Panencephalopathic Creutzfeldt-Jakob disease in the Netherlands and the UK: clinical and pathological characteristics of nine patients.

AIMS: The panencephalopathic type of Creutzfeldt-Jakob disease (PECJD) has extensive abnormalities in cerebral white matter as well as the cortex. PECJD has rarely been described in Caucasians and debate continues on its classification and pathogenesis. We describe our experience of PECJD over a 14-year period of surveillance for CJD in the Netherlands and the UK. METHODS: Between 1993 and 2006, nine cases of PECJD were identified. Clinical, histological and biochemical characteristics of all patients were analysed and compared; all cases were classified clinically as sporadic CJD. RESULTS:

Catatonic schizophrenia and the use of memantine.

OBJECTIVE: To report a case of catatonic schizophrenia treated with memantine that resulted in a rapid reduction of catatonic symptoms. CASE SUMMARY: A 68-year-old male with catatonic schizophrenia presented with bizarre and catatonic behavior manifested by mutism, waxy flexibility, immobility, staring, nonresponsiveness to verbal commands, grimacing, rigidity, and posturing. During the course of his hospitalization, he was treated with memantine up to 10 mg/day. The catatonia responded rapidly and significantly to memantine. DISCUSSION: Few treatment options are available for the management

[Creutzfeldt-Jakob disease: alterations in an isolated cortical signal in diffusion magnetic resonance imaging].

INTRODUCTION: Neuroimaging is playing an increasingly important role in the diagnosis of Creutzfeldt-Jakob disease (CJD), as shown by the fact that it is giving rise to the exclusion of other processes and providing invaluable diagnostic data -above all alterations in the signal in basal ganglia and in the cerebral cortex. CASE REPORTS: Case 1: A 66-year-old male who had symptoms of rapidly progressing cognitive impairment that began in the occipital region, with ataxia and myoclonias. The electroencephalogram (EEG) recordings revealed the presence of periodic biphasic sharp waves, and determ

Biphasic illness pattern due to early relapse in Japanese-B virus encephalitis.

Japanese-B virus encephalitis (JE) is considered a uniphasic illness with a variable outcome. Biphasic illness patterns have never been reported previously. From an endemic zone in India we observed six patients of JE (from 62 patients treated in 7 years) who had an early relapse resulting in the biphasic clinical course. Five had poor socio-economic status and three had laboratory evidence of nutritional deficiency. Two patients were adults and the other four were children. Fever, rigors, headaches, body aches, altered consciousness, rigidity and tremors predominated the first phase of illnes

Clinical and differential diagnosis of Creutzfeldt-Jakob disease.

Until recently, the clinical diagnosis of CJD relied mainly on three criteria. These include patient history (rapidly progressive dementia), neurological findings (ataxia, pyramidal/extrapyramidal signs, myoclonus, akinetic mutism) and typical electroencephalographic (EEG) findings. These criteria are fulfilled in typical cases. The occurrence or increase of certain proteins in cerebrospinal fluid (CSF; 14-3-3, neuron-specific enolase) now provide important adjuncts in recognizing variant forms. Although these proteins can be detected in other neurological diseases accompanied with substantial

ECT in the treatment of the catatonic syndrome.

OBJECTIVE: To determine the efficacy of ECT in the treatment of the catatonic syndrome and to identify predictors of good response. METHOD: 28 cases of catatonia in 22 patients admitted to a psychiatry or medical psychiatry inpatient unit between January 1989 and June 1992 were retrospectively evaluated. Cases were included if they met criteria for catatonia as described by Kahlbaum, i.e., 4 or more signs including immobility, mutism, withdrawal, staring, rigidity, posturing/gimacing, negativism, waxy flexibility, echo phenomena, stereotypy, and verbigeration. Primary diagnoses were: Major De

[Creutzfeldt-Jakob disease transmitted by cadaveric dural graft: a case report].

