The far-lateral approach (FLA) to the foramen magnum, lower clivus, and craniovertebral junction has become a cornerstone in accessing these regions. Historically, lesions arising at the ventral craniovertebral junction posed significant surgical challenges because of limited visualization, poor exposure or considerable brainstem retraction, and high complication rates with midline posterior and anterior transoral approaches. To overcome these limitations, early pioneers proposed the FLA, which extended the lateral suboccipital exposure and enabled controlled drilling of the occipital condyle,

BACKGROUND: Intraventricular schwannomas are exceptionally rare benign tumors, accounting for a minute fraction of intracranial schwannomas. Their occurrence within the lateral ventricle is particularly uncommon, and the clinical presentation is often nonspecific, posing significant diagnostic challenges. To date, only a limited number of cases have been reported worldwide. We present a rare case of a left lateral ventricular schwannoma manifesting predominantly as transient isolated memory impairment, an unusual presentation that broadens the recognized clinical spectrum of this entity.

INTRODUCTION: Cognitive impairment is a common consequence after stroke. Intermittent theta burst stimulation (iTBS) has emerged as a promising cognitive therapy. However, traditional iTBS typically employs lower doses and one-size-fits-all stimulation targets, which may not fully capitalise on the potential of this therapy and warrants further evaluation for both efficacy and safety. This study aims to evaluate the efficacy and safety of high-dose iTBS targeting the individualised frontoparietal cognitive network (FCN) identified by precision functional neuroimaging for post-stroke cognitive

OBJECTIVES: Persistent headaches with frequent debilitating headache exacerbation are some of the most common debilitating chronic pain conditions in either military or civilian populations with mild traumatic brain injury (MTBI). This high prevalence of MTBI-related headaches (MTBI-HA), also known as persistent headache attributed to mild traumatic injury to the head by the International Classification of Headache Disorders, is often associated with neuropsychologic dysfunction in mood, attention, and memory. Thus, MTBI-HA casts a profound, negative impact on patients' quality of life and inc

BACKGROUND AND OBJECTIVES: Upper cervical spine bony tumors pose a challenge to surgeons because of their extensiveness and proximity to vital structures. Reconstruction following resection is necessary to prevent instability. Our objective is to describe a technical note on the C1 anterior arch screw with mesh cage reconstruction for anchoring the upper end of the cervical plate after resection of the upper cervical tumor.
METHODS: A retrospective review of patients with primary bony tumors of the upper cervical spine after surgical resection between 2018 and 2022 was included in the study.

OBJECTIVE: Cavernous malformations (CMs) of the optic chiasm, optic tract, and hypothalamus are exceedingly rare. Most patients present with acute or progressive visual loss, and surgery aims to maximize visual recovery while minimizing neural manipulation. We describe an updated review of all reported operated optic-pathway cavernomas to clarify surgical strategies and visual outcomes.
METHODOLOGY: PubMed, MEDLINE, and Google Scholar were searched with the terms optic pathway cavernoma, optic chiasm cavernoma, hypothalamic cavernoma, and surgical outcomes. Eighty published cases (1979 - May

Intracranial metastasis disease (IMD) has proven to be a frequent secondary occurrence, usually for primary cancers such as lung, breast, and melanoma, which have a high possibility of metastasizing to the brain. Due to the reasons listed above, treatment and early diagnosis are incredibly challenging. In the past decade, medicine has developed much better imaging solutions and radiological and surgical approaches, increasing the postoperative survival prognosis and achieving more time-efficient results. It is still exceptionally difficult to be able to prevent what type of metastasis a patien

OBJECTIVE: Progressive myoclonic epilepsy type 1 (EPM1) is caused by biallelic alterations in the CSTB gene, most commonly dodecamer repeat expansions. Although transcranial magnetic stimulation (TMS)-induced long-interval intracortical inhibition (LICI) was previously reported to be normal in EPM1, short-interval intracortical inhibition (SICI) was reduced. We explored the association between these measures and the clinical and genetic features in a separate group of patients with EPM1.
METHODS: TMS combined with electromyography was performed under neuronavigation. LICI was induced with an

Cerebro-spinal fluid (CSF) leak in cranial and spinal surgery seriously threatens the clinical prognosis of surgical cases exposing the patients to higher risk of infection, prolonged hospital stay, and need of reoperation. Ultimately, this cascade of events may lead to life-threatening complications, as in cases of meningitis/encephalitis, spondylodiscitis, subdural hematomas, and severe pneumocephalus. Moreover, CSF leak causes a consistent rise in healthcare-related costs which are nearly doubled in CSF leak cases after craniotomy or in spinal surgery. The risk of CSF leak tends to be highe

BACKGROUND: Motor deficits are common after stroke and are a major contributor to stroke-related disability and the potential for long-lasting neurobiological consequences of stroke remains unresolved. There are only a few treatments available for the improvement of motor function in stroke patients. However, the mechanisms underlying stroke recovery remain poorly understood, and effective neurorehabilitation interventions remain insufficiently proven for widespread implementation.
METHODS: Herein, we propose to enhance the effects of brain plasticity using a powerful noninvasive technique fo

Objective. We present an easy-to-implement technique for accurate electrode placement over repeated transcranial electrical stimulation (tES) sessions across participants and time. tES is an emerging, non-invasive neuromodulation technique that delivers electrical stimulation using scalp electrodes.Approach.The tES electrode placement technique was developed during an exploratory clinical trial aimed at targeting a specific MNI-atlas cortical coordinate inN= 59 depressed participants (32 F, mean age: 31.1 ± 8.3 SD). Each participant completed 12 sessions of active or sham stimulation, administ

Cavernous malformations located within the brainstem present with a high rate of neurological symptoms and carry a more aggressive course in both pediatric and adult populations.1,2 Cavernomas within the medulla are the rarest form, representing only 5% of all brainstem lesions.3 Repeated hemorrhage of brainstem cavernomas is associated with significant and cumulative neurological deficits and thus requires treatment.4 Microsurgical resection has become the optimal mode of treatment with the aim of resecting the live malformation and not merely the multiaged, organized hematoma.4 This is best

BACKGROUND: External ventricular drain (EVD) placement is mandatory for several pathologies. The misplacement rate of the EVD varies widely in literature, ranging from 12.3 to 60%. The purpose of this simulation study is to provide preliminary data about the possibility of increasing the safety of one of the most common life-saving procedures in neurosurgery by testing a new device for EVD placement.
METHODS: We used a novel guide for positioning the ventricular catheter (patent RM2014A000376). The trajectory was assessed using 25 anonymized head CT scans. The data sets were used to conduct t

Background Transpedicular screws are a common adjunct for lumbar spine fusion. Accurate screw placement to prevent neurological injury has been the subject of many studies. The adoption of spine neuronavigation has shown a significant decrease in screw malposition morbidity. We aim to evaluate the accuracy of pedicle screw insertion using intraoperative CT-guided navigation in lumbar spondylosis. Methods We reviewed a prospective registry-based cohort study. This included patients who underwent transpedicular screws insertion for lumbar spondylosis under intraoperative CT-guided navigation (iC

BACKGROUND: Perinatal brain injuries often impact the corticospinal system, leading to motor impairment and cerebral palsy. Although transcranial magnetic stimulation (TMS) has been widely used to study corticospinal connectivity in adults and older children, similar studies of young infants are limited.
OBJECTIVES: The objective was to establish the safety and feasibility of advanced TMS assessments of the corticospinal connectivity of young infants with perinatal brain injury.
DESIGN: This was a pilot, cross-sectional study of 3- to 12-month-old (corrected age) infants with perinatal strok
