Rehabilitation of partially edentulous patients following fibular free-flap reconstruction presents significant functional and biomechanical challenges due to altered soft tissue morphology, reduced vestibular depth, and compromised load-bearing capacity. In cases involving extensive tissue loss, fixed prosthetic designs may be limited in their ability to support the tongue and cheeks, while implant-assisted removable partial dentures (IARPDs) offer better hygiene access and prosthetic flexibility. This case report describes a prosthetically driven approach that integrates CBCT-guided planning

BACKGROUND: Anterior repositioning splint (ARS) alleviates temporomandibular disorders (TMD) by sagittal correction of jaw via cusp-to-fossa relation, which can also correct transverse positional deficiency. Therefore, ARS may have therapeutic effects on mandibular deviation (MD) related TMD.
OBJECTIVES: To investigate the temporomandibular joint (TMJ) positional and morphological parameters and facial symmetry in patients with MD and evaluate the therapeutic effects of ARS treatment on these parameters in patients with TMD and functional MD.
METHODS: CBCT, MRI, and 3D facial images of patie

We report a case of generalized chronic periodontitis requiring periodontal treatment, including smoking cessation care and surgical procedures. With a chief complaint of swelling and pain in the gingiva around #47, a 41-year-old man visited the Tokyo Dental College Suidobashi Hospital. The patient had been smoking cigarettes for 31 years. The initial periodontal examination identified a probing depth (PD) of ≥4 mm in 72.6% of sites and bleeding on probing (BOP) in 45.2% of sites. Radiographic examination demonstrated bone resorption extending to the root apex in #37 and 47, angular bone resor

PURPOSE: Many artifacts and obstacles associated with cone-beam computed tomography (CBCT) scan can obscure or distort the details of the teeth and occlusal surface, like distorted teeth, streak artifacts, noise, and some malocclusion cases with excessive overlapping between jaws cause decrease the interocclusal space, which can impact diagnosis and treatment planning, and the 3D reconstruction accuracy. Optimizing dental precision by Integrating CBCT scans with other imaging modalities, supply more information to enhance CBCT accuracy, mainly in dental areas with limited clarity.

Sleep-related bruxism (SRB) is a motor oral behavior characterized by tooth grinding and jaw clenching activity, reported by 8%-12% of the adult general population and 3% of older individuals. The frequency of one of its biomarkers, rhythmic masticatory muscle activity (RMMA), remains elevated across ages. Obstructive sleep apnea (OSA) is associated with the brief and repetitive pause of breathing (apnea) and with transient reduction in oxygen (hypoxia). OSA is observed at all ages and in about 50% of older individuals with a male preponderance. SRB clinical assessment is based on self-reporti

BACKGROUND: Occlusal appliances are still widely used instruments in the management of orofacial pain in dentistry, yet alone or as a part of multimodal therapy. However, some of those modalities have been lacking thorough randomized assessment, and there is a conflicting evidence available. It is hypothesized that pain symptoms might improve faster and in more tangible way due to combined therapy. Also, to our best knowledge, nimesulide was never examined in this aspect, too.
OBJECTIVE: The aim of this study was to compare early effectiveness of routine intervention methods in patients with

The aim of this cohort study was to evaluate the stability after multi-segmentation of the maxilla for correction of anterior open bite deformities. A total of 33 patients who underwent segmented maxillary osteotomy between 1994 and 2006 were included in the study. Rigid fixation with plates and postsurgical intermaxillary fixation for 6 weeks was applied to each patient. All patients were then followed in a standardized examination procedure at months 6, 12, 18 and up to 30 months postoperatively. Vertical and horizontal relation of the incisors was measured both clinically and on cast models

OBJECTIVES: The effects of the Michigan splint on the change in disc displacement (DD) as well as the position of the condyles were determined by metrical analysis using magnetic resonance imaging (MRI).
METHODS: Twenty-five patients with clinical DD symptoms were treated by means of the Michigan splint, and an assessment of the effects of the splint was conducted or verified by MRI before and during the period of therapy, 5 months follow-up. The positions of the condyles and the disc were calculated from the MRI in the parasagittal plane.
RESULTS: There were no changes in the positions of t

Mandibular advancement splints (MASs) are used to advance the mandible forward in patients with sleep-disordered breathing. The conventional rigid MAS restricts the movement of the mandible, and this immobility sometimes produces discomfort, including temporomandibular disorders. A simple method for fabricating a mobile MAS was devised, using a connector made from a polyethylene toothed belt, with the intention of making the MAS more comfortable. The experimental connector is easily constructed, inexpensive, and small enough for use as an intraoral MAS. To evaluate durability, the axial and di

The purpose of this study was to investigate whether any physiologic or cephalometric parameters could be used to predict the efficacy of an adjustable mandibular advancement appliance for treating obstructive sleep apnea (OSA). Forty-two male and 5 female patients with OSA were recruited on the basis of baseline polysomnography with a documented Apnea and Hypopnea Index (AHI) greater than 15 per hour. Repeat polysomnography was performed with the appliance in place. Baseline cephalometry was performed for each patient, and follow-up cephalometry was completed for 19 of the subjects. The subje

The introduction of immediate loading was a paradigm shift in implant dentistry as it was previously believed that an unloaded period was essential for bone healing in order to promote osseointegration. However, this belief could not be confirmed by clinical studies or by human histology. Hitherto, numerous reports have been published on immediate loading in various indications. An important factor for success is primary implant stability. The latter can be improved by adapting drilling protocols to enhance lateral compression of the bone and by using tapered implant designs with apical thread

Finite element analysis of dynamic temporomandibular joint (TMJ) loading reveals a predominance of localization of loading laterally towards the collateral ligament regions and disc/capsule attachments to the mandibular condyle. A previous publication (Kirk, Kirk. OMS Clin North Am 2006;18:345-68) introduced biomechanical principles for surgeons to consider in the diagnostic phase of management as well as initial surgical procedure selection. The concept of impingements and their impact with development of derangement is presented in this paper with an expanded collection of imaging characteri

The objective of this study was to assess variations in the occurrence of temporomandibular disorders (TMDs) and the risk of developing pain and function impairment of the temporomandibular complex in obstructive sleep apnea syndrome (OSAS) patients treated with either an oral appliance (mandibular advancement device) or continuous positive airway pressure (CPAP) in a 2-year follow-up study. In addition, we assessed the relationship between the mean mandibular protrusion and the frequency of wearing the appliance during follow-up with the occurrence of pain and function impairment of the tempo
