Phenoconversion in pure autonomic failure: a multicentre prospective longitudinal cohort study.

We aimed to describe the clinical features of patients with pure autonomic failure (PAF) preceding phenoconversion that could be useful as predictive markers for advancing α-synuclein-associated neurodegeneration of the brain. Patients diagnosed with PAF were evaluated at eight centres (seven US-based and one European) and enrolled in a longitudinal observational cohort study (NCT01799915). Subjects underwent detailed assessments of motor, sleep, olfactory, cognitive and autonomic function and were followed prospectively to determine whether they developed parkinsonism or dementia for up to 10

Cerebral hyperemia and impaired cerebral autoregulation associated with diabetic ketoacidosis in critically ill children.

OBJECTIVE: Cerebral edema associated with diabetic ketoacidosis is an uncommon but severe complication of insulin-dependent diabetes mellitus with unclear pathophysiology. We sought to determine whether cerebral edema in patients with diabetic ketoacidosis was related to changes in cerebral blood flow, autoregulation, regional cerebral saturation, or S100B. DESIGN: Prospective case series. SETTING: Pediatric intensive care unit of a tertiary children's hospital. PATIENTS: Six patients with diabetic ketoacidosis and altered mental status, requiring computed tomographic scan of the head. INT

Clinical significance of REM sleep behavior disorder in Parkinson's disease.

OBJECTIVE: Rapid eye movement (REM) sleep behavior disorder (RBD) may be a risk factor for dementia development in patients with Parkinson's disease (PD); however, the role of subclinical RBD remains unknown. Patients with PD and clinical RBD, subclinical RBD, or with normal REM sleep were examined in a cross sectional study and a longitudinal follow-up. METHODS: Interviews regarding RBD symptoms and polysomnographies were performed on 82 PD patients divided into RBD subcategories based on the presence/absence of REM sleep without atonia (RWA) and/or RBD symptoms. Descriptive variables were c

In the Search of the Ideal Moroccan Feminine Beauty.

BACKGROUND: The perception of facial beauty is shaped by a dynamic interplay of cultural, generational, and media-driven influences. Despite growing interest in aesthetic standards worldwide, there is a lack of data on facial beauty preferences within the Moroccan population. OBJECTIVE: This study aims to identify the facial features considered most attractive by Moroccan individuals and to examine how these preferences vary across age, gender, and geographic region. METHODS: A cross-sectional survey was conducted among 1000 Moroccan participants using digitally modified, hand-drawn anatomic

Cerebrovascular regulation and vasoneuronal coupling.

Maintenance of cerebral perfusion pressure is a prerequisite for the prevention of cerebral ischemia. Physiological fluctuations in systemic perfusion pressure are compensated by cerebrovascular autoregulation. Cerebral hypoperfusion could result from (1) systemic hemodynamic failure (eg, distal to severe arterial stenosis), overcharging the vasoregulatory capacity; (2) dysfunction and exhaustion of cerebrovascular autoregulation; or (3) both. Ultrasound offers an excellent temporal resolution, is noninvasive, and is easily applicable for follow-up investigations. Despite its poor spatial reso

Paradoxical effect of sibutramine on autonomic cardiovascular regulation.

BACKGROUND: Sibutramine, a serotonin and norepinephrine transporter blocker, is widely used as an adjunctive obesity treatment. Norepinephrine reuptake inhibition with sibutramine conceivably could exacerbate arterial hypertension and promote cardiovascular disease. METHODS AND RESULTS: In 11 healthy subjects (7 men, age 27+/-2 years, body mass index 23.1+/-0.7 kg/m2), we compared the effect of sibutramine or matching placebo (ingested 26, 14, and 2 hours before testing) on cardiovascular responses to autonomic reflex tests and to a graded head-up tilt test. In addition, we tested sibutramine

Clinical Presentation, Diagnostic Delays, and Treatment Outcomes in Postural Orthostatic Tachycardia Syndrome (POTS): An Observational Case Series Study in a Single-Centre District General Hospital.

BACKGROUND: Postural orthostatic tachycardia syndrome (POTS) is a heterogeneous disorder of autonomic regulation characterised by unexplained orthostatic tachycardia in the absence of postural hypotension. POTS is a complex and challenging diagnosis owing to the non-specific nature of the presentations, which frequently overlap with other medical conditions. There is limited availability of data and research describing the spectrum of clinical presentations, diagnostic pathways, comorbidities, and management outcomes. This study aims to describe the above in a single-centre district general ho

Nitric oxide-sensitive guanylyl cyclase signaling affects CO2-dependent but not pressure-dependent regulation of cerebral blood flow.

Cerebrovascular CO2 reactivity is affected by nitric oxide (NO). We tested the hypothesis that sildenafil selectively potentiates NO-cGMP signaling, which affects CO2 reactivity. Fourteen healthy males (34 ± 2 yr) were enrolled in the study. Blood pressure (BP), ECG, velocity of cerebral blood flow (CBF; measured by transcranial Doppler), and end-tidal CO2 (EtCO2) were assessed at baseline (CO2 ~39 mmHg), during hyperventilation (CO2 ~24 mmHg), during hypercapnia (CO2 ~46 mmHg), during boluses of phenylephrine (25-200 µg), and during graded head-up tilting (HUT). Measurements were repeated 1 h

Persistent unexplained chest pain and dyspnea in a patient with coronary artery disease: a case report.

BACKGROUND: Neurogenic orthostatic hypotension, a sustained decrease in blood pressure upon standing, is caused by autonomic nervous system failure and characterized by an insufficient increase in heart rate needed to maintain blood pressure upon standing. In this case, neurogenic orthostatic hypotension symptoms preceded a diagnosis of Parkinson disease. A diagnosis of underlying neurogenic orthostatic hypotension significantly changed the course of treatment for this patient. CASE PRESENTATION: An 84-year-old woman was referred to a cardiologist by her primary care practitioner for evaluati

Treatment-induced neuropathy of diabetes: an acute, iatrogenic complication of diabetes.

