Treatment of basilar artery embolism with a mechanical extraction device: necessity of flow reversal.

BACKGROUND AND PURPOSE: The success of local fibrinolysis in vertebrobasilar thromboembolism depends on the volume and composition of the clot. Since thrombolysis can also be time consuming and cause bleeding, we investigated the feasibility of a mechanical clot retraction device based on a nitinol basket advanced through a microcatheter. METHODS: Five patients with acute embolism of the basilar artery who presented with progressive stroke and impaired consciousness were included in a multicenter study (Neuronet Evaluation in Embolic Stroke Disease [NEED]). In 3 patients flow reversal was ind

Acute limb ischemia and anticoagulation in patients with history of atrial fibrillation.

BACKGROUND: Atrial fibrillation (AF) is the commonest source of arterial embolization causing acute limb ischemia (ALI). The aim of this study was to assess the adherence of anticoagulation in AF patients and the treatment of underdiagnosed AF patients, presenting with ALI in our service and to evaluate the risk factors associated with morbidity and mortality. METHODS: A retrospective analysis of prospectively collected data was carried out from 115 limbs of 112 consecutive patients presenting with ALI of AF origin from 2008 until 2012 to a tertiary vascular service. RESULTS: The mean transf

Pulmonary Embolism: The Battle to Save Life in a Resource Poor Setting.

INRODUCTION: Pulmonary embolism (PE) is a cardiovascular emergency caused by occlusion of one or more pulmonary arteries by thrombi that originate from a deep venous thrombosis (subsequently called an embolus), typically in the large veins of the lower limb or pelvis. It is a common cause of preventable hospital death and a cause of mortality in obstetrics setting because it largely remains under diagnosed especially in a resource poor setting. CASE REPORT: Mrs OG, a 30-year-old primigravida, had a spontaneous delivery of a baby boy. A few hours after delivery, she had two episodes of syncope

Primary tumors of the thoracoabdominal aorta: surgical treatment of 5 patients and review of the literature.

The objectives of this study were to describe five cases involving primary tumors of the thoracoabdominal aorta and to review the pertinent literature. Between April 1990 and April 2000, we performed surgery on five patients with primary tumors of the aorta (PTA). There were three men and two women ranging in age from 37 to 65 years (mean, 49.8 years). The presenting manifestations were renovascular hypertension in four cases, including three associated with abdominal angina and lower extremity embolism in one case. In all patients aortograms identified atherosclerotic-like occlusive lesions i

[Massive kidney infarct by occlusion of the main artery].

Massive kidney infarct, due to total occlusion of the main artery, is not a frequent process in clinical urology. The most frequent causes are endocarditis, arteritis, atheromatosis and traumatisms. The complete blockage of the renal artery means that the tissue irrigated by the same is bloodless and prone to necrosis and it must be taken into account that although the renal parenchyma cannot withstand for more than 1 to 2 hours the lack of a blood supply, the obstructions or ischemias of shorter duration cause tissue disorders of greater or lesser importance, affecting more quickly and more i

Right ventricular vulnerability: a case study on anthracycline-induced cardiotoxicity post-pulmonary embolism recovery.

BACKGROUND: Right ventricular (RV) dysfunction may arise from several reasons, including pulmonary embolism (PE) or cancer therapy-related cardiotoxicity. PE elevates pulmonary pressures, straining the RV, which can cause dysfunction, while anthracyclines, though primarily recognized for impacting the left ventricle, can also impair the RV. The interplay between these conditions remains poorly understood, particularly the RV's susceptibility to anthracycline cardiotoxicity after recovering from PE-induced strain. This case study highlights a patient who initially had PE-related RV dysfunction;

Mural aortic thrombi: An important cause of peripheral embolization.

PURPOSE: Arterial thromboembolism in patients with an unknown source of embolization is still associated with significant morbidity and mortality. The advent of transesophageal echocardiography (TEE) and magnetic resonance imaging (MRI) and the more frequent use of computed tomography (CT) have led to the identification of mural aortic thrombi (MAT) as a source of distal embolization in a much higher proportion of patients than previously appreciated. The incidence, diagnosis, and treatment of patients with MAT is reported. METHODS: In a prospective study, from January 1996 to December 1998,

Stent placement in acute cerebral artery occlusion: use of a self-expandable intracranial stent for acute stroke treatment.

