[Hypopituitarism after traumatic brain injury].

Hypopituitarism after moderate or severe traumatic brain injury (TBI) is usually underdiagnosed and therefore undertreated. Its course can be divided in an acute phase during the first 14 days after TBI with 50 to 80% risk of hypopituitarism, and a chronic phase, beginning three months after the event, with a prevalence of hypopituitarism that ranges from 2 to 70%. Its pathophysiology has been addressed in several studies, suggesting that a vascular injury to the pituitary tissue is the most important mechanism during the acute phase, and an autoimmune one during chronic stages. In the acute p

Home-cage monitoring as a sensitive tool for detecting subtle behavioral alterations following mild traumatic brain injury.

Home-cage monitoring (HCM) has emerged as a powerful approach for detecting subtle and context-dependent behavioral alterations in rodent models. In this mini-review, we integrate clinical observations and preclinical data from the closed-head weight-drop model, emphasizing studies from our laboratory, to argue that mTBI should be conceptualized as a disorder of behavioral regulation rather than of basic behavioral capacity. We first outline the limitations of short, novelty-driven gold-standard assays (e.g., Open Field, Elevated Plus Maze, Rotarod), which often indicate intact motor function

An approach for the analysis of axonal neuroinflammation by measuring dual biomarkers of oligodendrocytes and inflammatory cytokine in human plasma.

The myelin sheath surrounding axons is vulnerable to mechanical stresses after head injuries, as well as autoimmune attacks and degeneration in neurological disorders. Unfortunately, there is currently no effective method to assess these axonal conditions in individual patients. We have developed a sandwich immunoassay detecting dual signals of myelin oligodendrocyte glycoprotein (MOG) and interleukin 1B (IL1B) in human plasma ([IL1B on MOG]). While IL1B is one of common inflammation markers, its lack of tissue specificity is addressed by identifying IL1B on extracellular vesicles from oligode

Cerebral Blood Flow Predicts Recovery in Children with Persistent Post-Concussion Symptoms after Mild Traumatic Brain Injury.

Persistent post-concussion symptoms (PPCS) following pediatric mild traumatic brain injury (mTBI) are associated with differential changes in cerebral blood flow (CBF). Given its potential as a therapeutic target, we examined CBF changes during recovery in children with PPCS. We hypothesized that CBF would decrease and that such decreases would mirror clinical recovery. In a prospective cohort study, 61 children and adolescents (mean age 14 [standard deviation = 2.6] years; 41% male) with PPCS were imaged with three-dimensional (3D) pseudo-continuous arterial spin-labelled (pCASL) magnetic res

Confirmatory factor analysis of two computerized neuropsychological test batteries: Immediate post-concussion assessment and cognitive test (ImPACT) and C3 logix.

Introduction: Tests of memory and speed of cognitive and motor responses have been the primary foci in sports-related concussion assessment. This study sought to assess the construct validity of neuropsychological tests within C3 Logix. Method: Results of both baseline C3 Logix and the Immediate Post-Concussion Assessment and Cognitive Test (ImPACT) computerized tests from 86 Division I collegiate athletes were submitted to a two-factor confirmatory analysis using structural equation modeling. The two factors of Speed and Memory have been confirmed in previous studies of ImPACT. Results: Resul

[Formula: see text] Effects of screening for postconcussive syndrome (PCS) on PCS symptom self-report and neuropsychological test performance.

OBJECTIVE: Many non-neurological factors are related to postconcussive syndrome (PCS) symptom report and neuropsychological test performance in mild traumatic brain injury (mTBI). Use of mTBI screening instruments may also contribute to report of PCS symptoms and neuropsychological performance. We examined the impact of randomized PCS screening feedback on PCS symptom report and neuropsychological performance in 152 young adults with no history of mTBI. METHOD: Participants were 158 undergraduates attending a medium-sized Midwestern university who completed the Postconcussive Syndrome Questio

Prevalence of diagnosed depression in adolescents with history of concussion.

PURPOSE: Previous studies in adults have suggested concussion and other brain injury presents a risk factor for depression. The goal of our study was to analyze the association between previous concussion and current depression diagnosis in a large nationally representative adolescent data set. METHODS: Retrospective cohort study using the National Survey of Children's Health 2007-2008, a nationally representative survey conducted via random digit dialing. Data were obtained by parental report. We included youth 12-17 years old without a current concussion (N = 36,060), and evaluated the asso

The role of emotions reported in the emergency department in four-month chronic pain development: Effects of sadness and anger.

OBJECTIVE: This study aims to assess whether the emotions experienced during an urgent health problem represent risk factors for developing chronic pain. METHOD: A pain study was carried out as part of a randomized multicentre study on the prevention of post-concussion syndrome and post-traumatic stress syndrome (SOFTER) following emergency hospitalisation. Nine hundred and fourteen patients not suffering from chronic pain at admission provided information on the presence and intensity of eight emotions (anger, fear, regret, sadness, relief, contentment, joy, and interest) during their stay a

Hyperbaric oxygen for blast-related postconcussion syndrome: three-month outcomes.

OBJECTIVE: Mild traumatic brain injury (mTBI) and postconcussion syndrome (PCS) are common among military combatants. Hyperbaric oxygen (HBO2 ) is a proposed treatment for these conditions, but it has not been rigorously studied. The objective of this study was to determine the effects of HBO2 by 3 months post compression at 2 commonly employed dosing levels to treat PCS; whether specific subgroups may have benefited; and if no overall effect was found, whether benefit is masked by other conditions. METHODS: This randomized, double-blind, sham-controlled study was conducted at the Naval Air S

Deployment-related mild traumatic brain injury, mental health problems, and post-concussive symptoms in Canadian Armed Forces personnel.

BACKGROUND: Up to 20% of US military personnel deployed to Iraq or Afghanistan experience mild traumatic brain injury (mTBI) while deployed; up to one-third will experience persistent post-concussive symptoms (PCS). The objective of this study was to examine the epidemiology of deployment-related mTBI and its relationship to PCS and mental health problems (MHPs) in Canadian Armed Forces (CAF) personnel. METHODS: Participants were 16153 personnel who underwent post-deployment screening (median =136 days after return) following deployment in support of the mission in Afghanistan from 2009 - 201

Treatment of headache in the elderly.

Most primary headaches in the elderly are similar to those in younger patients (tension, migraine, and cluster), but there are some differences, such as late-life migraine accompaniments and hypnic headaches. Although migraine in younger persons usually presents with headache, migraine in older persons may initially appear with visual or sensory phenomena, instead of headache ("migraine accompaniments"). Hypnic headaches awaken patients from sleep, are short-lived, and occur only in the elderly. The probability of secondary headache increases steadily with age. Secondary headaches include thos