We report a case of Creutzfeldt-Jakob disease in a 38-year-old man, transmitted by a cadaveric dural graft. In August 1985, he underwent cranial nerve decompression for hemifacial spasm and received a cadaveric dural graft for dural closure. He had been well until he began to complain of blurred vision and headache in May, 1990. He developed dementia, myoclonus and urinary incontinence over the subsequent 3 months. He was admitted to our hospital in August, 1990. On admission, he was somnolent and showed gait disturbance, myoclonus in extremities and elevated deep tendon reflexes symmetrically

[Releasing phenomenon of well learned praxis with right hand].

We report a case with "releasing phenomenon of well learned praxis" with right hand caused by cerebral infarction in the territory of left anterior cerebral artery. A 71-year-old right-handed woman suddenly developed motor paralysis in right lower and upper limbs associated with mutism. Motor paralysis of the right upper limb and speech disturbance improved gradually. At this period it was noted that she grasped and used an object in front of her with right hand against her own will. The neurological findings about one month after the onset of the disease revealed very mild weakness of the rig

[A case of toxic leucoencephalopathy induced by 5FU derivatives].

A case of toxic leucoencephalopathy induced by 5 FU derivatives is reported. A 46-year-old woman was diagnosed as having breast cancer, and radical mastectomy was performed on May, 1982. After operation, she was given irradiation and 5FU derivative (tegafur or carmofur) 600 mg and Nolvadex 20 mg (tamoxifen citrate) were administered every day. After taking the medication for a month, she began to stagger and developed a tremor in both arms. She was admitted to our hospital on August 16, because she showed evidence of dysarthria and memory disturbance in addition to her initial complaints. Soon

Late-Onset Depression Mimicking a Primary Psychiatric Disorder: Diagnostic Pitfalls in Sporadic Creutzfeldt-Jakob Disease.

Human prion diseases are rare neurodegenerative disorders that typically present with rapidly progressive neurological decline. Sporadic Creutzfeldt-Jakob disease (sCJD) is a fatal neurodegenerative disorder characterized by rapidly progressive encephalopathy. Although classically presenting with cognitive decline and myoclonus, early manifestations may be predominantly psychiatric, particularly in older adults, leading to diagnostic delay. We report the case of a 75-year-old woman with no prior psychiatric history who initially developed late-onset depressive symptoms accompanied by behaviora

Psychosis induced by invocation presenting as possession state: A case of Kitousei-Seishinbyo still emerging in modern Japan.

BACKGROUND: Kitousei-Seishinbyo, or psychosis induced by invocation, was first described by Morita in 1915 as a culture-bound syndrome characterized by possession-like states and personality transformation triggered by prayer or spiritual practices. New psychosocial triggers (e.g., self-improvement seminars, emerging religious groups) have been identified. However, reports of possession-related episodes have become extremely rare. Diagnostic confusion with dissociative identity disorder or other specified dissociative disorders persists. CASE PRESENTATION: A woman in her 50s, with no prior ps

Catatonia associated with epileptic seizures: A systematic review of case reports.

The association of catatonia with epileptic seizures is a rare phenomenon that is poorly understood and needs more clinical research. This systematic review included all published case reports of patients with catatonia meeting ICD-11 criteria associated with epileptic seizures, published until December 2021 in PubMed. Case reports were synthesized and results were expressed as percentages. In total, 42 articles with 52 case reports were included. Most patients were adults with a dispersed age (mean age 44.9 ± 19.3), slightly more males (59.6 %), with psychiatric history (76.9 %) of affective

Hyperleukocytosis-induced stroke and tonsillar herniation: Case report.

INTRODUCTION: and Importance: Acute ischemic stroke is a rare event in children with leukemia, yet with long-term morbidity, substantial health, and economic cost. Central nervous system (CNS) leukemic involvement and chemotherapy-related stroke are the most common causes. Hyperleukocytosis induced stroke is very rarely reported. CASE PRESENTATION: A 2-year-old male child presented with hyperleukocytosis (leukocyte count was 320x109/L). Bone marrow evaluation revealed T-cell acute lymphoblastic leukemia. He was treated with dexamethasone, vincristine and daunorubicin, and on day 4 of chemothe

Brain dysfunction in COVID-19 and CAR-T therapy: cytokine storm-associated encephalopathy.

