Rapamycin as a therapy of choice after renal transplantation in a patient with tuberous sclerosis complex.

Herein we have reported the use of rapamycin in immunosuppressive treatment after renal transplantation as a therapy of choice in a patient with diagnosis of tuberous sclerosis complex (TSC). TSC is a genetic disorder, caused by mutations of TSC1 or TSC2 genes. Products of these genes, hamartin and tuberin, create a complex that inhibits mammalian target of rapamycin (mTOR), a key protein engaged in regulation of the cell cycle. Mutations of TSC genes lead to constitutive activation of mTOR resulting in uncontrolled proliferation, differentiation, and migration of cells. As a consequence malfo

New parameter of hemoglobin status as an indicator of efficacy of preoperative angioembolization in extracranial hypervascular tumours.

BACKGROUND: Routinely, the standard for measuring the success of preoperative embolization procedure as an adjunct in the management of head and neck vascular tumours has been to evaluate the amount of blood loss, duration of surgery, and intraoperative neurovascular injuries. OBJECTIVE: We hypothesized that the rate of change in the preoperative hemoglobin status would more accurately and objectively reflect the effectiveness of the embolization technique. MATERIALS AND METHODS: Twenty-six patients with extracranial vascular tumours were divided into two groups (A and B) of 13. Group A pati

Tuberous sclerosis-associated lesions of the kidney, brain, and skin are angiogenic neoplasms.

BACKGROUND: Tuberous sclerosis is an autosomal dominant condition characterized by the development of benign neoplasms of the brain, kidney, and skin. Progressive growth and malignant transformation of brain and kidney lesions constitute the major cause of morbidity and mortality in adults with tuberous sclerosis. In addition, growth of skin lesions may be disfiguring to patients. OBJECTIVE: The purpose of this study was to determine whether benign tumors in patients with tuberous sclerosis are angiogenic. METHODS: Brain, kidney, and skin tumors from patients with tuberous sclerosis were sta

Comparison of angiomyofibroblastoma and aggressive angiomyxoma in both sexes: four cases composed of bimodal CD34 and factor XIIIa positive dendritic cell subsets.

Aggressive angiomyxoma (AA) is a distinctive, locally aggressive, fibromyxoid tumor of the pelvic and genital soft tissues. AA is of unknown histogenesis but the cytologically bland spindled tumor cells, which surround characteristic variegated blood vessels, show fibroblastic or myofibroblastic features. AA may be related to angiomyofibroblastoma (AMF), another cytologically bland fibromyxoid genital spindle cell tumor with variable myoid differentiation that does not, as a rule, recur. Recently, CD34+ primitive fibroblasts and factor XIIIa+ dendritic histiocytes have been found in varying co

Intraoral Endoscopic Ligation of Maxillary Artery in the Infratemporal Fossa.

Ligation of the sphenopalatine and posterior nasal arteries is indicated for posterior epistaxis as initial treatment or when conservative measures fail. In some patients, a transnasal approach or its alternative transantral approach are not possible due to tumor filling the nasal corridor, pterygopalatine fossa, or maxillary sinus. Aim of this study was to evaluate feasibility of endoscopically assisted transoral approach for the ligation of the maxillary artery (MA). Six fresh cadaver specimens (12 sides), previously prepared with intravascular injections of colored latex, were dissected. A

Arachnoid cyst with extraordinary extracranial extension in the skull base as a result of an iatrogenic defect of the middle cranial fossa floor: case report and literature review.

Arachnoid cysts are benign intracranial lesions that are typically diagnosed incidentally. They are divided into two types: congenital and acquired. Acquired arachnoid cysts are rare and usually arise after trauma, infection, or haemorrhage. In this report, a rare case of an iatrogenic multiloculated arachnoid cyst as an unusual complication of a skull base defect is presented. It extended extracranially into the sphenoid sinus, the ethmoid, the infratemporal fossa, the nasopharynx and the nasal cavity, as well as into the pterygomaxillary and retromaxillary space, appearing with a distinct cl

2D and 3D visualisation of head and neck tumours from spiral-CT data.

PURPOSE: This paper intends to demonstrate the possibilities of two- and three-dimensional visualisation methods from spiral-CT data sets in the head and neck region and demonstrates their value based on various studies. MATERIAL AND METHODS: The scanner was a Somatom PLUS 4 (Siemens, Forchheim/Germany). The patients were scanned using a slice thickness of 3 mm and a table feed of 5 mm (pitch 1,6). The images were reconstructed with an increment of 1 mm. Contrast agent (150 ml) was applied intravenously with a flow of 2.5 ml/s and a start delay of 80 s. In one study the start delay was 20 s i

Sinonasal Neoplasia - Clinicopathological Profile And Importance of Computed Tomography.

