BACKGROUND: Most lateral extra-articular tenodesis (LET) procedures rely on passing a strip of the iliotibial band (ITB) under the fibular (lateral) collateral ligament and fixing it proximally to the femur. The Ellison procedure is a distally fixed lateral extra-articular augmentation procedure with no proximal fixation of the ITB. It has the potential advantages of maintaining a dynamic element of control of knee rotation and avoiding the possibility of overconstraint.
HYPOTHESIS: The modified Ellison procedure would restore native knee kinematics after sectioning of the anterolateral capsu

We report a case of ulnar subluxation of a ruptured extensor pollicis longus tendon at the metacarpophalangeal joint due to tethering of the distal tendon and an abnormal vector of pull. In this case, thumb flexion with the tethered EPL tendon resulted in attenuation of the radial sagittal band of the extensor hood. After plication of the radial sagittal band and relocation of the EPL tendon, adequate thumb extension was restored.

Recent anterior cruciate ligament (ACL) research focuses on risk factors for ACL graft failure and techniques and augmentations to limit failure. One of the most recognized risk factors is sagittal malalignment in the form of high posterior tibial slope (PTS), especially PTS ≥12°, which leads to increased force through the ACL and ACL graft. To reduce the risk associated with increased PTS, lateral augmentation techniques, typically either a lateral extra-articular tenodesis or an anterolateral ligament reconstruction, improve clinical outcomes, and the authors preferred graft choice, particul

Anterior cruciate ligament (ACL) injuries are on the rise at all levels of sport, including elite athletics. ACL injury can have implications on the athlete's sport longevity, as well as other long-term consequences, such as the development of future knee osteoarthritis. In the elite athlete, ACL injury can also have ramifications in terms of contract/scholastic obligations, sponsorships and revenue-generating potential. Although the goal of anterior cruciate ligament reconstruction (ACLR) is to return any athlete to the same preinjury level of sport, management of ACL injuries in the elite at

Turf toe is hyperextension injury of the plantar plate at the first metatarsophalangeal joint. Etiologies have often included sports/activities with excessive forefoot axial loading and/or violent pivotal movements. The purpose of the systematic review was to systematically review and present an overview for the current evidence-based treatment options of turf toe. Both authors systematically reviewed the PubMed and EMBASE databases from inception to April 2016 based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. The level of evidence and quality of evide

Iatrogenic hallux varus is a dreaded complication of hallux valgus surgery, consisting in 1st-ray deformity in the form of medial malalignment of the 1st phalanx with respect to the metatarsal axis. Such over-correction results from imbalance between excessive medial capsule retraction or tensioning and excessive lateral laxity or soft-tissue release. There may be loss of medial stability of bone origin due to excessive "exostosectomy" or excessive intermetatarsal angle closure. Following excessive lateral release, the imbalance gradually induces a varus deformity of the 1st phalanx due to tra

BACKGROUND: Studies establishing the feasibility of anterior cruciate ligament (ACL) reconstruction as an outpatient procedure in France were usually conducted with hamstring tendon grafts. The objective of this study was to evaluate the outcomes of outpatient ACL reconstruction according to whether the graft was harvested from the hamstring tendons or patellar tendon.
HYPOTHESIS: Outpatient ACL reconstruction can be performed using any type of graft.
METHODS: A single-centre retrospective study was conducted in consecutive patients older than 16 years who had primary ACL reconstruction usin

The use of shoulder arthroplasty has been increasing over the last decade, with nearly 20,000 shoulder arthroplasties being performed each year. Although many patients have excellent results, there exists a subset of patients in whom anterior catching shoulder pain develops after arthroplasty. The purpose of this study was to examine this group of patients and explore treatment options and outcomes for this condition. We undertook a review of 8 shoulders in 7 patients who were treated for anterior shoulder pain radiating into the biceps muscle after shoulder arthroplasty. Three patients had a

INTRODUCTION: The cup and cone technique, first described by Carrol and Hill is very simple. It affords excellent contact between bones, and allows all possible adjustments of the arthrodesis in three planes before its final fixation.
MATERIAL AND METHODS: Fifty seven cup and cone arthrodesis of the thumb were performed in 41 adult tetraplegic patients. Level of arthrodesis was i.p. in 28 cases, TM in 25 cases and MP in 4 cases. Distribution in Giens classification was 3 group 1, 14 group 2, 9 group 3, 23 group 5, 2 group 6, 1 group 7, 2 group 10. Mean follow-up was 51 months. Three criteria

