Long term follow-up of semi-constrained total elbow arthroplasty: a single centre retrospective analysis.

Total elbow arthroplasty (TEA) is an effective treatment for end-stage elbow arthritis and fractures, with semi- constrained designs demonstrating superior outcomes in stability and implant longevity. However, long-term survival and clinical performance data remain limited. This single-center retrospective study analyzed 122 patients who underwent semi-constrained TEA between 2003 and 2019. The cohort included 94 Coonrad-Morrey, 17 Nexel, and 11 Latitude prostheses. Clinical outcomes were assessed using Visual Analogue Scale (VAS) for pain and satisfaction, Disabilities of the Arm, Shoulder an

Musculoskeletal modeling-based definition of load cases and worst-case fracture orientation for the design of clavicle fixation plates.

Mechanical performance of clavicle fracture fixation plates is often evaluated using finite element (FE) analysis. Typically, these studies use simplified loading conditions and assume a transversal fracture orientation. However, the loading conditions and fracture orientation influence how the fracture site and thus fixation plate is loaded. In this study, a musculoskeletal model that included the clavicle muscles and scapulohumeral rhythm was defined based on previously published models. The standard OpenSim workflow (inverse kinematics, inverse dynamics, static optimization, and joint react

Chondroblastoma-like primary malignant giant cell tumor of the humerus - a case report.

35-year-old woman suffered prolonged pain in the left shoulder, where an aggressively growing tumor of the proximal humerus was revealed thereafter. The lesion caused massive osteolysis of the metaepiphysis with cortical disruption, but no soft tissue extension was evident. Given the unsatisfactory effect, the ongoing neoadjuvant chemotherapy was prematurely ceased and the resection 13 cm long segment of bone with modular prosthesis replacement followed. Histologically, clear-cut malignant tumor with both the presence of numerous reactive osteoclast-like giant cells and geographic structural d

Metallic Press-fit Radial Head Replacement for Radial Head Fractures.

This study evaluated the clinical and radiographic results of patients who sustained nonreconstructible radial head fractures that subsequently were treated with a metallic press-fit radial head prosthesis (RHP). Nine patients with comminuted radial head fractures were treated with a metallic press-fit RHP; mean follow-up was 38.7 months. The radial head fracture was acute in 6 patients (group A) and chronic in 3 patients (group B). Functional evaluation was assessed using the Mayo Elbow Performance Score (MEPS) and Disabilities of the Arm, Shoulder and Hand (DASH) Questionnaire. Radiographs w

Partial allograft replacement of the radial head in the management of complex fracture-dislocations of the elbow.

BACKGROUND: There have been reports of total radial head allografts showing variable outcomes. This case series reports the novel use of an allograft for partial radial head replacement in the treatment of elbow fracture-dislocation. METHODS: Eight patients underwent partial radial head allograft reconstruction to restore stability for complex fracture-dislocations involving fractures of the coronoid and radial head, with collateral ligament disruption. Two patients were treated acutely. Six were referred from other centers 2 to 48 weeks after injury following failure of primary treatment. In

Combined latissimus dorsi myocutaneous flap and 3D-printed PEEK implant for reconstruction of a large full-thickness chest wall defect: A retrospective study.

BACKGROUND: Reconstruction of extensive full-thickness chest wall defects is a noticed challenge within the field. Although 3D-printed polyether ether ketone (PEEK) has exhibited promising outcomes in cranial and maxillary reconstruction, its application combined with flap in reconstruction of a full-thickness chest wall defects remains unexplored. METHODS: Over the span from November 2018 to March 2024, a retrospective study involving 12 patients who underwent chest wall reconstruction using latissimus dorsi myocutaneous flap (LDMF) combined with 3D-printed PEEK prostheses after tumor resect

Mid-term clinical outcome comparison of long-stemmed monopolar osseointegrated and short-stemmed bipolar radial head prostheses.

INTRODUCTION: Comparative data to guide implant choice for radial head replacements are lacking. Here, we compared the clinical results achieved using two different types of radial head prostheses. METHODS: Data from patients with comminuted radial head fractures (n = 66), who underwent radial head arthroplasty with either short-stemmed bipolar (n = 31, Group 1: rHead Small Bone Innovations/USA) or monopolar long-stemmed osseointegrated rigidly fixed (n = 35, Group 2: MoPyC Tornier/France) prostheses, were retrospectively reviewed. Pyogenic arthritis in adults.

Septic arthritis has shown no change in incidence, and despite advances in antimicrobial therapy is often responsible for residual functional impairment and for a high mortality rate among debilitated patients. Risk factors include older age, diabetes mellitus, rheumatoid arthritis, immunodeficiency, and a preexisting joint disease (e.g., rheumatoid arthritis) to which the symptoms of septic arthritis are sometimes ascribed. Staphylococcus aureus contributes over two-thirds of identified organisms; a range of streptococci and gram-negative bacilli are next in frequency. The most common site is

What Factors are Predictive of Patient-reported Outcomes? A Prospective Study of 337 Shoulder Arthroplasties.

BACKGROUND: Although shoulder arthroplasties generally are effective in improving patients' comfort and function, the results are variable for reasons that are not well understood. QUESTIONS/PURPOSES: We posed two questions: (1) What factors are associated with better 2-year outcomes after shoulder arthroplasty? (2) What are the sensitivities, specificities, and positive and negative predictive values of a multivariate predictive model for better outcome? METHODS: Three hundred thirty-nine patients having a shoulder arthroplasty (hemiarthroplasty, arthroplasty for cuff tear arthropathy, ream

Instability of the shoulder after arthroplasty.

