Analysis of a Medically Certified, Wrist-Worn Sensor for the Assessment of Heart Rate and Energy Expenditure During Daily Activities in Patients With Chronic Heart Failure or Coronary Artery Disease and Recreational Athletes: Validation Study.

BACKGROUND: Exercise capacity and lifestyle have proven to be important prognostic factors for cardiovascular patients. Both can be ameliorated through different preventive interventions. Cardiac rehabilitation and remote patient monitoring have been proven to reduce cardiac events and cardiovascular mortality. One of the most important goals of cardiac rehabilitation and remote patient monitoring is improving physical fitness and monitoring of cardiovascular parameters, which could predict cardiac deterioration. In order to monitor cardiac patients successfully, reliable and nonobtrusive devi

Multicenter Evaluation of Machine-Learning Continuous Pulse Rate Algorithm on Wrist-Worn Device.

INTRODUCTION: Though wrist-worn photoplethysmography (PPG) sensors play an important role in long-term and continuous heart rhythm monitoring, signals measured at the wrist are contaminated by more intense motion artifacts compared to other body locations. Machine learning (ML)-based algorithms can improve long-term pulse rate (PR) tracking but are associated with more stringent regulatory requirements when intended for clinical use. This study aimed to evaluate the accuracy of a digital health technology using wrist-worn PPG sensors and an ML-based algorithm to measure PR continuously. METHO

Toward personalized sexual medicine (part 2): testosterone combined with a PDE5 inhibitor increases sexual satisfaction in women with HSDD and FSAD, and a low sensitive system for sexual cues.

INTRODUCTION: Low sexual desire in women may result from a relative insensitivity of the brain for sexual cues. Administration of sublingual 0.5 mg testosterone (T) increases the sensitivity of the brain to sexual cues. Sexual stimulation in the brain is necessary for phosphodiesterase type 5 inhibitor (PDE5i)-mediated increase in genital sexual response. Accordingly, a single dose of T+PDE5i might enhance sexual responsiveness, especially in women with low sensitivity for sexual cues. AIM: To assess the hypothesis that treatment with on-demand use of T+PDE5i improves sexual functioning, part

Results of valvuloplasty in patients presenting deep venous insufficiency and recurring ulceration.

The purpose of this retrospective study was to assess mid-term results of valvuloplasty in patients presenting chronic recurring venous stasis ulceration. From 1988 to 1993, valvuloplasty was performed in the superficial femoral vein of 33 lower extremities in 28 patients presenting recurring ulceration. In 23 cases, previous surgery in the superficial venous system or perforating vein had failed. Preoperative work-up demonstrated primary deep venous insufficiency (PDVI) in 22 extremities (group I), proximal PDVI in association with distal postthrombotic syndrome (PTS) in 10 (group II), and Kl

A Smart Wheeled Walker With Integrated Physiological Monitoring to Improve Mobility in Older Adults: Randomized Crossover Trial.

BACKGROUND: Mobility limitations in older adults are associated with an increased physiological cost of walking, reduced functional independence, and elevated fall risk. Conventional wheeled walkers improve stability but lack real-time physiological monitoring and safety feedback. Digital health-enabled assistive devices integrating physiological monitoring and fall detection may enhance mobility performance and safety. OBJECTIVE: This study aimed to evaluate the short-term effects of a smart wheeled walker equipped with real-time physiological monitoring and fall detection compared with a st

Intelligent fall risk prediction and real-time warning system for elderly care based on multimodal deep learning and wearable sensor fusion.

Falls remain a leading cause of injury-related morbidity and mortality among adults aged 65 and older, and these human and economic costs keep rising as populations age worldwide. Most existing risk assessment tools rely on periodic clinical evaluations or single-sensor wearable devices, neither of which tracks the dynamic, multifactorial nature of fall risk throughout daily life. In this paper we describe the design, implementation, and pilot-level field validation of an integrated system that pairs a multi-sensor wearable unit (triaxial accelerometer, gyroscope, insole pressure array, and wr

Investigation of non-saphenous veins in C0S patients.

BACKGROUND: C0S patients have symptoms of chronic venous disorders (CVDs), though an anatomical or pathophysiological explanation is lacking. C0 and C1 classified lower limbs can present with valve incompetence from the second to sixth generation of tributaries of saphenous veins despite the absence of a detectable saphenous truncal reflux. We hypothesized that C0S venous symptoms could stem from the second to sixth generation of saphenous tributaries and small veins that are not connected to the saphenous system. The aim was to explore these veins in C0S subjects and compare them with asympto

Adherence, Efficacy, and Safety of Wearable Technology-Assisted Combined Home-Based Exercise in Chinese Patients With Ankylosing Spondylitis: Randomized Pilot Controlled Clinical Trial.

BACKGROUND: Clinical practice guidelines recommend that exercise is essential in the self-management of ankylosing spondylitis (AS). Attending supervised interventions requiring periodic medical center visits can be difficult and patients may decline participation, whereas effective home-based exercise interventions that do not require regular medical center visits are likely to be more accessible for AS patients. OBJECTIVE: The goal of the research was to investigate the adherence, efficacy, and safety of a wearable technology-assisted combined home-based exercise program in AS. METHODS: Th

The biology of stress in cancer: Applying the biobehavioral framework to adolescent and young adult oncology research.

The stress response influences the development and trajectory of cancer through a host of complex neuroimmune mechanisms. Basic, translational, and clinical research has elucidated these biobehavioral connections and offers a new paradigm for scientific investigation and patient care. Using a biobehavioral approach could offer new diagnostic and therapeutic opportunities in oncology, and this approach will be particularly impactful for adolescent and young adult (AYA) patients with cancer. To date, nearly all biobehavioral oncology research has been done in the adult population. And yet, AYAs

Toe-brachial index and toe systolic blood pressure for the diagnosis of peripheral arterial disease.

BACKGROUND: Peripheral arterial disease (PAD) of the lower limbs is caused by atherosclerotic occlusive disease in which narrowing of arteries reduces blood flow to the lower limbs. PAD is common; it is estimated to affect 236 million individuals worldwide. Advanced age, smoking, hypertension, diabetes and concomitant cardiovascular disease are common factors associated with increased risk of PAD. Complications of PAD can include claudication pain, rest pain, wounds, gangrene, amputation and increased cardiovascular morbidity and mortality. It is therefore clinically important to use diagnosti

Brave new world: expanding home care in stem cell transplantation and advanced therapies with new technologies.

Hematopoietic stem cell transplantation and cell therapies like CAR-T are costly, complex therapeutic procedures. Outpatient models, including at-home transplantation, have been developed, resulting in similar survival results, reduced costs, and increased patient satisfaction. The complexity and safety of the process can be addressed with various emerging technologies (artificial intelligence, wearable sensors, point-of-care analytical devices, drones, virtual assistants) that allow continuous patient monitoring and improved decision-making processes. Patients, caregivers, and staff can also
