacerbated during the COVID-19 pandemic, increasing the incidence of stress-related disorders, including burnout. METHODS: We evaluated the influence of mindfulness and coping flexibility in the early days of burnout development in a prospective study of 50 military caregivers working in a mobile resuscitation unit during the COVID-19 pandemic. We visited the participants at the end of deployment (day 0; D0) and three weeks later (D21). On D0, the participants completed questionnaires assessing mi

Relevance of Antibiotic Prophylaxis in Non-Mesh Inguinal Hernia Repair in Remote and Poor Medical Settings in Sub-Saharan Africa.

Background: Because of the poor local medical conditions, and because the surgical site infection (SSI) rate after hernia repair in sub-Saharan Africa is higher than in developed countries, deployed surgeons within Role 2 usually perform non-mesh inguinal herniorrhaphy. Regarding antimicrobial prophylaxis, the latter currently is not recommended in non-mesh inguinal hernia repairs. Our study aimed at assessing the relevance of antibiotic prophylaxis in non-mesh inguinal hernia repair within a Role 2 surgical structure deployed in sub-Saharan Africa. Methods: From January 1 to December 31, 2019

Trends in Upper Extremity Injuries in Female Marines following the Addition of the Pull-up as a Physical Fitness Test Event.

INTRODUCTION: In November 2012, the U.S. Marine Corps announced a policy change to the female Physical Fitness Test (PFT), replacing the flexed arm hang with pull-ups. This change, which became effective on January 1, 2017, aimed to standardize fitness requirements and facilitate gender integration into combat roles. Musculoskeletal injuries (MSKIs) are a leading cause of medical encounters and non-deployability among service members. Upper extremity (UE) injuries are particularly relevant given the physical demands of pull-ups. MATERIALS AND METHODS: A retrospective open cohort study was con

Mediastinal injury is the strongest predictor of mortality in mounted blast amongst UK deployed forces.

BACKGROUND: Blast injury has been the most common cause of morbidity and mortality encountered by UK forces during recent conflicts. Injuries sustained by blast are categorised by the injuring component of the explosion and depend upon physical surroundings. Previous work has established that head injuries and intra cavity haemorrhage are the major causes of death following exposure to under body (mounted) blast but has yet to explore the precise nature of these torso injuries nor the effect of particular injuries upon survival. This study examines the patterns of torso injury within the mount
