Ligamentoplasty using the ulnotriquetral ligament with wafer procedure for chronic triangular fibrocartilage complex foveal tear: Short-tem outcome.

BACKGROUND: We developed a new ligamentoplasty technique using the ulnotriquetral ligament (UTL) via a palmar approach for cases with dorsal instability of the ulna due to a foveal tear of the triangular fibrocartilage complex (TFCC). The study aimed to evaluate the clinical outcomes of this procedure. METHODS: We retrospectively analyzed 19 cases with foveal tears of TFCC who underwent the ligamentoplasty using UTL. We harvested the UTL from the triquetrum as a pedicle graft, avoiding damage of the connection between the base of the UTL and palmar radioulnar ligament (PRUL) as possible. The

Rotator Cuff Repair With Bone Grafting of Greater Tuberosity Cyst: A Technique Video.

BACKGROUND: Nearly 500,000 rotator cuff repairs are performed annually in the United States. Cysts within the humeral head have been reported to occur in more than half of patients diagnosed with a rotator cuff tear. They are related to age-related degeneration and rotator cuff dysfunction, and may arise from congenital abnormalities. INDICATIONS: Humeral head cysts may pose technical challenges during rotator cuff repair. Cysts located at the footprint of a planned rotator cuff repair can decrease biological healing capacity and reduce the fixation strength of suture anchors. One treatment s

Modern Treatment of Recurrent Patellofemoral Instability - Combined Medial Patellofemoral Ligament Reconstruction and Tibial Tubercle Transfer.

PURPOSE OF THE STUDY The treatment of recurrent patellofemoral instability has undergone a paradigm change during the last 15 years. Instead of non-anatomical operations, the current concept favors the reconstruction of injured structures and the normalization of the biomechanical environment. Our aim was to briefly summarize the etiology, diagnostic regimen, and therapeutic decision making of this varied patient group and to review our own patients who underwent combined medial patellofemoral ligament reconstrucion and tibial tubercle transfer. MATERIAL AND METHODS Between 2015-2017 we perfor

Beyond the posterior tibial slope: Rethinking anterior cruciate ligament (ACL) re-rupture risk through integrated scoring.

Anterior cruciate ligament (ACL) graft failure remains a relevant challenge, particularly in revision settings where anatomical and clinical risk factors converge to an increased retear risk. Posterior tibial slope (PTS) is a well-established biomechanical contributor. Each degree of increased PTS leads to greater anterior tibial translation (ATT) and higher ACL graft strain. Nevertheless, current decision-making frameworks often consider PTS in isolation and may underappreciate important patient-specific risk factors. To address this gap, the A + STRA score (ACL + slope tracing risk-factor al

Current development in surgical techniques, graft selection and additional procedures for anterior cruciate ligament injury: a path towards anatomic restoration and improved clinical outcomes-a narrative review.

BACKGROUND AND OBJECTIVE: Anterior cruciate ligament (ACL) reconstruction has been widely used for ACL injury for a long time. However, residual rotational instability and osteoarthritic changes after ACL reconstruction have been identified as problems. Thus, anatomic reconstruction techniques, various types of grafts and additional procedures have been desired to improve the clinical outcomes and knee instability. Although clinical outcomes and knee stability are better than in the past, ipsilateral graft failures still occur in 4-17% and osteoarthritic changes are seen in about 20% of patien

"Functional outcomes and MRI-based tendon healing after (antero-) superior rotator cuff repair among patients under 50 years: retrospective analysis of traumatic versus non-traumatic rotator cuff tears".

BACKGROUND: Rotator cuff tears among patients under 50 years either result from an adequate trauma or are considered non-traumatic due to work-related or athletic overuse. The impact of these different mechanisms on postoperative functional outcomes and tendon healing has not yet been fully understood. Therefore, it was the purpose of this study to investigate the influence of etiology of (antero-)superior rotator cuff tears on postoperative outcomes and the healing rates after arthroscopic rotator cuff repair in a young patient population. METHODS: Patients under 50 years who underwent arthr
