New treatments for panic.

Panic disorder is a chronic condition for many patients and can be socially, emotionally and occupationally disabling. Until recently, clomipramine and alprazolam were the only drugs approved for its treatment. While widely used in the US and Europe, both belong to drug classes (tricyclics and benzodiazepines) with well-recognised side effects that can be problematic and thus limit their use. Recently, paroxetine became the first selective serotonin reuptake inhibitor to receive approval and licensing for panic disorder. The short- and long-term efficacy and tolerability of paroxetine in panic

[Two case reports of cerebral autosomal dominant arteriophaty with subcortical infarctions and leukoencephalopathy (CADASIL)].

Cerebral autosomal dominant arteriopathy with subcortical infarctions and leukoencephalopathy (CADASIL) was first reported in European families and since 1993 it has been observed in America, Africa and Asia, suggesting that today the disease probably still remains largely underdiagnosed. CADASIL appears to be essentially a disorder of the arteries linked to single missense mutations in the Notch3 gene locus on chromosome 19; the aberrant dimerisation of Notch3, due to abnormal disulphide bridging with another Notch3 molecule or with another protein, may be involved in the pathogenesis of the

An investigation of outcome expectancies as a predictor of treatment response for combat veterans with PTSD: comparison of clinician, self-report, and biological measures.

BACKGROUND: Outcome expectancy, or the degree to which a client believes that therapy will result in improvement, is related to improved treatment outcomes for multiple disorders. There is a paucity of research investigating this relation in regards to posttraumatic stress disorder (PTSD). Additionally, the bulk of the research on outcome expectancy and treatment outcomes has relied mostly on self-report outcome measures. METHODS: The relation between outcome expectancy on self-report measures, clinician-rated measures, and two biological indices (fear-potentiated startle and cortisol reactiv

Issues in the assessment of treatment response in panic disorder with special reference to fluvoxamine.

Assessment of treatment response in panic disorder is complicated by the multidimensional aspects of panic disorder and agoraphobia, the short-term benefits from nonspecific aspects of treatment, and placebo response. Response to treatment with psychological and pharmacologic treatments of panic disorder is reviewed in this context. The experience of several Phase III studies of fluvoxamine in the treatment of panic disorder is examined as an illustrative example. When the response to placebo or comparison treatment in a study is high, no conclusion can be drawn about the true efficacy of the

Treatment of comorbid substance use and anxiety disorders: a case study.

This case study of combined anxiety with both alcohol and benzodiazepine dependence illustrates key issues in presentation, differential diagnosis and management. The case is discussed from a biopsychosocial perspective with each of the discussants focusing on their particular area of experience and expertise, then the treatment package is presented in an integrated fashion. Of particular interest is how social anxiety disorder may become a significant barrier to engagement and retention, and thus outcome in persons presenting for addiction treatment, and how a treatment plan for such patients

Identification of the Potential Pharmacotherapeutic Risk in Hospitalized Geriatric Patients.

INTRODUCTION: Geriatric patients need carefully tailored pharmacotherapeutic approaches to minimize the risks of inappropriate treatment and hospitalizations. In Slovakia, there is a lack of studies documenting the medication use patterns in hospitalized geriatric patients. The primary outcome of our study was to identify potential pharmacotherapeutic risks in hospitalized geriatric patients. METHODS: We performed a retrospective analysis of the medical records from 122 patients (65 years and older) hospitalized in the geriatric department. Potential pharmacotherapeutic risk was assessed by i

In Vitro to In Vivo Scalars for Drug Clearance in Nonalcoholic Fatty Liver and Steatohepatitis.

In vitro-in vivo extrapolation (IVIVE) allows prediction of clinical outcomes across populations from in vitro data using specific scalars tailored to the biologic characteristics of each population. This study experimentally determined scalars for patients with varying degrees of nonalcoholic fatty liver disease (NAFLD), ranging from fatty liver to nonalcoholic steatohepatitis (NASH) and cirrhosis. Microsomal, S9, and cytosol fractions were extracted from 36 histologically normal and 66 NAFLD livers (27 nonalcoholic fatty liver [NAFL], 13 NASH, and 26 NASH with cirrhosis). Corrected microsoma

LC-MS-MS Determination of 88 Psychotropic Drugs in 1,865 Hair Samples from Addicts in Drug Abstinence.

The emergence of novel drugs and the continuous expansion of the scope of the types of drugs under control have greatly increased requests for screening of a range of drugs in hair. Here, a multi-analyte method for the detection and quantification of 88 psychotropic drugs in the hair of addicts in drug abstinence was developed and fully validated using liquid chromatography-tandem mass spectrometry (LC-MS-MS). Hair samples (25 mg) were washed, cut into pieces, cryogenically ground and extracted in methanol. The extracted analytes were separated on an Allure PFP Propyl column (100 × 2.1 mm, 5 m

New GABA-Targeting Therapies for the Treatment of Seizures and Epilepsy: II. Treatments in Clinical Development.