A replacement arthroplasty was performed in 236 shoulders at The Hospital for Special Surgery from 1984 through 1989. Ten patients (eight women and two men) from that group were identified as having instability of the shoulder at the time of follow-up, and the results for these patients were reviewed retrospectively. The ages of the patients ranged from fifty-six to seventy-nine years. The instability was anterior in seven of the patients and posterior in three. The anterior instability was caused by a rupture of the repaired subscapularis tendon. The operative treatment of the anterior instab

Impact of machined versus structured implant shoulder designs on crestal bone level changes: a randomized, controlled, multicenter study.

PURPOSE: The collar region of an implant is its connection to the oral cavity. A balance between osseointegration on one hand and the absence of plaque accumulation on the other hand is necessary for successful implantation. It is yet to be determined which implant collar design, polished or rough, is best to stabilize the crestal bone level, avoiding peri-implantitis and subsequent risk of implant loss. The aim of this study was to investigate the influence of the architecture of the collar region on marginal bone and soft tissue response. METHODS: This prospective, randomized, clinically co

Retrograde intramedullary multiple pinning through the deltoid "V" for valgus-impacted four-part fractures of the proximal humerus.

BACKGROUND: The "palm tree-like wiring" introduced by Kapandji and its modifications for proximal humeral fractures should be given the generic name "retrograde intramedullary multiple pinning through the deltoid 'V'" (IMPV). IMPV is still preferable for treating valgus-impacted four-part fractures in which K-wires have the advantage of working as an internal fixation material as well as a tool in fracture reduction. METHODS: Three 2.4-mm K-wires formed into the desired shape are used as the intramedullary pins and a 3.0- to 3.2-mm upwardly angled hole for each wire is opened in the deltoid "

Gleno-Humeral arthritis in young patients: clinical and radiographic analysis of humerus resurfacing prosthesis and meniscus interposition.

Recently, interest about glenoid resurfacing with lateral meniscus in osteoarthritis of the shoulder has been increasing. Aim of this study is to evaluate the results of this procedure, from a clinical and a radiographic point of view. Constant score, DASH (Disability of the Arm, Shoulder, and Elbow) questionnaire, some specific questions about satisfaction after surgery, and radiographic exams were performed pre-operatively and post-operatively, at a minimum follow-up of 2 years. Constant score improved from 49.8 to 66.2. 84.4% of the patients were satisfied. Mean Dash was about 24.2 points.

Total joint replacement in HIV positive patients.

BACKGROUND: Recent HIV therapies have improved life expectancy in HIV positive patients. For the purpose of the following retrospective investigation, we analyzed the results of total joint replacement in HIV positive patients. This study exemplifies orthopaedic treatment options and perioperative problems in HIV positive patients. Our population included a high proportion of hemophilic patients. DESIGN AND METHODS: Between 1988 and 2000, we performed 55 endoprosthetic procedures (20 total hip replacements (THR), 33 total knee replacements (TKR), two shoulder replacements) in 41 patients suff

Failure to awaken after general anesthesia due to cerebrovascular events.

Failure to awaken after general anesthesia is a rare complication that may be caused by residual drug effect, systemic abnormalities, or neurologic injury. Failure to awaken often is initially attributed to residual drug effect, potentially delaying correct diagnosis. Three cases of failure to awaken because of neurologic injury are reviewed. Differential diagnosis of failure to awaken is discussed, and the importance of distinguishing pathologic causes from residual anesthetic is stressed.

Surgical management of advanced hemophilic arthropathy. An overview of 20 years' experience.

Advanced hemophilic arthropathy often causes moderate to severe pain and significant incapacity by the third or fourth decade of life. Most of these patients can be managed conservatively with clotting factor replacement on demand, therapeutic exercises, antiinflammatory drugs, orthoses, and activity restriction. Some patients, however, have such severe pain and disability and frequent need for clotting factor replacement that reconstructive surgery is required. Over the last 20 years at the present authors' institution, 168 such procedures have been performed. Of these, 141 were primary proce

Prior Nonshoulder Periprosthetic Joint Infection Increases the Risk of Surgical Site Infection, Sepsis, and All-Cause Revision After Primary Total Shoulder Arthroplasty.

INTRODUCTION: Periprosthetic joint infection (PJI) after total joint arthroplasty is a known risk factor for infection in subsequent joint arthroplasty. The purpose of this study was to determine whether prior nonshoulder PJI contributes to the increased risk of infectious complications, greater healthcare utilization, and increased revision surgery after primary total shoulder arthroplasty (TSA). METHODS: Patients who underwent primary TSA for osteoarthritis with prior nonshoulder PJI were identified in a national database (PearlDiver Technologies) using Current Procedural Terminology and In

Detritic synovitis can mimic a Propionibacterium periprosthetic infection.

PURPOSE: The purpose of this article is to illustrate that the clinical findings of detritic synovitis complicating a total shoulder arthroplasty can strongly resemble those of a 'stealth' periprosthetic shoulder infection with a low-virulence organism such as Propionibacterium. METHODS: We present a review of the literature and illustrate an example of detritic synovitis following a total shoulder arthroplasty. RESULTS: The combination of glenoid component loosening and humeral osteolysis after total shoulder arthroplasty are commonly attributed to periprosthetic infection with low virulenc

Reconstruction of humeral length and centering of the prosthetic head in hemiarthroplasty for proximal humeral fractures.

Anatomic reconstruction of humeral length in hemiarthroplasty for complex proximal humeral fractures is difficult because reliable surgical landmarks are missing or are destroyed by the fracture. The pectoralis major tendon is a reliable landmark to determine prosthetic height intraoperatively. This study analyzed the clinical outcome, reconstruction of humeral length, centering of the prosthetic head in the glenoid, and tuberosity positioning and healing, using the pectoralis major tendon as a reference intraoperatively. The study included 30 patients. In 21 patients (group 1), humeral length