OBJECTIVE: Many neurological manifestations are associated with COVID-19, including a distinct form of encephalopathy related to cytokine storm, the acute systemic inflammatory syndrome present in a subgroup of COVID-19 patients. Cytokine storm is also associated with immune effector cell-associated neurotoxicity syndrome (ICANS), a complication of chimeric antigen receptor T-cell (CAR-T) therapy, a highly effective treatment for refractory hematological malignancies. We investigated whether COVID-19-related encephalopathy, ICANS, and other encephalopathies associated with cytokine storm, shar

MM1-type sporadic Creutzfeldt-Jakob disease with unusually prolonged disease duration presenting with panencephalopathic-type pathology.

We report an autopsy case of MM1-type sporadic Creutzfeldt-Jakob disease (sCJD) with an unusually prolonged disease duration of 58 months. The initial symptom was progressive mental disorder followed by advanced cognitive impairment. Clinical progression was generally slow; myoclonus appeared at 17 months and periodic sharp-wave complexes on electroencephalogram at 21 months. A state of akinetic mutism occurred 29 months after the onset of symptoms. MRI showed gradually progressive cerebral atrophy. Neuropathologic examination showed widespread severe brain involvement. In the cerebral neocort

[Bilateral caudate head infarcts].

We reported a 67-year-old woman with bilateral caudate head infarcts. She developed sudden mutism followed by abulia. She was admitted to our hospital 2 months after ictus for further examination. She showed prominent abulia and was inactive, slow and apathetic. Spontaneous activity and speech, immediate response to queries, spontaneous word recall and attention and persistence to complex programs were disturbed. Apparent motor disturbance, gait disturbance, motor aphasia, apraxia and remote memory disturbance were not identified. She seemed to be depressed but not sad. Brain CT and MRI reveal

Unexplained Acute Homonymous Hemianopia as a Presentation of Creutzfeldt-Jakob Disease.

Creutzfeldt-Jakob disease (CJD) is the most common prion disorder that affects humans. The Heidenhain variant (HvCJD) of sporadic CJD (sCJD) is characterized by an array of often bizarre visual symptoms, which may precede progressive fatal neurodegeneration. Here, we describe a patient who presented with left homonymous hemianopia following a fall. Initial MRI and workup for vascular, autoimmune, infectious, paraneoplastic, or metabolic causes were unremarkable. Ophthalmological assessments suggested unexplained visual field loss with a possible functional overlay due to variability on examina

First Case of Creutzfeldt-Jakob Disease in Ghana: Diagnostic Challenge in a Resource-Limited Setting.

INTRODUCTION: Creutzfeldt-Jakob disease (CJD) is a rare, but invariably fatal neurodegenerative disorder with multiple subtypes, region-specific prevalence patterns, and significant underreporting, particularly in resource-limited settings due to limited diagnostic infrastructure and surveillance capacity. We report what is, to the best of our knowledge, the first documented case of CJD in Ghana. This case highlights the importance of recognizing atypical neurodegenerative presentations and the critical role of neuroimaging in settings where access to advanced diagnostic tools is limited. Midazolam in the treatment of catatonia: A case series.

PURPOSE: Catatonia is a condition estimated to affect 10% to 15% of patients admitted to psychiatric hospitals. It is characterized by motor and behavioral changes including staring, mutism, changes in muscle tone such as waxy flexibility, posturing, and changes in movement patterns. Benzodiazepines are considered a first-line treatment option for catatonia, and most people achieve symptom remission within days. Most literature supports use of lorazepam for treatment of catatonia, and there is limited data about use of other benzodiazepines such as midazolam. SUMMARY: Here we report a 6-perso

Identification and Treatment of Catatonia Presenting as Agitation and Self Injury in an Adolescent With Rett Syndrome.

Catatonia is a complex psychomotor syndrome associated with several psychiatric disorders, including schizophrenia and autism. It is also associated with neurologic conditions such as encephalitis and epilepsy. Catatonia has also been described in genetic syndromes such as Down Syndrome. Catatonia presents with two main subtypes. Retarded catatonia is characterized by stupor, immobility, mutism, rigidity, and withdrawal, as well as negativism, posturing, and echolalia/echopraxia. Excited catatonia is primarily characterized by psychomotor agitation and occasionally self-injurious behaviors. Th