Treatment-induced neuropathy in diabetes (also referred to as insulin neuritis) is considered a rare iatrogenic small fibre neuropathy caused by an abrupt improvement in glycaemic control in the setting of chronic hyperglycaemia. The prevalence and risk factors of this disorder are not known. In a retrospective review of all individuals referred to a tertiary care diabetic neuropathy clinic over 5 years, we define the proportion of individuals that present with and the risk factors for development of treatment-induced neuropathy in diabetes. Nine hundred and fifty-four individuals were evaluat

Dysautonomia and Postural Orthostatic Tachycardia Syndrome (POTS) in the ENT Clinic: Differentiating Orthostatic Dizziness From Vestibular Migraine and Persistent Postural-Perceptual Dizziness (PPPD).

One of the most common complaints in ENT clinics is dizziness. Although most cases are caused by vestibular neuritis and benign paroxysmal positional vertigo (BPPV), a significant number of patients experience non-vestibular dizziness. One non-vestibular cause is postural orthostatic tachycardia syndrome (POTS), which presents with symptoms similar to vestibular migraine (VM) and persistent postural perceptual dizziness (PPPD). Dizziness from POTS is related to dysautonomia rather than the vestibular system. In otolaryngology, understanding the autonomic nature of non-vestibular dizziness is i

Exposure of a liquefied gas container to an external fire.

In liquefied gas, bulk-storage facilities and plants, the separation distances between storage tanks and between a tank and a line of adjoining property that can be built are governed by local regulations and/or codes (e.g. National Fire Protection Association (NFPA) 58, 2004). Separation distance requirements have been in the NFPA 58 Code for over 60 years; however, no scientific foundations (either theoretical or experimental) are available for the specified distances. Even though the liquefied petroleum gas (LPG) industry has operated safely over the years, there is a question as to whether

Stage-dependent clinical burden in hospitalized Parkinson's disease with a positive bedside orthostatic hypotension screen: a retrospective cross-sectional analysis.

BACKGROUND: Orthostatic hypotension (OH) is common in Parkinson's disease (PD), but what is known of its clinical associations comes from ambulatory or population-based cohorts assessed under protocolised or tilt-table conditions. The patients whom a routine bedside screen identifies on a general ward have not been described. OBJECTIVE: To characterise the modified Hoehn-Yahr (H-Y) stage distribution of hospitalized PD patients with a positive bedside OH screen, and to describe how clinical burden is distributed across stage within that group. METHODS: Retrospective cross-sectional analysis

Effect of delayed supine positioning after induction of spinal anaesthesia for caesarean section.

BACKGROUND: The study tested the hypothesis that the incidence of hypotension during spinal anaesthesia for caesarean section is less in parturients who remain in the sitting position for 3 min compared with parturients who are placed in the modified supine position immediately after induction of spinal anesthesia. METHODS: Spinal anaesthesia was induced with the woman in the sitting position using 2.8 ml hyperbaric bupivacaine 0.5% at the L(3-4) or L(2-3) interspace. Ninety-eight patients scheduled for elective caesarean section under spinal anaesthesia were randomised to assume the supine p

Safety and efficacy of droxidopa for dysautonomia in adults with Menkes disease and occipital horn syndrome in the USA: a randomised phase 1/2a crossover trial.

BACKGROUND: Menkes disease is an X-linked recessive disorder of human copper metabolism. Droxidopa is a synthetic amino acid effective in reversing neurogenic orthostatic hypotension and correcting neurochemical abnormalities in congenital absence of dopamine-beta-hydroxylase (DBH), a copper-dependent enzyme that influences autonomic function. Individuals with disorders associated with variants in the copper transport gene ATP7A may manifest symptoms of dysautonomia, due to deficient DBH activity. We aimed to evaluate the safety and efficacy of droxidopa for dysautonomia in adults with Menkes

Heart rate variability analysis in patients with multiple sclerosis.

BACKGROUND: Multiple sclerosis can cause cardiovascular autonomic dysfunction. It is assumed that is caused by multiple demyelinating plaques localized in the brain stem and spinal cord. Previous studies have determined this using tilt table test, heart rate responses to Valsalva maneuver and deep breathing and heart rate variability analysis with 24 h Holter monitoring. However there is not a consensus regarding the presence of the relationship between autonomic dysfunction and severity of multiple sclerosis, type of multiple sclerosis and expanded disability status scale. The aim of the stud

Prevalence and incidence of orthostatic hypotension in patients with Parkinson's disease: an updated systematic review and meta-analysis.

AIM: Systematically quantifying the prevalence and incidence of orthostatic hypotension in Parkinson's disease patients. BACKGROUND: Orthostatic hypotension is a common non-motor symptom in Parkinson's disease patients. Although systematic reviews have been published previously, existing data are outdated, and there is a lack of research examining differences in the prevalence and incidence of this symptom across various etiologies. An update of current studies is urgently needed. DESIGN: Systematic review and meta-analysis. METHODS: A systematic search was conducted across the following da

Interventions for managing skeletal muscle spasticity following traumatic brain injury.

BACKGROUND: Skeletal muscle spasticity is a major physical complication resulting from traumatic brain injury (TBI), which can lead to muscle contracture, joint stiffness, reduced range of movement, broken skin and pain. Treatments for spasticity include a range of pharmacological and non-pharmacological interventions, often used in combination. Management of spasticity following TBI varies from other clinical populations because of the added complexity of behavioural and cognitive issues associated with TBI. OBJECTIVES: To assess the effects of interventions for managing skeletal muscle spas