BACKGROUND: While authors have subclassified long head of biceps tendon (LHBT) lesions in numerous ways, there is no classification system based solely on the arthroscopic appearance of the LHBT. We present a scoping review of the existing classification systems for LHBT tendinopathy and propose a novel classification based on its intraoperative, arthroscopic appearance.
METHODS: We conducted a comprehensive search of classification systems for LHBT pathology using EMBASE and Ovid-Medline platforms. Our proposed classification system consists of four types based on intraoperative arthroscopic

PURPOSE: Disruption of the scapholunate (SL) ligament and secondary stabilizers impacts carpal kinematics, leading to instability and arthritis. There is no consensus on the most effective reconstruction technique for irreparable scapholunate dissociation. The purpose of this biomechanical study was to compare three scapholunate reconstruction techniques in a cadaveric model.
METHODS: Eighteen cadaveric upper extremities without prior scapholunate disruption or degenerative changes were used. Markers were affixed to the scaphoid, lunate, and Lister's tubercle. After transection of primary and

BACKGROUND: After high-level spinal cord injury (SCI), motor recovery of wrist extensors has significant functional consequences as it can facilitate grasp through tenodesis. A small seminal study from 1993 established widely used prognostic rules for wrist extensor recovery, though these have not been replicated with larger, real-world samples since inception.
OBJECTIVE: To determine the prognostic value of initial C5 sharp-dull discrimination, C5 motor strength, and C6 motor strength on predicting 1-year ipsilateral wrist extensor motor recovery for patients with SCI with neurological level

BACKGROUND: Regaining hand and upper extremity function is a functional priority for patients with cervical spinal cord injuries (SCI). Despite the high demand for upper extremity reconstruction (UER) to restore function, the utilization of these procedures remains low. This study evaluates the incidence and timing of UER in patients with cervical SCI.
METHODS: The TriNetX Research Network was queried to identify patients with cervical SCI. Then, we identified patients who underwent procedures to restore motor function (nerve transfer, tendon transfer, tenodesis, or arthrodesis) and procedure

BACKGROUND: Degenerative rotator cuff tears are common and are often treated with surgical repair. Randomised trials have not shown a clear advantage to surgery over non-surgical treatment, but there have been no published placebo-controlled trials investigating rotator cuff repair. This study aims to compare arthroscopic shoulder surgery with rotator cuff repair to surgery without rotator cuff repair (placebo) for improving shoulder pain and function in people with shoulder pain and full-thickness degenerative rotator cuff tears.
METHODS: The study is a multicentre two-parallel arm, blinded,

BACKGROUND: Intra-articular soft arthroscopic Latarjet technique (in-SALT) involves augmentation of arthroscopic Bankart repair (ABR) with soft tissue tenodesis of long head of biceps to upper subscapularis. This study was conducted to investigate superiority of outcomes of in-SALT-augmented ABR over those of concurrent ABR and anterosuperior labral repair (ASL-R) in management of type V superior labrum anterior-posterior (SLAP) lesion.
METHODS: This prospective cohort study (conducted between January 2015 and January 2022) included 53 patients with arthroscopic diagnosis of type V SLAP lesio

PURPOSE: The impact of posterolateral tibial plateau impaction fractures (TPIF) on posttraumatic knee stability in the setting of primary anterior cruciate ligament (ACL) tear is unknown. The main objective was to determine whether increased bone loss of the posterolateral tibial plateau is associated with residual rotational instability and impaired functional outcome after ACL reconstruction.
METHODS: A cohort was identified in a prospective enrolled study of patients suffering acute ACL injury who underwent preoperative standard radiographic diagnostics and clinical evaluation. Patients we

Reasons for failure of anterior cruciate ligament (ACL) reconstructions are manifold and require a multifactorial explanatory approach. In addition to technical failures, many modifiable and non-modifiable risk factors for a new ACL injury have to be considered. Technical failures primarily include non-anatomical tibial and femoral tunnel position. In comparison to the transtibial drilling technique, the tibial tunnel-independent technique results in a more anatomical position of the femoral tunnel and should therefore be preferred. One can differentiate between non-modifiable and modifiable r