The inhibitory neurotransmitter γ-aminobutyric acid (GABA) plays an important role in the modulation of neuronal excitability, and a disruption of GABAergic transmission contributes to the pathogenesis of some seizure disorders. Although many currently available antiseizure medications do act at least in part by potentiating GABAergic transmission, there is an opportunity for further research aimed at developing more innovative GABA-targeting therapies. The present article summarises available evidence on a number of such treatments in clinical development. These can be broadly divided into th

Presentations to the emergency department with non-medical use of benzodiazepines and Z-drugs: profiling and relation to sales data.

BACKGROUND: Non-medical use of benzodiazepines and Z-drugs is common; however, there is limited information available on the extent of harm related to this in Europe, as well as the relationship between misuse and availability. AIM: To describe presentations to the emergency department in Europe related to the recreational use of benzodiazepines and Z-drugs and compare regional differences in these presentations with legal drug sales of benzodiazepines and Z-drugs within each country. METHODS: Emergency department presentations with recreational misuse of benzodiazepines and Z-drugs were obt

Risk of miscarriage after benzodiazepine use during pregnancy: updated systematic review and meta-analysis.

BACKGROUND: Benzodiazepine use during pregnancy remains common despite guidelines discouraging it, except for short-term treatment of severe anxiety or agitation. Previous studies have suggested an increased risk of miscarriage after in utero exposure, raising concern at the European level. However, the most recent meta-analysis, published in 2020, included neither several large-scale observational studies nor an assessment by specific benzodiazepine agents. This study aimed to provide an updated synthesis of evidence on the association between early pregnancy exposure to benzodiazepines and m

Clinical Characteristics and Treatment of Benzodiazepine Withdrawal Delirium in Adults: An Illustrative Case and Systematic Review of Published Cases.

BACKGROUND: Benzodiazepines (BZDs) are widely prescribed in diverse populations and BZD withdrawal can cause delirium, especially for individuals with chronic use patterns. OBJECTIVE: This paper identifies an illustrative case of benzodiazepine-withdrawal delirium and reviews the literature regarding the complex clinical presentation and subsequent management strategies. METHODS: We present a case in which withdrawal from long-term use of lorazepam before a medical admission contributed to complex delirium and how this was managed. This is followed by a literature search of English language

Interdisciplinary pharmaceutical care model engaging patient participation alleviate the anticholinergic burden in community-dwelling older adults: subgroup analysis of a randomized controlled trial.

BACKGROUND: Drugs with anticholinergic (AC) effects can cause adverse reactions such as dry mouth, blurred vision, urinary retention, and cognitive impairment, and have been linked to poorer health outcomes, including reduced quality of life, higher mortality, and longer hospital stays. Older adults are particularly susceptible due to age-related changes, and polypharmacy may further increase risk. While tools exist to identify anticholinergic burden (ACB), real-world management remains challenging. Pharmacist-led or multidisciplinary reviews can reduce ACB, but such interventions are uncommon

Prescription drug dependence with and without concurrent illicit drug use: a multicenter cross-sectional survey among an addiction treatment seeking population.

BACKGROUND: Dependence on prescription drugs and illicit drugs imposes a global health and social burden. Despite accumulating evidence of prescription drugs and illicit drugs dependence, none of the systematized studies has explored the magnitude of this problem in Pakistan. The aim is to investigate the extent and associated factors of prescription drug dependence (PDD), as opposed to concomitant prescription drug dependence and illicit drug use (PIDU), within a sample of individuals seeking addiction treatment. METHODS: The cross sectional study was conducted on the sample recruited from t

Deprescribing zolpidem reduces falls and fractures in patients with cirrhosis.

BACKGROUND & AIMS: Benzodiazepines are associated with an increased risk of harm in patients with cirrhosis. However, stopping benzodiazepines must be done with care to avoid withdrawal or other unintended consequences. The impact of deprescribing on patients with cirrhosis is unknown. METHODS: We emulated a hypothetical 3-year trial of benzodiazepine deprescription among Medicare enrollees with compensated cirrhosis who lacked other life-limiting diagnoses. All received continuous benzodiazepine prescriptions for the 6-months prior to their diagnosis of cirrhosis. During a 90-day landmark pe

Drug consumption of suspected drug-influenced drivers in Hungary (2016-2018).

The hazard caused by driving under the influence of drugs (DUID) is determined by the time of consumption, dose and biological effects of a substance, as well as by synergistic drug interactions after multi-drug use. The aim of this work was to investigate the prevalence and pattern of psychoactive substance use of suspected DUID drivers and to present the advantages and disadvantages of the system currently used for determination of impairment in Hungary. Blood and urine samples, collected between 2016 and 2018, were taken from 2369 drivers with a positivity rate of 95% for at least one subst

Quantitative analysis of drugs in hair by UHPLC high resolution mass spectrometry.

Liquid chromatographic methods coupled to high resolution mass spectrometry are increasingly used to identify compounds in various matrices including hair but there are few recommendations regarding the parameters and their criteria to identify a compound. In this study we present a method for the identification and quantification of a range of drugs and discuss the parameters used to identify a compound with high resolution mass spectrometry. Drugs were extracted from hair by incubation in a buffer:solvent mixture at 37°C during 18h. Analysis was performed on a chromatographic system comprise
