Severe Class II malocclusion with facial asymmetry treated with intraoral vertico-sagittal ramus osteotomy and LeFort I osteotomy.

In this article, we report the successful treatment of a patient, aged 19 years 11 months, who had a severe Class II malocclusion and facial asymmetry. A combination of intraoral vertical-sagittal ramus osteotomy (IVSRO), intraoral vertical ramus osteotomy (IVRO), and LeFort I osteotomy was used for mandibular advancement and maxillary impaction. The patient had a convex profile because of a retrognathic mandible. She also had severe mandibular deviation and vertical maxillary excess with a canted occlusal plane. A deep overbite of 8.0 mm and an excessive overjet of 10.0 mm were observed. Afte

[Maxillo-facial surgery in skeletal Class II: repercussions on the temporo-mandibular joints].

These interventions usually aimed at the correction of the skeletal discrepancy by mandibular retrognatism with an advancement of the distal portion of the mandible after mandibular osteotomy. The position of the condyle is determined during the osteosynthesis with miniplates. Condyles are set back in relation with the supine position of the patient and the weakness of his (her) curarized muscle. All studies show that surgery of mandibular advancement causes a lateral, torque and backward movement of the condyles, all harmful to the condyles. Factors that predispose to condylar resorption are

[Incisor repositioning: a new approach in orthodontics].

Lower incisors axis has a "key" position in different cephalometric analysis. However, several critics are directed towards the cephalometric profile and cephalometric landmarks (point, line and angle). The published norms and the cephalometric standards recommended for the optimal positioning of incisors could only be used as general clinical guidelines. Incisor repositioning to achieve optimal facial aesthetics requires taking into consideration the hard and soft tissues of the face, the profile, the muscular dynamics as well as the facial growth. In this work, we propose a new approach of i

Management of occlusal canting with miniscrews.

A 14-year-old boy with a skeletal Class II malocclusion and open bite whose chief complaint was a posterior crossbite showed a canted occlusal plane with asymmetric gummy smile and mandibular deviation at clinical examination. The treatment with miniscrews focused on the bilateral intrusion of the maxillary posterior teeth and, after resolving the open bite, a new biomechanical technique involving joined miniscrews was applied for an en masse intrusion of the left side. This treatment strategy achieved optimal occlusion with improvements to the sagittal, vertical, and transverse relationships

Three dimensional approach for realistic simulation of facial soft tissue response: a pilot study.

Facial attractiveness is ranked as a principal priority among patients seeking orthodontic treatment or combined surgical orthodontic therapy. A successful treatment planning process necessitates an accurate prediction of the postoperative facial profile. In this manuscript, the simulation procedure on a real clinical case using virtual volumetric 3D mesh through different scenarios of orthognathic procedures was done. Results depict several facial soft tissue outcomes, with the likelihood of sharing with the patient the most esthetically pleasing end result prior to carrying out the surgical
