Update on systemic lupus erythematosus pregnancy.

Women with Systemic Lupus Erythematosus (SLE) still face significant risks when embarking on a pregnancy. Improvements in the field of pathophysiology, in diagnosis and a greater number of therapeutic options in the treatment of SLE, have made the medical community regard these patients with less trepidation. Despite these advances, however, the risk of significant morbidity to both the mother and the fetus still exists. THE INTERACTION OF LUPUS AND PREGNANCY IS VERY COMPLEX: the consensus is that pregnancy can worsen the lupus disease process, even if this is not predictable, and pregnancy ca

State of the art in conventional mechanical ventilation.

Despite a shift to noninvasive respiratory support, mechanical ventilation remains an essential tool in the care of critically ill neonates. The availability of a variety of technologically advanced devices with a host of available modes and confusing terminology presents a daunting challenge to the practicing neonatologist. Many of the available modes have not been adequately evaluated in newborn infants and there is paucity of information on the relative merits of those modes that have been studied. This review examines the special challenges of ventilating the extremely low birth weight inf

Diagnostic role of plasma BNP levels in neonates with signs of congenital heart disease.

BACKGROUND: Only a few studies have examined the relationship of plasma BNP levels and congenital heart disease (CHD) in neonates and these mainly concern preterm neonates with patent ductus arteriosus. We aimed to investigate the diagnostic role of plasma BNP in neonates admitted in the neonatal intensive care unit, (NICU), with signs of congenital heart disease (CHD). METHODS: Prospective assessment of plasma BNP levels in 75 consecutive neonates with suspected CHD (heart murmur, respiratory distress, or cyanosis), admitted in the NICU of our university hospital. The final diagnosis was don

Increased incidence of sepsis at birth in neutropenic infants of mothers with preeclampsia.

Neutropenia is often found at birth in infants born to mothers with preeclampsia, and is most likely present in utero. To determine whether this neutropenia is associated with an increased incidence of early-onset sepsis, we reviewed the hospital records of 301 low birth weight infants of mothers with preeclampsia. Early-onset sepsis was proved if the result of a culture of blood or cerebrospinal fluid in the first 48 hours of life was positive, or presumed if culture results were negative but two or more clinical signs of sepsis were present and the attending neonatologist believed that an in

Neonatal intestinal obstruction.

Our experience in the management of 138 infants with various causes of intestinal obstruction has provided us with important principles of diagnosis and treatments that we adhere to in the course of our practice. 1. The overall mortality of neonates suffering from intestinal obstruction should be fewer than 5 per cent. The high survival rate is accounted for by improved care these patients receive in specialized units. 2. Prematurity did not appear to play a significant factor in the outcome of these infants with intestinal obstruction. Our experience shows that premature infants tolerate oper

Disseminated fungal infections in very low-birth-weight infants: clinical manifestations and epidemiology.

In 1979 and 1980, an apparent increase in the occurrence of disseminated fungal infections was observed. The clinical features of such infections in very low-birth weight infants are poorly described, and diagnosis is often delayed. Over a 24-month period, a discrete group of ten clinically diagnosed and four autopsy-diagnosed cases of systemic fungal infections in very low-birth-weight infants was observed. Prior to developing systemic fungal illness, these infants required prolonged total parenteral nutrition, central arterial or venous catheters, and multiple courses of broad-spectrum antib

Morphological and phylogenetic analyses reveal new species of the Fusarium lateritium species complex and Fusarium torreyae species complex (Nectriaceae, Hypocreales) from Hainan Province, China.

Hereditary thrombotic thrombocytopenic purpura (hTTP) is a thrombotic microangiopathy due to severe ADAMTS13 deficiency. hTTP typically presents in adulthood with thrombocytopenia, hemolytic anemia, and/or neurologic abnormalities. However, nearly half of the patients with hTTP are symptomatic at birth, most commonly presenting as severe jaundice secondary to hemolysis. Turbulent blood flow during birth puts neonates at high risk for thrombotic events and subsequent hemolytic anemia. We present a case series of 3 siblings with ADAMTS13 loss-of-function variants, all of whom required neonatal e

An Account of Acute Myeloid Leukemia Complicating Pregnancy and Literature Review.

Background and Clinical Significance: The occurrence of acute myeloid leukemia (AML) in pregnancy represents a diagnostic and management challenge in the attempt to balance and achieve both maternal and fetal wellbeing. Pregnancy-specific manifestations mimic the initial symptoms of leukemia and may lead to a delay in diagnosis, especially during the first trimester of pregnancy. Decision-making strategies involve the patient and couples counseling with a multidisciplinary team of hematologists, obstetricians, neonatologists and psychologists. Maternal outcome depends on the disease subtype, p

Process Improvement for Family-Centered Congenital Heart Disease Deliveries.

BACKGROUND: Perinatal care coordination for the birth of infants with prenatally detected ductal-dependent congenital heart disease (DDCHD) has historically been limited by the siloing of cardiologists and neonatologists and by singular focus on risk reduction. The aim of this study was to increase time for parent-infant bonding and decrease overutilization of medical resources by developing and implementing a standardized delivery stratification system for infants with CHD. METHODS/INTERVENTIONS: Over the course of multiple Plan-Do-Study-Act cycles from 2015 to 2023, we identified risk facto

Outcomes of cases with complete hydatidiform mole coexisting with a fetus: a single-center study.

BACKGROUND: Complete hydatidiform moles coexisting with a fetus (CHMCF) are uncommon. Although CHMCF is associated with perinatal complications and post-molar gestational trophoblastic neoplasia (GTN), necessitating post-delivery chemotherapy, live birth remains feasible. This report presents 14 cases of CHMCF in Japan. METHODS: We reviewed medical records of patients with CHMCF treated at our hospital from 2000 to 2020 and summarized clinical data, including maternal age, pregnancy details, delivery outcomes, fertility treatments, serum human chorionic gonadotropin (hCG) levels, and ultrason

Fresh Milk Administration and Cytomegalovirus Infection in Preterm Neonates: A Case Study Approach.

INTRODUCTION: Fresh human milk for preterm infants is associated with a reduction of bronchopulmonary dysplasia, to improve survival without major complications, and with an increased breastfeeding duration. Nevertheless, its administration is frequently restricted due to the risk of post-natal cytomegalovirus infection. Looking at 12 years of medical records in a tertiary neonatal intensive care unit using fresh milk without restriction, we retrospectively collected cases of symptomatic postnatal cytomegalovirus infection in preterm infants born under 32 weeks receiving fresh milk. We present
