Tourette's syndrome: a pilot study for the discontinuance of a movement disorder.

For a century and a half, Tourette's syndrome (TS) has been a mystery to the medical profession with physicians believing that it was a psychological disorder, then a neurological brain disorder, and then, an infectious disease caused by streptococcus. What was never considered was that this disorder called Tourette's was due to a structural deformity which would manifest itself as a neurological problem. What has been discovered is that Tourette's syndrome is neither psychological, infectious, genetic or environmental in origin, but is what we have termed a structural-reflex disorder, and hav

Three-dimensional virtual operations can facilitate complicated surgical planning for the treatment of patients with jaw deformities associated with facial asymmetry: a case report.

This article describes a case we experienced in which good postsurgical facial profiles were obtained for a patient with jaw deformities associated with facial asymmetry, by implementing surgical planning with SimPlant OMS. Using this method, we conducted LF1 osteotomy, intraoral vertical ramus osteotomy (IVRO), sagittal split ramus osteotomy (SSRO), mandibular constriction and mandibular border genioplasty. Not only did we obtain a class I occlusal relationship, but the complicated surgery also improved the asymmetry of the frontal view, as well as of the profile view, of the patient. The vir

Implant replacement of congenitally missing lateral incisor: a case report.

The therapeutic goal of reconstruction dentistry is to provide our patients with a dentition that is in harmony with the patient's musculature and temporomandibular joints. Malocclusion can contribute to both muscular and joint dysfunction. This case report demonstrates the reconstruction of a malocclusion caused by a congenitally missing lateral incisor utilizing a combination of orthodontic treatment and a dental implant to replace the congenitally missing tooth.

Effect of occlusal appliances and clenching on the internally deranged TMJ space.

AIMS: Stabilization appliances and mandibular anterior repositioning appliances have been used to treat patients with internal derangement of the temporomandibular joint (TMJ) based on the assumption that these appliances work by decompressing the TMJ. The purpose of this study was to indirectly test this assumption. METHODS: Bilateral TMJ tomograms of 7 subjects with unilateral anterior disc displacement without reduction (ADDwor) were taken during comfortable closure and during maximum clenching in maximum intercuspation; tomograms were also taken with the 2 types of occlusal appliances in

Effects of splint therapy in TMJ dysfunction: a study using magnetic resonance imaging.

This pilot study was undertaken to correlate clinical and MRI diagnoses in seven symptomatic TMJ dysfunction patients and to account, if possible, for the clinical improvement in the signs and symptoms after the use of a maxillary stabilizing splint. The symptomatic TMJs were evaluated by means of MRI prior to splint insertion. Sagittal open/closed, and coronal closed images were obtained with a 0.3 T Fonar MR Scanner. A follow-up MRI was taken after three months of splint therapy for the purposes of a comparative study. All subjects responded positively even at the early phase of splint treat

The skeletal stability after maxillo-mandibular osteotomy with a "physiological positioning strategy".

The aim of this study was to estimate skeletal and dental stability after maxillomandibular osteotomy with physiological positioning. Ten patients (7 men and 3 women) with skeletal mandibular prognathism were treated by conventional Le Fort I osteotomy for the maxilla and unfixed short lingual osteotomy for the mandible together with physiological positioning. We used cephalometric analysis to evaluate the skeletal and dental stability preoperatively, immediately after maxillomandibular osteotomy, and more than 1 year later. The immediately postoperative measurements for the SNA and the SN-pal

A pilot study to evaluate posttreatment cephalometric changes in subjects with OSA.

STATEMENT OF PROBLEM: Obstructive sleep apnea (OSA) is a life-threatening condition that is diagnosed and evaluated primarily by polysomnography (PSG). The procedure is time consuming, expensive, and inconvenient for the patient, and may not be universally available. PURPOSE: The purpose of this pilot study was to evaluate posttreatment results on lateral cephalograms in patients with OSA. MATERIAL AND METHODS: Lateral cephalograms of 16 patients diagnosed with OSA were made at the beginning of treatment and 4-6 weeks following treatment. Treatment modalities used in the study were the mandi

Resilient appliance-therapy treatment outcome in patients with TMD pain correlated to MRI-determined changes in condyle position.

The aim of this research was to study if changes in condyle position in temporomandibular disorders (TMD) patients could be a factor that is affected by resilient appliance therapy and if it influences the treatment outcome. The study investigated 48 patients randomly assigned to a treatment group (T group = 21 patients, using resilient appliance) or a control group (C group = 27 patients, using nonoccluding appliance). Changes in the condyle-fossa relationship (with and without the appliance) were determined in an MRI examination. Ten weeks after treatment, the treatment outcome was measured.

Temporomandibular disorders and facial pain: a psychophysiological perspective.

This article presents a psychophysiological perspective on temporomandibular muscle and joint disorders (TMJD) and facial pain. After a brief introduction to TMJD, the article presents data, largely derived from work carried out in my laboratory, that address four questions: (1) What are the consequences of parafunctional activities? (2) Do TMJD patients engage in parafunctional activities? (3) Why are TMJD patients unaware of these activities? and (4) What are the implications of these findings for treatment? The findings suggest that low-level parafunctions increase pain in otherwise pain-fr

Salvage Reconstruction of a Traumatic Oronasal Fistula Following Complex Panfacial Trauma: Durable 2-Layer Mucoperiosteal Repair.

INTRODUCTION: Oronasal fistula is an uncommon but functionally significant complication following severe palatal fractures. While fistula repair is well described in cleft populations, traumatic fistulas after complex panfacial reconstruction are less frequently reported and may present unique reconstructive challenges due to prior fracture displacement and surgical manipulation. CASE REPORT: A 48-year-old male sustained extensive facial trauma after a tractor rollover, resulting in bilateral Le Fort III fractures with additional Le Fort I and II components, a displaced hard palate fracture w

Abdominal Acupuncture for Non-Responding TMD Patients: a Retrospective Observational Study in General Practice.

BACKGROUND: Pain related to Temporomandibular Disorders (TMD) is severe, negatively affecting patients' quality of life, and often resistant to conventional treatments. Abdominal Acupuncture (AA) is known to be particularly effective for pain, especially chronic and musculoskeletal pain, but it is still poorly studied and never investigated in TMD patients. OBJECTIVES: To analyze the efficacy of AA for the treatment of patients with subacute and chronic pain related to TMD and non-responding to previous conventional therapies (occlusal splint, medications, physical therapy). METHODS: Twenty-
