Unsupervised Machine Learning of the Combined Danish and Norwegian Knee Ligament Registers: Identification of 5 Distinct Patient Groups With Differing ACL Revision Rates.

BACKGROUND: Most clinical machine learning applications use a supervised learning approach using labeled variables. In contrast, unsupervised learning enables pattern detection without a prespecified outcome. PURPOSE/HYPOTHESIS: The purpose of this study was to apply unsupervised learning to the combined Danish and Norwegian knee ligament register (KLR) with the goal of detecting distinct subgroups. It was hypothesized that resulting groups would have differing rates of subsequent anterior cruciate ligament reconstruction (ACLR) revision. STUDY DESIGN: Cohort study; Level of evidence, 3. ME

Limitations in predicting outcome following primary ACL reconstruction with single-bundle hamstring autograft - A systematic review.

BACKGROUND: Anterior cruciate ligament reconstruction (ACLR) is one of the most well studied and performed procedures in the world. Thus, it would be useful to develop a tool to predict patient outcomes post-operatively to allow clinicians to optimise management. PURPOSE: To identify and summarise factors predictive of graft failure and patient-reported outcome (PROM) measures at minimum two years after ACLR with single-bundle quadruple hamstring autograft via the anteromedial portal technique. METHODS: Studies reporting IKDC, KOOS, Lysholm and graft failure in ACLR were systematically revie

Extracorporeal Shockwave Therapy in the Management of Sports Medicine Injuries.

Treatment of musculoskeletal conditions in athletes with extracorporeal shockwave therapy (ESWT) is gaining popularity as greater evidence supports its use. ESWT protocols (describing energy flux density, number of impulses, type of shockwave (focused or radial), number/frequency/duration of treatment session, area of application, and postprocedural therapy protocols) can be adjusted in the clinical setting. Protocols vary across studies, and optimal protocols for most indications are yet to be determined. ESWT can safely be used to treat various musculoskeletal conditions in athletes, includi

Dynamic function after anterior cruciate ligament reconstruction with autologous patellar tendon.

The purpose of this study was to dynamically assess the functional outcome of patients who had undergone successful anterior cruciate ligament reconstruction using an autologous patellar tendon technique and to determine whether their dynamic knee function was related to quadriceps and hamstring muscle strength. The knee kinematics and kinetics of 22 subjects who had undergone anterior cruciate ligament reconstruction (mean age, 27 +/- 11 years) and of 22 age- and sex-matched healthy control subjects were determined during various dynamic activities using a computerized motion analysis and for

Therapeutic options in the management of articular contractures in haemophiliacs.

Haemophilic contracture is seen most commonly as an equinus deformity of the ankle, or at the knee or elbow in the form of a flexion deformity. Treatment options are varied, and decision-making is based on the degree of the contracture, its chronicity, the presence of articular subluxation, the patient's ability to participate in treatment, and the available medical facilities. The treatments available fall into four categories: physiotherapy, orthotics, corrective devices, and surgical procedures. Treatment should be primarily by physiotherapy, splintage, and corrective devices. The late or s

Gait pattern in patients with spastic diplegic cerebral palsy who underwent staged operations.

Fifteen patients with spastic diplegic cerebral palsy (CP) were monitored for a mean length of 9.5 years after they underwent staged operations and were evaluated by gait analysis, including joint motion in the sagittal plane and the ground reaction force (GRF) in three dimensions. Results showed an increased hip flexion (132%) at midstance, a reduction of peak knee flexion (PKF) during swing (45%) accompanied by an augmented time of PKF during swing (50%), and an increased dorsiflexion of the ankle during swing (293%) as well as its time during the gait cycle, in comparison with normal values

Gait analysis and muscle strength in children with surgically treated clubfeet.

Twenty-three children who had unilateral surgery for idiopathic clubfeet underwent gait analysis and isokinetic muscle-strength testing at an average of 10 years after surgical release. Ankle sagittal-plane kinematics were disturbed in 20 clubfeet. Fifteen children had an internal foot-progression angle. Genu valgum and knee hyperextension were common. Plantarflexion power was decreased by 23% on the side of surgery (p = 0.00005). Quadriceps weakness (defined as a decrease in strength >10% of the nonoperated-on limb's strength) was seen in nine of 22 operated-on limbs and hamstring weakness in

Residual imaging abnormalities after acute hamstring strain injury: implications for return-to-sport decision-making.

Hamstring strain injuries remain a major cause of time loss and recurrent injury in sprint- and kicking-based sports. MRI and ultrasound are frequently used to confirm diagnosis, characterize anatomical severity, and support prognosis. At the return-to-play (RTP) stage, however, their value is less certain. Persistent imaging abnormalities may reflect incomplete healing, expected remodeling, or potentially relevant structural concerns. Available evidence, derived largely from a limited number of partly overlapping cohorts and predominantly nonoperatively managed non-complete tears, shows that

[Synovial healing in reconstructed cruciate ligaments. Our personal experience compared in single interventions and combined reconstructions].

INTRODUCTION: The healing process of tendon grafts used in cruciate ligament reconstruction is called ligamentization. The tendon structural architecture changes progressively into the histological appearance of normal ligament. Amiel and Clancy have demonstrated that this process is time-dependent in anterior cruciate ligament (ACL) reconstruction, the tendon graft taking one year to become similar to a normal cruciate ligament. Three different maturation phases of anterior cruciate graft ligamentization can be seen at MRI: periligamentous proliferation, intraligamentous proliferation and def

Allograft vs. autograft for chronic acromioclavicular joint instability: a systematic review and meta-analysis of outcomes and complications.

BACKGROUND: Chronic acromioclavicular joint (ACJ) instability is commonly treated with surgical reconstruction of the coracoclavicular ligament using tendon grafts. Both autografts and allografts are widely utilized, with no clear consensus on the superior option. Autografts, such as hamstring tendons, provide favorable outcomes but are associated with donor-site morbidity, whereas allografts offer an alternative without harvesting morbidity but may introduce complications such as osteolysis and immune-mediated tissue rejection. The choice between these grafts remains a topic of debate. This s

Graft Dimensions and Clinical Outcomes Following Anterior Cruciate Ligament Reconstruction Using Hamstring Versus Peroneus Longus Autografts in Graft-Commensurate, Short-Statured Patients: A Prospective Observational Study.

Background Anterior cruciate ligament (ACL) reconstruction with hamstring tendon autografts is well-established, but peroneus longus autografts have emerged as a promising alternative. Optimal graft diameter is a prerequisite for knee stability and satisfactory postoperative outcomes. Attainable autograft diameter is shown to directly correlate with patient height. The relative efficacy of hamstring (HS-R) versus peroneus longus (PL-R) reconstruction for ACL tears among graft-commensurate, short-statured patients is unknown. Methodology In this prospective observational study, 44 patients with

Adding a modified Lemaire procedure to ACLR in knees with severe rotational knee instability does not compromise isokinetic muscle recovery at the time of return-to-play.

PURPOSE: To determine whether isokinetic muscle recovery following ACLR using a hamstring tendon (HT) would be equivalent (non-inferior) in knees that had high-grade pivot-shift and adjuvant modified Lemaire procedure versus knees that had minimal pivot-shift and no adjuvant modified Lemaire procedure. METHODS: We evaluated 96 consecutive patients that underwent primary ACLR. Nine were excluded because of contralateral knee injury, and of the remaining 87, ACLR was performed stand-alone in 52 (Reference group), and with a Lemaire procedure in 35 (Lemaire group) who had high-grade pivot-shift,