BACKGROUND AND PURPOSE: Stent placement has been applied in small case series as a rescue therapy in combination with different thrombolytic agents, percutaneous balloon angioplasty (PTA), and mechanical thromboembolectomy (MT) in acute stroke treatment. These studies report a considerable mortality and a high rate of intracranial hemorrhages when balloon-mounted stents were used. This study was performed to evaluate feasibility, efficacy, and safety of intracranial artery recanalization for acute ischemic stroke using a self-expandable stent. METHODS: All patients treated with an intracrania

Sex-specific disparities in clinical characteristics and outcomes of catheter-directed treatments for acute pulmonary embolism.

BACKGROUND: Acute pulmonary embolism (PE) represents a cardiovascular emergency with increasing incidence. Catheter-directed treatments (CDTs) are increasingly used for high-risk or intermediate-high-risk PE patients when surgical embolectomy or systemic thrombolysis (ST) is contraindicated or ineffective. Although prior research suggests poorer outcomes in women with PE, sex-specific differences in CDTs outcomes remain unclear. AIMS: To investigate the influence of sex on clinical characteristics, procedural complications, and clinical outcomes among patients undergoing CDTs for acute PE. M

Incidence and risk factors associated with index hospitalization leg ischemia following endovascular abdominal aortic aneurysm repair.

OBJECTIVE: Although the incidence of index hospitalization leg ischemia after abdominal aortic aneurysm repair is low (1%-3%), it is a serious complication that merits investigation. Preoperative risk factors, such as demographics, medical history, and medication use (or a lack of) that may be associated with this complication are understudied. Our aim was to predict the factors that may be associated with the development of leg ischemia after endovascular repair in the index hospitalization period. METHODS: All deidentified patient records from the Vascular Quality Initiative's abdominal aor

[Perioperative stroke effectively treated by an acute stroke team including anesthesia department: a case report].

Perioperative stroke is cerebral infarction occurring in the perioperative period. The incidence of perioperative stroke in non-cardiac, and non-neurologic surgery is about 0.7%, but the mortality can be as high as 26% to 40%. The outcome of the patients with perioperative stroke can be disastrous. Here we report a case of perioperative ischemic stroke that occurred after surgery of lumbar decompression and pedical screw fixation. A 76-year-old female admitted to our hospital because of lumbar spinal stenosis. Her medical history included hypertension and diabetes for ten years. Her personal h

Predictors and outcome of gastrointestinal complications after cardiac surgery.

AIM: Gastrointestinal (GI) complications following cardiac surgery are uncommon but may be life-threatening. The aim of this prospective study is to determine the incidence, independent risk factors, and outcomes following GI complications after cardiac surgery. METHODS: Between March 2006 and February 2011, postoperative GI complications were diagnosed in 35 of 7175 consecutive patients who underwent cardiac surgery. Preoperative, intraoperative, and postoperative predictors of complication and death were identified and compared with a control group. We also sought to compare the incidence o

The Impact of Simulation-Based Training in Cardiovascular Medicine: A Systematic Review.

Cardiovascular medicine and practice in recent times have evolved as complex procedures are performed to manage difficult cases. The majority of these interventions are done percutaneously in order to minimize patient risk. Additionally, training specialist in handling these interventions require a lot of exposure to them; as such, patients are at higher risk of errors and complications from trainees before attaining expertise. In order to avoid these possible risks to patients and ensure their safety, using simulation commonly in cardiovascular specialist education is a possible trend in the

Implications of in situ thrombosis and distal embolization during superficial femoral artery endoluminal intervention.

BACKGROUND: Endoluminal therapy for superficial femoral artery (SFA) occlusive disease is commonplace, but the incidence and outcomes of in situ thrombosis (IST) and distal embolization (DE) have not been well defined. The aim of this study was to examine the impact of IST and DE on long-term outcomes of SFA interventions. METHODS: A database of patients undergoing endovascular treatment of the SFA was queried. Patients who developed either IST treated by pharmacomechanical lytic therapy or DE were selected and compared to those without either event (control). Kaplan-Meier survival analyses w
