A review of open biopsy for mediastinal masses.

OBJECTIVE: To review the recent experience with biopsied mediastinal lesions in children and to assess the impact of recent advances in imaging and surgical techniques on diagnosis. METHODOLOGY: The clinical and radiological features of 55 patients who had mediastinal biopsies at The Royal Alexandra Hospital For Children (RAHC) over 15 years were reviewed. RESULTS: Fifty-five patients presented to RAHC between 1978 and 1993 with lesions of the mediastinum requiring biopsy of that site. Thirty-one of the 55 (56%) lesions were malignant. Neurogenic tumours were the most common (40%). In order

Congenital cystic adenomatoid malformation of the lung.

Twenty cases of cystic adenomatoid malformation of the lung were observed: 2 had died in utero; the diagnosis was made at birth in 13 infants of which 3 were premature. A Bochdalek's hernia had been diagnosed before birth in 2 cases by echo-tomography. The correct antenatal diagnosis had been made in 2 cases. Two infants had no symptoms, 3 were dyspneic, 8 were in respiratory distress and had to be intubated and ventilated. Two had a prune belly syndrome. Eight infants had a thoracotomy during their first week of life. Pulmonary resections concerning those 13 patients comprised 10 lower lobect

Rare localized extralobar sequestration with congenital cystic adenomatoid malformation: a case report.

Extralobar sequestrations constitute a rare form of congenital pulmonary airway malformations that are difficult to diagnose. Here, we report a rare case of a localized extralobar sequestration in the right superior portion of the mediastinum accompanied by congenital cystic adenomatoid malformation.A 19-year-old man presented with a right upper mediastinal mass that was detected using chest radiography, had a history of left spontaneous pneumothorax, and had undergone a bullectomy 4 years previously. The initial diagnosis included a mature teratoma and a bronchogenic cyst in the mediastinum; h

Video-assisted thoracoscopic surgical excision of cystic lung disease in children.

PURPOSE: Video-assisted thoracoscopic surgical (VATS) technique for resection of cystic lung disease (CLD) may offer some advantages when compared with thoracotomy in children. METHODS: From September 1999 to August 2004, 6 pediatric patients underwent VATS for CLD. Patients were chosen for VATS based upon surgeon's choice. Data are expressed as mean +/- SD. The Children's Healthcare of Atlanta institutional review board approved this study. RESULTS: The types of lesions included congenital cystic adenomatoid malformations (n = 1), extrapulmonary sequestrations (n = 3), congenital lobar emph

Multimodality Imaging Assessment of a Giant Middle Mediastinal Schwannoma.

Mediastinal schwannomas primarily originate from the intercostal and sympathetic nerves located in the posterior mediastinum. Tumors originating from the vagus nerve in the middle mediastinum are extremely rare, accounting for 2% of all intrathoracic schwannomas. Although schwannomas are typically asymptomatic and discovered by accident, they can produce symptoms when they compress adjacent structures, which may lead to severe complications and are hard to manage. These cardiovascular risk factors include superior vena cava syndrome, cardiac tamponade, heart failure, and pulmonary hypertension

Clinical spectrum and management of congenital pulmonary cystic lesions.

INTRODUCTION: Congenital cystic lesions of the lung in children are uncommon but potentially life-threatening and warrant an urgent diagnostic work-up. Pulmonary sequestration (PS), congenital cystic adenomatoid malformation (CCAM), congenital lobar emphysema (CLE), and bronchogenic cyst (BC) are the four major congenital cystic lesions, but they share similar embryologic and clinical characteristics. The purpose of this study is to review our institutional experience with congenital cystic lung disease emphasizing on diagnosis and management. PATIENTS AND METHODS: Between January 1975 and Oc

Video-assisted thoracic surgery for diseases within the mediastinum.

VATS and concepts of minimal access thoracic surgery have revitalized many aspects of general thoracic surgery, including the surgical approach to diseases and conditions of the mediastinum. Proven surgical options that have been shunned by patients and referring physicians because of the perceived morbidity of thoracotomy have been reconsidered with the emergence of these minimal access surgical options. Continued critical review of the accumulating experience in VATS techniques will refine the surgical indications for VATS and open thoracotomy.

Advantages of prenatal diagnosis and early surgery for congenital cystic disease of the lung.

Prenatal ultrasonography makes possible the diagnosis of congenital cystic lung disease in utero, enabling elective surgery for the lesions early in infancy before development of respiratory distress or potentially life-threatening infection. From 1986 to 1994, ten infants with congenital lung cyst underwent corrective surgery. Nine of them had a prenatal diagnosis of congenital lung cyst confirmed by ultrasound or computed tomography (CT) after birth. In 7 of these infants, congenital adenomatoid malformation (CAM) was found at surgery, and pulmonary sequestration was found and removed in two

Klippel-Feil syndrome associated with congenital cervical dislocation: report of an autopsy case.

Klippel-Feil syndrome is an uncommon congenital anomaly that is characterized by abnormal fusion of the cervical vertebrae and occasionally accompanied by various anomalies of other bones and internal organs. We report the autopsy case of a 5-year-old girl with this syndrome ssociated with congenital cervical dislocation, with special reference to the pathological findings of the vertebral column and spinal cord. Principal anomalies of the cranio-spinal axis were as follows: partial defect of the clivus, scoliosis, hypoplasia of the whole cervical vertebrae, anterior dislocation of C7 with S-s

[Endoscopic ultrasound of pathological mediastinal findings].

INTRODUCTION: Mediastinal diseases are mostly diagnosed by CT and MRI. The applicability of ultrasound is limited by the surrounding air- and bone-containing thorax, which permits only restricted echo windows. Transesophageal endoscopic ultrasonography circumvents this problem and ensures visualization of parts of the mediastinum. PATIENTS AND METHODS: We report our results in 38 patients with pathological mediastinal findings who were examined by endoscopic ultrasound between 1988 and 1993. The diagnoses were established by imaging and/or histological procedures. RESULTS: The following medi

Congenital Lung Malformations: A Comprehensive Overview of Current Knowledge-Narrative Review.

Congenital lung malformations (CLMs) encompass a range of developmental anomalies, including congenital pulmonary airway malformation (CPAM), bronchopulmonary sequestration (BPS), congenital lobar overinflation (CLO), bronchogenic cysts (BC), and bronchial atresia (BA). These lesions present with varying severity, from incidental findings to severe neonatal respiratory distress. Advances in prenatal imaging, such as ultrasound and fetal magnetic resonance image (MRI), have significantly improved early detection, aiding in better management planning. CLMs arise from abnormalities during specifi

Congenital bronchial atresia. A report of 4 cases and a review of the literature.

The clinical, radiographic, and pathologic findings in 82 patients with congenital bronchial atresia (CBA) have been reviewed, and we have discussed 4 additional cases. Most patients are asymptomatic and come to attention because of abnormal radiographic findings of a round or lobulated perihilar, solid, or cystic mass--the mucoid impaction sign. Typically, the region distal to the mass is hyperinflated. Recently, computed tomography has been shown to be diagnostic and its use obviates the need for other more complex imaging modalities or surgical exploration. Excisional surgery has been perfo
