A pseudoepidemic of Sporothrix cyanescens pneumonia occurring during renovation of a bronchoscopy suite.

Sporothrix cyanescens, an uncommonly isolated mold, was cultured from biopsy tissue, bronchial washings and sputum obtained at bronchoscopy from four patients during a one-month period. These patients had undergone fiberoptic bronchoscopy to aid in the diagnosis of pneumonic infiltrates. During this period of time the bronchoscopy suite was being extensively renovated. Cultures of bronchoscopes, biopsy forceps and effluents from the bronchoscopy injection ports were negative for fungi, as were all environmental samples taken from the bronchoscopy suite. Known pulmonary pathogens were eventuall

Taxonomy of Trichophyton interdigitale (Trichophyton mentagrophytes var. interdigitale) by restriction enzyme analysis of mitochondrial DNA.

The relationship between Trichophyton interdigitale (Trichophyton mentagrophytes var. interdigitale) and the other members of the T. mentagrophytes complex was studied by restriction enzyme analysis of mitochondrial DNA (mtDNA). The mtDNA of 22 isolates of T. interdigitale from Japanese patients with dermatophytosis was extracted, digested with the restriction enzymes Hae III, Msp I, or Hind III, and the restriction profiles obtained compared with those of Arthroderma simii, Arthroderma benhamiae and Arthroderma vanbreuseghemii. The restriction profiles obtained for T. interdigitale were ident

Hendersonula toruloidea as an agent of mycotic foot infection in Gabon.

Pathogenic fungi were isolated from 149 of 210 Gabonese patients with clinical signs of tinea pedis. Hendersonula toruloidea was isolated from 51 patients (34.2%), either alone (in 25%) or in association with a dermatophyte species or Candida albicans. Clinical signs of H. toruloidea infection closely resembled those of dermatophyte infection except that hyperkeratinization of the soles was most often associated with infection by H. toruloidea. H. toruloidea mycelium could be readily distinguished from dermatophyte mycelium on direct examination of skin scrapings. All the H. toruloidea isolate

Treatment of paracoccidioidomycosis with itraconazole.

Forty-seven patients with active paracoccidioidomycosis were treated with itraconazole. The majority of the patients (45) had the chronic adult form of the disease while the remaining exhibited the juvenile form. All of the patients received itraconazole, 100 mg day-1, with the exception of two for whom the clinical condition necessitated a larger dose. The mean duration of therapy was 6 months, with a range of 3-24 months. Clinically, the skin and mucous membrane lesions as well as abdominal pain, auscultatory alterations and dysphonia improved markedly during treatment with a concomitant inc

Comparison of antigens from agents of actinomycetoma by immunodiffusion and electrophoresis procedures.

Antigenic extracts were prepared from culture filtrates of the principal agents known to cause actinomycetoma, namely Actinomadura madurae, Actinomadura pelletieri, Nocardia asteroides, Nocardia brasiliensis, Nocardia otitidis-caviarum, and Streptomyces somaliensis. These antigenic preparations were compared by immunodiffusion (ID), counterimmunoelectrophoresis (CIE), line immunoelectrophoresis (LIE) and rocket line immunoelectrophoresis (RLIE), with rabbit antisera prepared against each of the extracts. Cross-reactivity between antigenic extracts from the different actinomycetes, measured by

Saksenaea vasiformis infection in a burn wound.

Saksenaea vasiformis was isolated, after 18 days, from a burn wound caused by a petrol bomb. The fungus was locally invasive and was treated successfully by excision and systemic administration of amphotericin B. Identification of the isolate was accomplished by inducing sporulation on 1% agar containing grass clippings and on hay infusion agar.

Pulmonary mycosis in farmed deer: allergic zygomycosis and invasive aspergillosis.

During 1988, pulmonary mycosis was diagnosed in four of 116 farmed deer examined on suspicion of tuberculosis. The histopathology showed allergic bronchopulmonary mycosis in a red deer (Cervus elaphus) and the agent was identified as a zygomycete, probably Absidia corymbifera, by immunofluorescence staining. Three fallow deer (Dama dama) had invasive necrotizing mycotic pneumonia and progressive exudative mycotic alveolitis caused by Aspergillus fumigatus. In the red deer, weakness due to paratuberculosis had probably promoted the mycotic infection. The three fallow deer were bred on another f

Invasive aspergillosis: clinical and pathological features of a new animal model.

A new animal model of invasive aspergillosis is described in which female New Zealand White rabbits were immunosuppressed with corticosteroids and cyclophosphamide and were given an intratracheal inoculation of 4 x 10(4) conidia of Aspergillus fumigatus. Thirteen of 15 animals survived during a 10-day-period of observation. Most had clinical signs of a respiratory infection (dyspnoea) and at autopsy there was macroscopic and microscopic evidence of invasive pulmonary aspergillosis. Six control animals (infected but not immunosuppressed) showed no such signs. The extent of hyphal invasion was a

Identification of antigenic polypeptides of Paracoccidioides brasiliensis by immunoblotting.

Exoantigens from cultures of Paracoccidioides brasiliensis were separated by SDS-PAGE and subsequently blotted onto nitrocellulose paper. Immunologically reactive components were detected by reaction with sera from patients with paracoccidioidomycosis (PCM). Peroxidase-conjugated anti-human IgG was used as a detector antibody to reveal immunocomplexes. Anti-P. brasiliensis IgG reacted with four major components of 70, 52, 43 and 20-21 kDa and with eight minor components. The 43 kDa glycoprotein (gp) was the predominant IgG reactive antigen, recognized by 100% of the patient's sera. The antibod

Evaluation of the pathogenicity and immunogenicity of seven Paracoccidioides brasiliensis isolates in susceptible inbred mice.

