Sacral neuromodulation in the golden years: Treatment outcomes in elderly 75 years and older.

INTRODUCTION: Despite high prevalence and increased severity and burden of overactive bladder (OAB) and fecal incontinence (FI) in the elderly, sacral neuromodulation (SNM) is often overlooked as a potential treatment option for this demographic. In this study, we report the outcomes of SNM in patients aged 75 years or older at the time of surgery. METHODS: We conducted a retrospective cohort study of patients who underwent SNM implantation between 2013 and 2022 performed by a single, high-volume urologist at a tertiary center. Success, complication, and adjunct therapy rates were analyzed by

Short-term and long-term results of cardiac surgery in elderly and very elderly patients.

OBJECTIVE: Cardiac operations in elderly patients are increasingly frequent and imply major clinical, ethical, and economic issues. Operative and 5-year results of cardiac operations in patients aged 79 years or more are known in limited series, and a debate is ongoing on the appropriateness of selection of patients for surgery. METHODS: We retrospectively reviewed our experience in 6802 patients aged 79 years or more who had received a cardiac operation. Surgical candidates were selected according to functional status, crude operative risk, and social context and were managed according to a

Minimally invasive technology for mitral valve surgery via left thoracotomy: experience with forty cases.

BACKGROUND: Recent evolution of minimally invasive technology has expanded the application of the right thoracotomy approach for mitral valve surgery. These same technological advances have also made the left posterior minithoracotomy approach attractive in complex mitral procedures. METHODS: From 1996 to 2003, 921 isolated mitral valve procedures were performed without sternotomy; 40 (4.3%) of these were performed via left posterior minithoracotomy. In the left posterior minithoracotomy group, ages ranged from 18 to 84 years; 36 patients had had previous cardiac surgery (9 on > or =2 occasio

The influence of severe bone loss on mitral annular calcification in postmenopausal osteoporosis of elderly Japanese women.

We assessed the influence of aging bone calcium metabolism on mitral annular calcification (MAC) and aortic valve calcification (AVC) in 239 septua- and octogenarians (62 men, 177 women; 80.2 +/- 4.4 years). Osteoporosis was diagnosed by vertebral bone fracture. Both MAC and AVC were derived by 2-dimensional echocardiography. Bone mineral content (BMC) of the lumbar vertebral body was obtained by single-energy quantitative computed tomography using a calibration phantom. Serum calcium, phosphorus, parathyroid hormone, calcitonin, and osteocalcin were examined. Patients were classified into 3 a

Punch skin grafting for patients with no-option for revascularisation chronic limb-threatening ischaemia: A case series.

OBJECTIVE: No-option for revascularisation Chronic Limb-Threatening Ischaemia (CLTI) is often a terminal diagnosis for older patients, leading to survival and limb preservation rates that are of concern, as it frequently results in amputation and increased mortality. This study aimed to describe the clinical outcomes of wounds in patients with no-option CLTI treated with punch skin grafting (PSG), a partial-thickness skin transplantation procedure. METHOD: This clinical case series included patients with no-option for revascularisation CLTI ulcers from an intermediate-hospital Wound Care Unit

Considerations with Respect to Elective Abdominal Aortic Aneurysm Repair in Older People.

BACKGROUND: The epidemiology of abdominal aortic aneurysm (AAA) is changing, with progressively more older patients receiving repair. Advancing age associates with an increased procedural risk but also with a decreasing lifetime rupture risk. This national study aims to provide background information for decision-making in older people. METHODS: National administrative data (Statistics Netherlands) were used to estimate time-varying trends in the age and sex distribution of patients who underwent AAA repair, the age-procedural-risk relationships, and survival-time following successful AAA rep

The evolution and interpretation of seroprevalence of SARS-CoV-2 antibodies among South African blood donors from the Beta to Omicron variant-driven waves.

BACKGROUND AND OBJECTIVES: Confirmed COVID-19 diagnoses underestimate the total number of infections. Blood donors can provide representative seroprevalence estimates, which can be leveraged into reasonable estimates of total infection counts and infection fatality rate (IFR). MATERIALS AND METHODS: Blood donors who donated after each of three epidemic waves (Beta, Delta and first Omicron waves) were tested for anti-SARS-CoV-2 nucleocapsid antibodies using the Roche Elecsys anti-SARS-CoV-2 total immunoglobulin assay. Roche Elecsys anti-spike antibody testing was done for the post-Omicron samp

A new viewpoint on endoscopic CABG: technique description and clinical experience.

BACKGROUND: The aim of this paper is to describe a newly developed endoscopic coronary artery bypass graft (Endo-CABG) technique to treat patients with single- and multi-vessel disease and discuss the short-term clinical results in a large patient cohort. This technique avoids a median sternotomy by combining a thoracoscopic technique via three ∼5 mm thoracic ports and a mini-thoracotomy utility 3-4 cm port through the intercostal space. METHODS: From January 2016 to January 2018, data from consecutive patients undergoing an elective Endo-CABG were prospectively entered into a customized data

Risk Factors for Complications after Peripheral Vascular Surgery in 3,202 Patient Procedures.

BACKGROUND: Complications after open vascular surgery are a major health challenge for the healthcare system and the patients. Infrainguinal vascular surgery is often perceived as less risky than aortic surgery and the aim of this study was to identify which risk factors correlated with postoperative complications after open vascular surgery for infrainguinal occlusive disease in an 8-year cohort using the Danish National Vascular Registry (Karbase), which gathers information on all vascular procedures in Denmark. METHODS: This study is a retrospective cohort study. The Karbase was searched f