BACKGROUND: Nasal cavity and Paranasal sinus malignancies are very rare, in which maxillary sinus is the commonest, followed by ethmoid, frontal and sphenoid sinus. Computed Tomography (CT) & Magnetic Resonance Imaging (MRI) play a key role in diagnosis, staging and management of paranasal sinuses and nasal pathologies. Multiplanar imaging in CT helps better imaging of critical anatomical areas. Aim of our study was to study the incidence, clinical features, CT features and its importance in the management of sinonasal neoplasms. MATERIALS AND METHODS: This prospective study was carried out i

Recent developments in vulvovaginal pathology.

This review discusses recent developments in vulvovaginal pathology. A variety of morphologically bland mesenchymal lesions occur at this site with considerable histological and immunohistochemical overlap. Aggressive angiomyxoma exhibits HMGA2 immunoreactivity in approximately 50% of cases, and this nuclear transcription factor is emerging as a useful and relatively specific marker for aggressive angiomyxoma, although occasional vulvovaginal smooth muscle neoplasms are positive. HMGA2 is useful in the diagnosis of aggressive angiomyxoma and its distinction from mimics, in the evaluation of re

Case Report: Co-occurrence of Wilson disease and tuberous sclerosis complex in a Chinese patient.

BACKGROUND: Wilson disease (WD) and tuberous sclerosis complex (TSC) are rare genetic disorders with distinct pathophysiologies. Their co-occurrence has not been previously reported. When atypical clinical features emerge in a patient with an established genetic diagnosis, comprehensive reassessment, including thorough physical examination and expanded genetic testing, becomes critical. CASE PRESENTATION: We describe a 28-year-old Chinese woman with a confirmed diagnosis of WD who developed focal seizures after substantial clinical improvement during copper chelation therapy. Her seizures wer

Multicenter phase 2 trial of sirolimus for tuberous sclerosis: kidney angiomyolipomas and other tumors regress and VEGF- D levels decrease.

BACKGROUND: Tuberous sclerosis (TSC) related tumors are characterized by constitutively activated mTOR signaling due to mutations in TSC1 or TSC2. METHODS: We completed a phase 2 multicenter trial to evaluate the efficacy and tolerability of the mTOR inhibitor, sirolimus, for the treatment of kidney angiomyolipomas. RESULTS: 36 adults with TSC or TSC/LAM were enrolled and started on daily sirolimus. The overall response rate was 44.4% (95% confidence intervals [CI] 28 to 61); 16/36 had a partial response. The remainder had stable disease (47.2%, 17/36), or were unevaluable (8.3%, 3/36). The

Possibilities and limitations of endoscopic management of nasal and paranasal sinus malignancies.

Over the last decade, the use of rigid endoscopes in surgery of tumorous lesions of the nose, paranasal sinuses, nasopharynx and anterior skull base has extended and diversified. Endoscopic surgical approaches for malignant lesions are very controversially discussed as of today, yet. From 1989-1999 we have treated 43 patients with invasive/destructive tumors of the paranasal sinuses and the anterior skull base strictly endoscopically, transnasally. These included 5 patients with juvenile angiofibromas and 36 patients with various malignant tumors, one with a large invasive macroadenoma of the

Use of mTOR inhibitors (rapalogs) for the treatment of skin changes in tuberous sclerosis complex.

BACKGROUND: Skin manifestations of Tuberous Sclerosis Complex (TSC) are present in more than 90% of patients. Facial angiofibromas (AF) are considered a skin hallmark of TSC. They are responsible for esthetic impact in patients. We aimed at reviewing the data available on the use of rapamycin (sirolimus) and everolimus for the oral or topical treatment of AF and other TSC-related skin changes and reporting our preliminary experience at Angers University Hospital. METHODS: The literature search has been performed in combining the terms "rapamycin", "sirolimus", "everolimus", "tuberous sclerosi

The gene for multiple endocrine neoplasia type 1: recent findings.

Multiple endocrine neoplasia type 1 (MENI) is a promising model to understand endocrine and other tumors. Its most common endocrine expressions are tumors of parathyroids, entero-pancreatic neuro-endocrine tissue, and anterior pituitary. Recently, collagenomas and multiple angiofibromas of the dermis also have been recognized as very common. MEN1 can be characterized from different perspectives: (a) as a hormone (parathyroid hormone, gastrin, prolactin, etc.) excess syndrome with excellent therapeutic options; (b) as a syndrome with sometimes lethal outcomes from malignancy of entero-pancreati

A modified midfacial degloving approach for the treatment of unilateral paranasal sinus tumours.

PURPOSE: The midfacial degloving approach (MFDA) is the primary option for the removal of benign and malignant sinonasal tumours. However, the classic MFDA does not compensate for the fact that most benign and malignant paranasal sinus (PNS) tumours are unilateral and the incisions may lead to some unnecessary complications?. Surgical exposure is limited to the upper and deep part of the PNS. Modifications of the classical MFDA that minimize complications and improve surgical field exposure are warranted. PATIENTS: The medical records of 27 consecutive patients who had undergone surgery using