In this investigation the pathogenicity and immunogenicity of seven isolates of Paracoccidioides brasiliensis were compared. The pathogenicity of each isolate was determined by 50% lethal dose estimations and histopathology analysed during a 24-week-period. Four basic patterns of virulence could be defined after intraperitoneal infection of susceptible, genetically homogenous, B10.A mice, namely, slightly virulent (isolates Pb 265 and IVIC Pb 267), intermediate (isolates Pb 192, IVIC Pb 9 and Pb SN), virulent (isolate Pb 2052) and highly virulent (isolate Pb 18). The granulomas induced by the

Suppression of IgE antibody production against an unrelated antigen in experimental murine paracoccidioidomycosis.

A suppression of the IgE antibody response to ovalbumin was obtained in susceptible mice infected with Paracoccidioides brasiliensis yeast cells a few days prior to immunization with the former antigen plus adjuvant. A direct relationship between the number of injected fungi and the suppressive effect was established. When infection with a pathogenic isolate of P. brasiliensis (Pb 18) was compared to a non-pathogenic isolate (IVIC Pb267), the IgE anti-ovalbumin response was reduced by both. A similar effect was observed if mice were injected with dead yeast cells prior to immunization. Two str

HLA-A29 and genetic susceptibility to chromoblastomycosis.

The distribution of 12 HLA-A, 14 HLA-B, seven HLA-C, seven HLA-DR and three HLA-DQ antigens was determined in 32 non-consanguineous white Brazilians suffering from chromoblastomycosis and 77 healthy controls, matched for ethnic background, sex and age and living in the same geographical area. A significant difference between the two groups was seen only in respect to one HLA-A antigen: A29 was present in 28% of patients as opposed to 4% of the controls (P corrected = 0.03). This finding indicates that susceptibility to chromoblastomycosis may be influenced by a gene located on chromosome 6, in

Correlative studies of blastogenic responses and interleukin 1 production by mononuclear cells from patients with zoopathogenic fungal infections.

The production of interleukin 1 (IL 1) by adherent peripheral blood mononuclear cells (PBMC) was quantitated in 16 individuals infected with Histoplasma capsulatum or Blastomyces dermatitidis and 16 age-matched controls. In parallel, we measured blastogenic responses to phytohemagglutinin (PHA) by PBMC from patients and controls. Of the 16 patients, six had pulmonary histoplasmosis, six had disseminated histoplasmosis, two had pulmonary blastomycosis, and two had disseminated blastomycosis. At the time of study, none of the patients were receiving immunosuppressive agents or had an underlying

In situ localization of T lymphocyte subsets in human paracoccidioidomycosis.

Immunohistochemical techniques using monoclonal antibodies to T lymphocyte subsets were used to characterize granulomas caused by Paracoccidioides brasiliensis. Punch biopsies of skin or mucosa from eight patients and of lymph nodes from two patients with disseminated paracoccidioidomycosis were studied. The T lymphocytes were distributed either in a localized pattern related to epithelioid granulomas or in a diffuse arrangement, predominantly around the vessels. In the granulomas, T cells formed a peripheral mantle surrounding central aggregates of macrophages. The majority of lymphocytes wer

Rhinocerebral zygomycosis caused by Saksenaea vasiformis.

A fatal rhinocerebral zygomycotic infection caused by Saksenaea vasiformis in a 71-year-old man was diagnosed based on the presence of broad, infrequently septate, branched, hyaline hyphae in tissue obtained from the right and left base of the skull, soft tissue, both maxillary sinuses and the sphenoid sinus; isolation of S. vasiformis from the tissue; and demonstration of mucoraceous antibodies by enzyme-linked immunosorbent assay. Fluorescent antibody studies carried out with a Rhizopus arrhizus conjugate, stained hyphal fragments in tissue with a 2+ intensity.

Corneal ulcer caused by Bipolaris hawaiiensis.

Following an injury to the right eye, a corneal ulcer with hypopyon developed in a leprosy patient. Direct examination of the corneal scrapings on three occasions showed septate, branched, dematiaceous hyphal elements. When scrapings were cultured on Sabouraud's glucose and brain heart infusion agars. Bipolaris hawaiiensis was isolated repeatedly. The patient responded successfully to treatment with nystatin ointment, although the central opacity of the cornea remained and visual acuity did not improve.

Experimental blastomycosis pneumonia in mice by infection with conidia.

We describe a murine model of acute blastomycosis pneumonia induced by Blastomyces dermatitidis conidia. Anesthetized mice were infected by placing a droplet containing conidia suspended in sterile saline on the nares and allowing the suspension to be aspirated into the lungs. Conidia were obtained from mycelia derived from the yeast forms of two well characterized strains of B. dermatitidis, ATCC 26199 and 26197. ATCC 26199 yeast and conidia were highly virulent in BALB/cByJ mice whereas both forms of ATCC 26197 were incapable of causing disease in 60 days. Conidia and yeasts of the virulent

Human Exserohilum and Bipolaris infections: report of Exserohilum nasal infection in a neutropenic patient with acute leukemia and review of the literature.

A neutropenic patient with acute myeloid leukemia developed nasal and perinasal infection caused by the fungus Exserohilum rostratum. Early amphotericin B treatment along with marrow recovery resulted in resolution of the infection. A review of other previously reported cases of Exserohilum and Bipolaris infections show a favourable outcome in most patients who receive systemic antifungal treatment with amphotericin B.
