Short-term hearing results using ossicular replacement prostheses of hydroxyapatite versus titanium.

The aim of this study was to compare the short-term audiologic results of the ossiculoplasty using partial or total ossicular replacement prostheses (PORP or TORP) made of hydroxyapatite (HA) and titanium (Ti). Fifty-two consecutive patients who underwent ossiculoplasty due to chronic otitis media with or without cholesteatoma were enrolled. Patients were assessed at 6 months postoperatively to establish short-term audiologic results. Preoperative and postoperative air-conduction and bone-conduction thresholds were measured at five frequencies: 0.25, 0.5, 1, 2, and 3 kHz. Postoperative air-bon

[Semi-implantable transcutaneous bone conduction hearing devices].

BACKGROUND: Patients with air-bone gaps or combined hearing loss that cannot be corrected by tympanoplasty can be treated with bone conduction hearing aids. The disadvantages of conventional and percutaneous systems are, on one hand, the obvious external fixation components and on the other hand, the biological and psychosocial problems of open implants. This project was therefore set up to develop a semi-implantable, magnetically anchored transcutaneous bone conduction device, introduce it into clinical application and follow-up the results. PATIENTS AND METHODS: The principle of this bone c

Effects of fenestra size and piston diameter on the outcome of stapes surgery for clinical otosclerosis.

OBJECTIVES: We determined the effect of piston diameter (0.8 or 0.6 mm) and fenestra size (total, three-quarter, or half removal or calibrated stapedotomy) on hearing gains after stapes surgery for clinical otosclerosis. METHODS: We analyzed the mean air conduction hearing gains at various frequencies in a sample of 911 strictly consecutive patients who underwent 1,168 stapes procedures with piston reconstruction carried out by the same surgeon between 1963 and 1979. Assiduous follow-up of patients was attempted for at least 10 years, and in some cases for 20 years. Results over time at the v

Biocompatibility of silver containing silica films on Bioverit® II middle ear prostheses in rabbits.

For several centuries silver is known for its antibacterial effects. The middle ear is an interesting new scope for silver application since chronic inflammations combined with bacterial infection cause complete destruction of the fragile ossicle chain and tympanic membrane. The resulting conductive deafness requires tympanoplasty for reconstruction. Strategies to prevent bacterial growth on middle ear prostheses are highly recommended. In this study, rabbits were implanted with Bioverit® II middle ear prostheses functionalized with silver containing dense and nanoporous silica films which wer

Comparison of two different titanium couplers for an active middle ear implant.

OBJECTIVE: To compare surgical methods, functional gain, and speech discrimination using two different coupling methods for an active middle ear implant. Of several couplers enabling placement of the active element at various locations, two function directly at a cochlear membrane, bypassing a missing or malformed ossicular chain. This study evaluates whether either of these methods is more beneficial. STUDY DESIGN: Retrospective case review. SETTING: ENT surgical clinic. PATIENTS: Forty-seven German-speaking patients with moderate to severe mixed hearing loss. INTERVENTIONS: Records of pa

[Stapes revisions: clinical findings and results].

By correlating the intraoperative findings and the postoperative hearing gain it was attempted to determine the recommendable intraoperative management in stapesrevisions in order to obtain the best postoperative hearing. In a retrospective study the findings of 39 stapes revision operations were analyzed. In most of the cases hearing one year postoperatively was compared with preoperative hearing. The prognosis for good hearing was best following replacement of the prosthesis where the prosthesis was too short or dislocated, or in the presence of a loosened wire. Where treatment only entailed

Patients' lives following stapedectomy complications.

Nowadays, it is widely accepted that patients must be informed about the risks associated with any type of surgical procedure. Usually, the information provided consists of quoting a list of risks and their probability of occurrence based on data from the literature, and, more appropriately, the figures reflecting the surgeon's personal experience. As a rule, such data are sufficient to hold up in court in the event of a lawsuit, but may be insufficient to help patients make the most appropriate choice. We report two cases that presented complications following stapedotomy. Both cases had a se

Surgical management of congenital aural atresia.

Congenital aural atresia is a relatively common anomaly in otology. It requires an early assessment of hearing status and an early evaluation of the degree of atresia. Its surgical management is aimed at obtaining hearing gain and at establishing appropriate auditory canal status for the application of hearing aids. The authors analyzed the preoperative otologic findings, surgical findings, postoperative complications, and postoperative hearing results of 25 ears in 21 patients with congenital aural atresia. From January 1987 to February 1992, 24 of these ears were surgically corrected by ante
